Apendicite aguda é causa comum de dor abdominal em crianças. O tratamento é cirúrgico e pode ser realizado por apendicectomia aberta (AA) ou laparoscópica (AL). Sabendo que a recomendação para pacientes pediátricos ainda não é bem estabelecida, esta revisão sistemática objetivou analisar ambos procedimentos, definindo as vantagens e desvantagens. METODOLOGIA: Foi realizada uma pesquisa nas bases de dados MedLine e Cochrane em agosto de 2020. Incluíram-se ensaios clínicos controlados e randomizados e ensaios clínicos, em inglês, publicados entre os anos 2000 e 2020, e excluíram-se estudos realizados em adultos e que não compararam as técnicas cirúrgicas AA e AL. Inicialmente encontraram-se 461 artigos, e após aplicação dos critérios de inclusão e exclusão e a leitura dos estudos, 10 foram selecionados. RESULTADOS: A AL demonstrou menor tempo de internação, menos complicações após a cirurgia, redução da dor pós-operatória e da necessidade de antibioticoterapia. Contudo, o tempo e o custo da operação foram menores na AA. Já o retorno às atividades normais e a resposta inflamatória não demonstraram diferenças significativas entre as duas abordagens. CONCLUSÃO: Cada paciente deve ser avaliado individualmente pelo cirurgião em relação à escolha da AL ou AA, analisando os benefícios e as limitações para cada caso.
