In 2012, the Baby-friendly Hospital Initiative for Neonatal Wards (Neo-BFHI) began providing recommendations to improve breastfeeding support for preterm and ill infants. This cross-sectional survey aimed to measure compliance on a global level with the Neo-BFHI's expanded Ten Steps to successful breastfeeding and three Guiding Principles in neonatal wards. In 2017, the Neo-BFHI Self-Assessment questionnaire was used in 15 languages to collect data from neonatal wards of all levels of care. Answers were summarized into compliance scores ranging from 0 to 100 at the ward, country, and international levels. A total of 917 neonatal wards from 36 low-, middle-, and high-income countries from all continents participated. The median international overall score was 77, and median country overall scores ranged from 52 to 91. Guiding Principle 1 (respect for mothers), Step 5 (breastfeeding initiation and support), and Step 6 (human milk use) had the highest scores, 100, 88, and 88, respectively. Step 3 (antenatal information) and Step 7 (rooming-in) had the lowest scores, 63 and 67, respectively. High-income countries had significantly higher scores for Guiding Principles 2 (family-centered care), Step 4 (skin-to-skin contact), and Step 5. Neonatal wards in hospitals ever-designated Baby-friendly had significantly higher scores than those never designated. Sixty percent of managers stated they would like to obtain Neo-BFHI designation. Currently, Neo-BFHI recommendations are partly implemented in many countries. The high number of participating wards indicates international readiness to expand Baby-friendly standards to neonatal settings. Hospitals and governments should increase their efforts to better support breastfeeding in neonatal wards.
OBJETIVOS: avaliar a prevalência de obesidade global e obesidade abdominal, em mulheres pós-menopausa, segundo o grau de instrução, nível de atividade física, uso de terapia hormonal na menopausa (THM) e paridade. MÉTODOS: foram entrevistadas 157 mulheres na pós-menopausa, atendidas em dois ambulatórios públicos da cidade de São Paulo, São Paulo, Brasil. A obesidade foi determinada segundo o índice de massa corporal (IMC > 30,0) e segundo o percentual de gordura corporal (%GC > 37%). A obesidade abdominal foi determinada pela relação cintura-quadril (RCQ > 0,85). RESULTADOS: a prevalência de obesidade global foi 34,4% (segundo o IMC) e de 40,1% (segundo o %GC). A prevalência de obesidade abdominal foi de 73,8%. Grande parte das entrevistadas referiu até sete anos de instrução formal (47,8%), foi considerada sedentária ou insuficientemente ativa (52,3%) e nunca tinha utilizado THM oral ou tinha utilizado por menos de 12 meses (72,0%). Foi constatada maior prevalência de obesidade global no grupo de mulheres sedentárias ou insuficientemente ativas e no grupo de não usuárias de THM (p<0,05). Quanto à obesidade abdominal, fator de risco para doenças cardiovasculares (DCV), apenas o grau de instrução mostrou-se associado à RCQ (p<0,05). CONCLUSÕES: mulheres após a menopausa, fisicamente ativas e usuárias de THM exibiram menor prevalência de obesidade global, porém a prevalência de obesidade abdominal foi alta e ainda maior nas mulheres com menor grau de instrução.
Resumo Para verificar a associação entre obesidade e variáveis demográficas, clínicas e relacionadas ao estilo de vida em mulheres no climatério, foi realizado um estudo transversal com 469 mulheres de 40 a 65 anos em dois ambulatórios públicos da cidade de São Paulo. As variáveis dependentes foram: obesidade, segundo índice de massa corporal (IMC) e obesidade, segundo percentual de gordura corporal (%GC). A variável explanatória principal foi: fase do climatério (pré ou pós-menopausa); e as variáveis de controle foram: idade; anos de estudo; paridade; uso de terapia hormonal da menopausa; prática de atividade física e hábito de fumar. Realizou-se análise de regressão “glm” múltipla, utilizando para as análises o software Stata 9.2. Segundo o IMC, a obesidade associou-se positivamente, à paridade (RP = 1,62; IC 95% = 1,11-2,37) e, negativamente, aos anos de estudo (RP = 0,71; IC 95% = 0,55-0,91) e à prática de atividade física (RP = 0,45; IC 95% = 0,33-0,61). De acordo com o %GC, a obesidade associou-se positivamente à paridade (RP = 1,60; IC 95% = 1,03-2,49) e, negativamente, à prática de atividade física (RP = 0,43; IC 95% = 0,29-0,63). Enquanto a prática de atividade física foi um fator protetor, a multiparidade constituiu-se como fator de risco para a prevalência de obesidade no grupo de mulheres deste estudo.

A quantitative and cross sectional study was conducted on a non-probability sample consisting of 135 recent mothers. The enquiry form applied allowed the assessment of parental skills on breastfeeding and in determining the conditions for the development of these skills. Most women (63%) provided information on the benefits of breastfeeding, 75% of the mothers did not show familiarity with strategies on how to continue breastfeeding. The study pointed to the existence of practices that impair the progress of acknowledgement on breastfeeding and contribute to early withdrawal, such as: delay in breastfeeding initiation, early introduction to infant formula, lack of nurses' guidance and support on breastfeeding, during the post-partum period. Clinical supervision can be a means for the development of nursing skills allowing a widespread improvement of practices and rates (exclusivity and duration) of breastfeeding, boosting parent's capacity, security, trust and satisfaction, which regards breastfeeding.Index Terms-Breastfeeding, clinical supervision, nursing care quality, parental skills. I. INTRODUCTIONThe practice of breastfeeding is an indicator of the quality of the child's and the mother's health and consequently of perinatal health care. Breastfeeding rates in Portugal are well below the recommended, in particular those concerning exclusive breastfeeding [1].Clinical supervision (CS) in nursing is a dynamic effort that maintains and develops the professional practice contributing to the safety and to the quality of care. In the present study, it is expected that the diagnosis of the situation and the proposal of a supervision model will contribute to the development of parental skills in breastfeeding.Clinical supervision is a dynamic, interactive, mediating, facilitating and experiential learning enhancer process, based on a relationship of trust and help among all stakeholders, where each one performs functions and establishes strategies [2], [3] in order to achieve a common purpose -the supervisor's personal and professional development. Reference [4] refers to it as an educational process, focusing on the development of skills, aiming organizational objectives. In this design, the clinical supervision is presented as a means to attain certain quality standards, optimize health indicators, increasingly demanded by institutions. Reference [5] states that the main purpose of clinical supervision is to increase the quality of care and through it supervisees achieve, continue and develop high quality practices. Reference [6] adds that supervision nowadays is one of the most important dimensions in promoting quality processes and accreditation, given the benefits it offers as far as assistance is concerned. Resumo: O Museu da Casa Brasileira promove anualmente exposição de design de curta duração atrelada ao Prêmio Design, originado em 1986. Contudo, identificou-se que as possibilidades materiais de tais realizações periódicas são pouco debatidas. Este artigo apresenta uma análise da mostra da 29ª edição do evento, ocorrida entre 26 de novembro de 2015 e 31 de janeiro de 2016. Tem como objetivo contemplar as estratégias expográficas em jogo de modo exploratório-descritivo.
