O objetivo do estudo é analisar as Comissões Locais de Saúde (CLS) do Sistema Único de Saúde de Belo Horizonte (SUS-BH) como espaços de ampliação da democracia à luz da Teoria Democrática Deliberativa. A pesquisa foi realizada no período de outubro/2013 a junho/2014 com análise de três CLS. Utilizou-se a técnica de observação participante com a adoção de um diário de campo. Como resultado do processo democrático, ainda que de maneira tímida e desorganizada, destaca-se o fluxo de informações horizontal, que através das discussões permite acompanhar e fiscalizar as políticas públicas, indo além do setor saúde, visando à redução das iniquidades sociais. Ao discutirem sobre temas que afetam suas vidas e decidirem por meio de acordos, as CLS revelam-se como espaços públicos verdadeiramente democráticos, de ampliação da democracia.
IntroductionDiabetes mellitus is one of the most common chronic diseases worldwide and its prevalence continues to increase. Adherence to good health behaviors provides better control of the disease. This study analyzed trends in the prevalence of diabetes among Brazilian adults between 2006 and 2016 and compared the frequency of key health behaviors between people with and without diabetes. MethodsWe analyzed data from 572,437 Brazilian adults interviewed between 2006 and 2016 by using the Surveillance System of Risk and Protection Factors for Chronic Diseases by Telephone Survey (Vigitel). We used regression models to investigate significant trends in the prevalence of diabetes and differences in adherence to health behaviors between people with and without diabetes.ResultsThe prevalence of diabetes increased significantly from 5.5% to 8.9% (P <.001), being higher among women, older adults, and those with less than a high school education. The greatest increase was observed among these groups with higher prevalence. People with diabetes had a lower frequency of risk behaviors and a higher frequency of protective behaviors when compared to people without diabetes. The greatest differences were observed in the consumption of soft drinks and sugar-sweetened beverages (9.5% vs 25.0%) and alcoholic beverages (9.0% vs 17.9%), and the smallest differences were related to regular consumption of fruits and vegetables (40.7% vs 34.0%) and meats with excess fat (24.3% vs 32.2%). People with diabetes reported less leisure-time physical activity (PRa, 0.92 [P < .001]) and less transport-related physical activity (PRa, 0.84 [P <.001]) than those without diabetes.ConclusionDiabetes prevalence increased between 2006 and 2016. People with diabetes had better dietary habits than those without diabetes, but still had risk behaviors such as insufficient physical activity.
Governmental institutions are employing artificial intelligence techniques to deal with their specific problems and exploit their huge amounts of both structured and unstructured information. In particular, natural language processing and machine learning techniques are being used to process citizen feedback. In this paper, we report on the use of such techniques for analyzing and classifying complaints, in the context of the Portuguese Economic and Food Safety Authority. Grounded in its operational process, we address three different classification problems: target economic activity, implied infraction severity level, and institutional competence. We show promising results obtained using feature-based approaches and traditional classifiers, with accuracy scores above 70%, and analyze the shortcomings of our current results and avenues for further improvement, taking into account the intended use of our classifiers in helping human officers to cope with thousands of yearly complaints.
É reconhecida a alta prevalência da deficiência de ferro na população, mas ainda não existem estudos suficientes que expliquem os motivos deste problema em nível local e nacional. Assim, o objetivo deste estudo foi avaliar a prevalência de anemia e sua relação com aspectos sociodemográficos e antropométricos de crianças residentes no município de Vitória, ES. Trata-se de um estudo transversal realizado com indivíduos de 12 a 72 meses, cujos dados foram coletados em Unidades Básicas de Saúde e envolveu a dosagem de hemoglobina, aferição de peso e estatura, além de aplicação, aos responsáveis legais, de questionário semiestruturado com informações sociodemográficas. Foram avaliadas 980 crianças, 51,1% do sexo feminino e mediana de idade de 46,00 (IC95%: 45,52-48,09) meses. Constatou-se 37% de anemia, sendo 11,4 (IC95%: 11,15-11,36) g/dL o valor mediano de hemoglobina. A estatura/idade da criança (p = 0,049) e o número de pessoas na família (p < 0,001) apresentaram-se como determinantes da anemia, e a idade como fator protetor desta carência (p = 0,010). A elevada prevalência de anemia e sua forte relação com dados sociodemográficos e antropométricos sugerem a necessidade de políticas públicas mais efetivas do que apenas a fortificação e a suplementação de alimentos.
From the comparison performed between the institutionalization of Servizio Sanitario Nazionale (SSN), in Italy, and the Unified Health System (SUS), in Brazil, both based on the philosophy of the right to health and under the aegis of the State, the article aims to analyze the reform implemented by SSN, which introduced the co-participation of the user in the cost of care, upon payment of the Ticket Sanitario. If on the one hand this reform aimed to tackle the fiscal crisis, on the other, it brought serious consequences to the life and health condition of the Italian population. Using also the methodology of comparison between health systems, the study draws attention to the adoption of reform proposals, now by the SUS. The proposals presented by the documents "Agenda Brasil" and "Uma ponte para o futuro", targeting the selective charge for care, decharacterizes SUS when institutionalizing differentiated entrance gateways. This way, as happens with the Italians, the fiscal crisis imposes, now, on the Brazilians, the disrespect to the right to health.
RESUMO O objetivo do estudo é analisar a dinâmica e a efetividade das Conferências Locais de Saúde (CLS), realizadas nas Unidades Básicas de Saúde. O estudo ocorreu durante a XII Conferência Municipal de Saúde em outubro de 2013. Foram utilizadas as técnicas de pesquisa de observação participante e análise documental. Verificou-se que as CLS são realizadas de portas abertas, não existindo um número predeterminado de participantes. Além disso, todos têm direito a voz e voto. O estudo conclui pela efetividade das CLS, pois possibilita a discussão das necessidades de saúde pelos usuários, trabalhadores e gestores na atenção primária, legitimando as decisões políticas, assegurando o direito à saúde.
