Objetivo: evaluar la asociación entre la imagen corporal y las conductas alimentarias de riesgo en pacientes con obesidad. Métodos: se realizó un estudio transversal analítico, mediante muestreo aleatorio simple en pacientes con obesidad, derechohabientes de la Unidad de Medicina Familiar (umf) No. 75 del Instituto Mexicano del Seguro Social (imss); previa firma de consentimiento informado. Se realizó estadística descriptiva y se determinaron frecuencias y porcentajes, la asociación entre imagen corporal y conductas alimentarias de riesgo se estimó mediante χ2. Resultados: se estudiaron 202 participantes con obesidad, de los cuales 68.8% eran mujeres y 32.2%, hombres, la media de edad fue de 40 años; de los que presentaron preocupación leve por la imagen corporal y las conductas alimentarias de riesgo, 37.9% presentó riesgo moderado (n=22) y 22.4%, riesgo alto (n=13); en cuanto a preocupación moderada, 26.9% representó riesgo moderado (n=7), 53.8%, riesgo alto (n=14); en relación con la preocupación extrema, 54.5% tuvo riesgo moderado (n=6) y 36.4%, riesgo alto (n=4). Se encontró una asociación estadísticamente significativa (p<0.0004) entre la imagen corporal y las conductas alimentarias de riesgo. Conclusión: la preocupación moderada a extrema por la imagen corporal se asoció a conductas alimentarias de riesgo en participantes con obesidad. Es necesario implementar programas en atención a las conductas alimentarias e identificar la percepción de la imagen corporal.
