BACKGROUND Guidelines recommend nonstatin lipid-lowering agents in patients at very high risk for major adverse cardiovascular events (MACE) if low-density lipoprotein cholesterol (LDL-C) remains ≥70 mg/dL on maximum tolerated statin treatment. It is uncertain if this approach benefits patients with LDL-C near 70 mg/dL. Lipoprotein(a) levels may influence residual risk. OBJECTIVES In a post hoc analysis of the ODYSSEY Outcomes (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial, the authors evaluated the benefit of adding the proprotein subtilisin/kexin type 9 inhibitor alirocumab to optimized statin treatment in patients with LDL-C levels near 70 mg/dL. Effects were evaluated according to concurrent lipoprotein(a) levels. METHODS ODYSSEY Outcomes compared alirocumab with placebo in 18,924 patients with recent acute coronary syndromes receiving optimized statin treatment. In 4,351 patients (23.0%), screening or randomization LDL-C was <70 mg/dL (median 69.4 mg/dL; interquartile range: 64.3–74.0 mg/dL); in 14,573 patients (77.0%), both determinations were ≥70 mg/dL (median 94.0 mg/dL; interquartile range: 83.2–111.0 mg/dL). RESULTS In the lower LDL-C subgroup, MACE rates were 4.2 and 3.1 per 100 patient-years among placebo-treated patients with baseline lipoprotein(a) greater than or less than or equal to the median (13.7 mg/dL). Corresponding adjusted treatment hazard ratios were 0.68 (95% confidence interval [Cl]: 0.52–0.90) and 1.11 (95% Cl: 0.83–1.49), with treatment-lipoprotein(a) interaction on MACE ( P interaction = 0.017). In the higher LDL-C subgroup, MACE rates were 4.7 and 3.8 per 100 patient-years among placebo-treated patients with lipoprotein(a) >13.7 mg/dL or ≤13.7 mg/dL; corresponding adjusted treatment hazard ratios were 0.82 (95% Cl: 0.72–0.92) and 0.89 (95% Cl: 0.75–1.06), with P interaction = 0.43. CONCLUSIONS In patients with recent acute coronary syndromes and LDL-C near 70 mg/dL on optimized statin therapy, proprotein subtilisin/kexin type 9 inhibition provides incremental clinical benefit only when lipoprotein(a) concentration is at least mildly elevated. Egészségügyi Informatikai Fejlesztő és Továbbképző Intézet, BudapestBevezetés: A heveny szívizominfraktust elszenvedett betegek halálozására vonatkozó hazai adatok hiányosak. Célki-tűzés: A szerzők a Magyar Infarctus Regiszterben szereplő 8582 infarktusos beteg (4981 ST-elevációval járó myocardialis infarctus) adatainak elemzésével a kórházi, a 30 napos és az egyéves halálozás vizsgálatát tűzték ki célul. Budapest öt kerületében rögzítették a prehospitális haláleseteket. Módszer: A halálozás kockázati tényezőinek vizsgálatára logisztikus regressziós analízist végeztek, majd ellenőrizték a modell illeszkedését. Eredmények: A kórházi, a 30 napos, illetve az egyéves halálozás az ST-elevációval járó infarktusos betegcsoportban 3,7%, 9,5%, illetve 16,5%, a nem ST-elevációs betegcsoportban 4%, 9,8% és 21,7% volt. A nem ST-elevációval járó infarktus miatt kezelt betegek egyéves halálozása szignifi kánsan magasabb volt. A halálozás kockázati tényezőit vizsgálva az életkor, a Killip-stá-dium, a kórelőzményben szereplő myocardialis infarctus, stroke, valamint a diabetes bizonyult prognosztikus jelentőségűnek. A percutan coronariaintervenció mindkét típusú infarktusban nagyon jelentősen javította a betegek rövid és hosszú távú életkilátásait. Következtetések: A prehospitális halálozás igen jelentős volt, a 30 napon belül bekövet-kező események 72,5%-a kórházon kívül történt. Orv. Hetil., 2013, 154, 1297 Short and long term prognosis of patients with myocardial infarction Hungarian Myocardial Infarction RegistryIntroduction: Mortality data of patients with acute myocardial infarction are incomplete in Hungary. Aim: The aim of the authors was to analyse the data of 8582 myocardial infarction patients (4981 with ST-elevation myocardial infarction) registered in the Hungarian Myocardial Infarction Register in order to defi ne the hospital, 30-day, and 1-year mortality. To evaluate the prehospital mortality of myocardial infarction, all myocardial infarction and sudden death were registered in fi ve districts of Budapest. Method: Multivariate logistic regression was performed to defi ne risk factors of mortality and the model were assessed using c statistics. Results: The hospital, 30-day and 1-year mortality of patients with ST elevation myocardial infarction were 3.7%, 9.5% and 16.5%, respectively. In patients without ST elevation myocardial infarction these fi gures were 4%, 9.8% and 21.7%, respectively. The 1-year mortality of patients without ST elevation was higher than those of with ST elevation and the difference was statistically signifi cant. Age, Killip class, diabetes mellitus, history of stroke and myocardial infarction were independent predictors of death. Coronary intervention improved the prognosis of patients with myocardial infarction signifi cantly. A szívelégtelenség az elmúlt évtizedek jelentős terápiás fejlődése ellenére is rossz prognózisú és különösen a nagyszá-mú kórházi felvétel miatt igen magas költségigényű kórkép. Mindezek miatt a magas szakmai színvonalú ellátás alapvető érdeke a betegeknek, az ellátóknak és a finanszírozóknak egyaránt. Egy adott kórkép vonatkozásában az ellátási színvonal értékelésének legjobb módszerét a betegségspecifikus regiszterek jelentik. Mind ez ideig Magyarországon a szívelégtelenségben szenvedő betegek jellemzőit, ellátását értékelő regiszter nem volt. E hiány pótlására hozta létre a Magyar Kardiológusok Társasága a Magyar Szívelégtelenség Regisztert. Jelen közlemény célja a regiszter céljainak, módszertanának, működésének és első éves eredményeinek bemutatása. A regiszter célja egy korszerű, internetalapú adatbázis kialakítása, ami nagyszámú, aktuálisan vagy korábban szívelégtelenség miatt kórházi felvételre került, illetve aktuálisan vagy korábban súlyos szívelégtelenség (NYHA III-IV.) miatt ambuláns ellátásban részesült beteg adatait összegzi. A regiszter kialakításában jelenleg 17 kardiológiai osztály vesz részt. A tervezett betegszám 2000. A betegeket első lépésben egy évig tervezzük követni (pilot vizsgálat), majd ezt követően, a megfelelő tapasztalatok értékelése után, hosszú távú követést tervezünk. A regiszterben adatokat gyűjtünk a szívelégtelenség típusára (csökkent -LVEF≤45% -vs. megtartott ejekciós -LVEF>45% -frakciójú szívelégtelenség), etiológiájára, a komorbiditásokra, a diagnózishoz felhasznált vizsgálatokra, a kezelés során alkalmazott terápiás módszerekre, valamint a hospitalizációra és a mortalitásra vonatkozóan. Az első év során a regiszterbe bevont 698 beteg kiindulási adatait értékelve azt láttuk, hogy a betegek többsége (87,8%) csökkent ejekciós frakciójú szívelégtelenségben szenved, 39,8%-ban a szívelégtelenség hátterében coronariabetegség áll, a társbetegségek közül leggyakoribb a hypertonia, ezt követik a diabetes mellitus, a veseelégtelenség és a COPD. A betegek 94,4%-a kapott ACE-gátlót vagy angiotenzinreceptor-blok kolót, 95,9%-a béta-receptor-blokkolót és 73,9%-a mineralokortikoidreceptor-antagonistát.
