Objetivo: Avaliar a associação da nutrição enteral precoce (NEP) com o alcance das cotas calórico-proteicas, tempo de internação, tempo de uso de ventilação mecânica e desfecho clínico, bem como, detectar os fatores causais para introdução tardia e interrupção da dieta. Métodos: Foi realizado um estudo prospectivo com crianças e adolescentes internados em unidade de terapia intensiva (UTI) e que tiveram a nutrição enteral como dieta na internação, no período de dezembro de 2015 a agosto de 2016, em um complexo hospitalar pediátrico de referência no estado da Paraíba, totalizando uma amostra de 54 pacientes. Resultados: A amostra foi homogênea quanto ao gênero, com predominância de lactentes (75,9%) e a média de internação na UTI foi de 14,4 ± 18,3 dias. A nutrição enteral (NE) foi iniciada precocemente em 57,4% dos pacientes e a NEP influenciou positivamente no alcance mais rápido das cotas calórica e proteica recomendadas para o quadro clínico (p=0,01 e p=0,04, respectivamente) e em menor tempo de uso de ventilação mecânica (p=0,04). Não foi observada, entretanto, associação da NEP com a interrupção da dieta (p=0,38), tempo de internação na UTI (p=0,06) e desfecho clínico (p=0,50). O principal fator causal para início tardio da NE e para interrupção da dieta foi a presença de resíduo gástrico (43,5% e 38,5%, respectivamente). Conclusão: Os achados apresentados neste estudo evidenciam a necessidade de um adequado monitoramento, por parte dos profissionais de saúde, da introdução do suporte nutricional adequado, em tempo hábil, das crianças e adolescentes admitidos em UTI pediátrica.
