O cisto dentígero, também conhecido como cisto folicular, origina-se a partir do acúmulo de líquido entre o epitélio reduzido do órgão do esmalte e a coroa dentária, o que resulta na separação do folículo que circunda a coroa de um dente não erupcionado. Geralmente são radiolúcidos e uniloculares. O objetivo do trabalho foi relatar um caso de cisto dentígero associado a um terceiro molar inferior impactado provocando reabsorção radicular externa do primeiro molar inferior. Paciente de 28 anos, sexo feminino, compareceu a clínica de ensino com queixa principal de “meu siso está incomodando” há cerca de 6 meses e sensibilidade no primeiro molar inferior esquerdo. Foram realizadas coleta de Fibrina Rica em Plaquetas e Leucócitos (L-PRF), exodontia do primeiro molar inferior e terceiro molar inferior esquerdo, enucleação do cisto como biópsia e a obtenção do resultado compatível com cisto dentígero. O diagnóstico e resultado obtidos nesse caso foram positivos. Os exames radiográficos e histopatológico foram realizados para complementar o diagnóstico. O histopatológico deverá ser realizado, pois o cisto dentígero é semelhante a outras lesões como o tumor odontogênico queratocístico e o ameloblastoma unicístico. As Fibrinas Ricas em Plaquetas e Leucócitos são inseridas formando um tampão no tecido lesado e são responsáveis pelo desencadeamento das próximas etapas da regeneração tecidual. As técnicas empregadas para tratamento do cisto dentígero são a enucleação e a marsupialização e em 85% dos casos a escolhida é a enucleação e a extração do dente associado. O cirurgião-dentista deve estar preparado para fazer o diagnóstico preciso frente ao cisto dentígero e realizar o tratamento adequado ou o encaminhamento a um especialista, quando for necessário.
