Background:Labor is a naturally occurring physiological process associated with uterine contractions, effacement, dilatation of cervix and descent of presenting part. Drotaverine hydrochloride is a non-anticholinergic isoquinoline derivative which acts by elevating intracellular cyclic Adenosine Mono Phosphate (cAMP) and cyclic Guanosine Mono Phosphate (cGMP) promoting smooth muscle relaxation. Materials And Method: This was a hospital based prospective study on effectiveness of Drotaverine Hydrochloride on enhancing dilatation of cervix and acceleration of active phase of labor. The sample size was 100. Fifty cases of women in active phase of labor received injection drotaverine hydrochloride 40 mg (group i) and fifty cases of women did not receive any drug (group ii) among those at term with singleton pregnancy and vertex presentation. Variables like maternal age, interval between administration of drug and delivery, mode of delivery, apgar score at 5 minutes, NCU (Neonatal Care Unit) admission and neonatal outcomes were recorded. Data analysis was done with the help of SPSS program. Results: The mean interval between drug adminstration to delivery in primipara and multipara in group i was 3.05 hours and 2.31 hours while in group ii was 4.5 hours and 3.75 hours respectively. The mean interval between drug adminstration and delivery was shorter in both groups of multipara. In group i, 96% had normal delivery and 4 % had vaccum delivery and in group ii 90% and 10% had normal delivery and vacuum delivery respectively. None of the participants had caeseraian section. There were no perinatal mortalities. Conclusion: The administration of drug Drotaverine Hydrochloride is effective in shortening duration of labor with favorable feto-maternal outcome.
Background: Obesity, a chronic disease that is increasing in prevalence in adults, adolescents and children, is now considered a global epidemic. Thyroid dysfunction contributes to the pathogenesis of obesity. Many clinical studies raise the questions of whether thyroid-stimulating hormone (TSH) changes in physiological limits is associated with obesity and whether there is a link between adipose tissue and hypothalamo-thyroidal axis. Materials and Method: This was a cross-sectional study. All clinically euthyroid patients and healthy volunteer adults of age 18 to 60 years of either gender were included in the study. Fasting blood sample was taken for thyroid function evaluation, which included Free T3, Free T4 and thyroid stimulating hormone. Height, weight, waist circumference and hip circumference were measured. The results were compared with calculated Body Mass Index (BMI). Results: 61 patients who met the inclusion criteria were studied. Among 61 patients 16 had subclinical hypothyroidism, 2 patients had hypothyroidism and 43 were euthyroid. Similarly, 2 underweight patients were observed, 7 had normal weight, 13 were over weight and 39 were obese. The mean TSH according to BMI were 3.8, 4.04, 3.88 and 6.19 respectively. Conclusion: The result in this study showed that the mean TSH increased as BMI increased with significant relationship between serum TSH and BMI (p <0.001). Thus thyroid dysfunction mainly subclinical hypothyroidism and hypothyroidism could be found in association with increased body weight.
Background: Urethral discharge is a symptom of urethritis. Common organisms causing urethral discharge are Neisseria gonorrhea and Chlamydia trachomatis. Cephalosporines, Macrolides and Tetracycline groups of drugs are commonly used for the treatment. Materials and Methods: This was a prospective, cross sectional, observational study. The patients were enrolled from the Sexually Transmitted Infections clinic and also from the hospital laboratory in between the period of one year. All the male cases with urethral discharge at STI clinic in between this period were included for clinical examination and laboratory testing. All urine and discharge specimens of urethral discharge patients as referred from other departments directly to the hospital laboratory for relevant tests were also included for the study. Results: Neisseria gonorrhea was the most common agent of urethral discharge in male patients. Urethral discharge, dysuria and inguinal lymphadenopathy were common symptoms. All the Neisseria gonorrhea isolates were sensitive to Ceftriaxone and Imipenam. Conclusion: Continuous monitoring with culture sensitivity testing of urethral discharge cases is required.
Introduction: The risk of active Tuberculosis increases after HIV seroconversion. It progressively increases with declining immunity. Tuberculosis (TB) epidemics in HIV has led to a dramatic upsurge in global TB incidence, resulting in remarkable increase in morbidity and mortality. Many clinical studies had shown that TB is one of the commonest opportunistic infection. This study is conducted to see the incidence of tuberculosis as an opportunistic infection in patients with HIV getting combined antiretroviral therapy (cART) Materials and Methods: This was a hospital based cross-sectional comparative study. Five hundred patients with HIV who are getting cART were enrolled in the study. Results: Analysis of individuals with HIV in our study amongst the 500 patients 266 (53.2%) were males and the remaining 234 (46.68%) were females. Amongst these HIV patients 48(18.04%) males and 17(7.26%) females had tuberculosis. 65(13%) of patients were TB/HIV co-infected. 2(4.1%) of these male TB/HIV patients had cervical gland TB. Conclusion: The incidence of tuberculosis in HIV infected individuals is high. TB is the commonest opportunistic infection (OIs) in people living with HIV/AIDS. Males are predominant gender to have TB/HIV co-infection.
Background: Dermatophytoses have a high prevalence worldwide and also in Nepal. Large scale studies with different antifungal agents for long fol¬low up periods to assess the long term outcome of treatments together with culture sensitivity testing are required to confirm the elimination of the fungi. Materials and Methods: Total of 97 patients were enrolled, 48 patients received treatment with terbinafine 1% cream for 2 weeks and 49 patients received clotrimazole 1% cream for 4 weeks. Clinical evaluation and microscopic examination of the potassium hydroxide (KOH) mount were done at presentation, at week 2 and at week 4. Clinical assessment scoring (CAS) and Mycological cure rate were compared between the study groups. Results: Terbinafine cured more cases of dermatophytoses compared to clotrimazole. The clinical cure rate was not statistically different among cases of tinea pedis. Terbinafine was better among cases of tinea cruris and tinea corporis only at the end of week 4. Mycological cure rate was higher for terbinafine both at the end of week 2 and week 4.
