This four-experiment series sought to evaluate the potential of children with neurosensory deafness and cochlear implants to exhibit auditory-visual and visual-visual stimulus equivalence relations within a matching-to-sample format. Twelve children who became deaf prior to acquiring language (prelingual) and four who became deaf afterwards (postlingual) were studied. All children learned auditory-visual conditional discriminations and nearly all showed emergent equivalence relations. Naming tests, conducted with a subset of the children, showed no consistent relationship to the equivalence-test outcomes. This study makes several contributions to the literature on stimulus equivalence. First, it demonstrates that both pre- and postlingually deaf children can acquire auditory-visual equivalence relations after cochlear implantation, thus demonstrating symbolic functioning. Second, it directs attention to a population that may be especially interesting for researchers seeking to analyze the relationship between speaker and listener repertoires. Third, it demonstrates the feasibility of conducting experimental studies of stimulus control processes within the limitations of a hospital, which these children must visit routinely for the maintenance of their cochlear implants.
ResumoEste estudo investigou os efeitos da formação de relações de equivalência entre palavras faladas, palavras impressas e fi guras sobre a correspondência na fala de crianças com implante coclear em tarefas de nomeação de fi guras e de leitura oral. Antes do estudo as crianças pronunciavam melhor as palavras quando liam do que quando nomeavam as fi guras. Foram ensinados dois conjuntos de três relações entre palavras ditadas e as fi guras correspondentes, entre as mesmas palavras ditadas e palavras impressas e entre sílabas ditadas e impressas. Sondas periódicas de nomeação das seis fi guras e de leitura das palavras impressas, planejadas de acordo com um delineamento de linha de base múltipla entre conjuntos, eram realizadas antes e depois do ensino de cada tipo de relação condicional e do teste de formação de classes de equivalência de cada conjunto. As vocalizações na nomeação de fi guras melhoraram progressivamente para cinco dos seis participantes ao longo do ensino do Conjunto 1 e para todos os participantes depois do ensino do Conjunto 2. Na leitura, as vocalizações também fi caram mais precisas. Os resultados estendem descobertas prévias sobre o potencial de procedimentos baseados no paradigma de equivalência de estímulos para a reabilitação da fala e da compreensão auditiva de indivíduos com implante coclear. AbstractThe present study investigated the effects of strengthening equivalence relations between spoken words, printed words, and pictures to improve the correspondence between vocalizations in picture naming and in reading tasks in children with cochlear implant. Before the study, the children pronounced words better while reading than while naming pictures. The procedure taught two sets of three relations between dictated words and pictures, between the same dictated words and printed words, and between dictated and printed syllables. Periodic probes evaluated the experimental effects according to a multiple baseline design between stimulus sets. Picture naming and reading of all six words were probed before and after teaching each type of relationship and testing for class formation for each stimulus set. Picture naming progressively improved for fi ve of six participants across teaching and probes with Stimulus Set 1, and for all participants across Stimulus Set 2. Reading scores, initially high, also reached perfect or near perfect accuracy. The results extend previous fi ndings on the potential of procedures based on the stimulus equivalence paradigm for improving speech and listening comprehension of children with cochlear implant.
Resumo Considerando a reabilitação da fala de crianças com defi ciência auditiva e usuárias de implante coclear e evidências da possibilidade de transferência de controle de estímulos da leitura para a nomeação, este estudo teve como objetivo avaliar os efeitos de um programa de ensino de leitura de palavras simples sobre a qualidade da fala de duas crianças em testes de leitura de palavras e nomeação de fi guras. O procedimento consistiu no ensino de relações condicionais baseadas em seleção de palavras impressas e de composição após a palavra ditada. A nomeação de fi guras foi avaliada em um delineamento de linhas de base múltipla. Os resultados mostram melhora na qualidade da fala, se comparados à linha de base, tanto frente a palavras quanto frente a fi guras. Futuros estudos, com maior número de participantes e outros delineamentos devem verifi car benefícios de programas de ensino ao processo de reabilitação dessa população. Children who use cochlear implants (CI) and who are readers usually produce more accurate speech in response to text than to pictures. Equivalence-based instruction (EBI) can be a route to establish functional interdependence between these verbal operants. The present study investigated whether children with CI who read would improve speech accuracy when tacting pictures of scenes after EBI that included dictated sentences, pictures of scenes, and printed sentences. This study evaluated whether teaching verbal relations to diagonal sentences from a matrix with subject-verb-object combinations promoted recombinative generalization to untrained sentences. Participants were three children with CI with a more accurate speech when reading print than when tacting pictures of scenes. They were taught to select pictures of scenes in response to dictated sentences (AB) by matching-to-sample (MTS) and to construct printed sentences in response to dictated sentences (AE) by constructed-response-matching-to-sample (CRMTS). Speech production in response to print (CD) and in response to pictures of scenes (BD) were probed for both trained and untrained sentences, using a multiple baseline design across participants. All participants learned the trained relations, showed emergence of derived relations, and improved speech accuracy when tacting pictures of scenes. They were able to recombine sentence components and tact novel pictures using untrained sentences from the matrix. These results indicate that speech accuracy and generative sentence production can be improved in children with CI from interventions that incorporate EBI and matrix training.
Os efeitos de uma intervenção em habilidades sociais foram avaliados em quatro universitários na faixa etária entre 19 e 31 anos, com diagnóstico de fobia social. O desempenho em habilidades sociais assim como desempenho compatível com o transtorno foi avaliado antes, após a intervenção, e em seguimento, quatro meses depois, pelos instrumentos: SCID-I, Mini-SPIN, QACC-VU e IHS em um delineamento de linha de base múltipla entre participantes. A intervenção programou 12 encontros semanais nos quais a pesquisadora discutia alternativas socialmente habilidosas para as dificuldades descritas, conduzia exposições teóricas dialogadas sobre o tema do encontro, bem como realizava avaliações funcionais e treino de repertório dos participantes. Observou-se déficits tais como falar em público, iniciar e manter conversação, no repertório de habilidades sociais dos participantes, coerentes com os critérios de fobia social e dificuldades na adaptação à universidade. Após a intervenção os participantes deixaram de apresentar dificuldade de falar em público, característica de desempenho compatível com o diagnóstico de fobia social, e ampliaram o repertório social, embora ainda haja a necessidade de aprimorar outras habilidades, como, por exemplo, expressar sentimento positivo.
RESUMOEste estudo buscou verificar o efeito do procedimento de Instrução com Múltiplos Exemplares (MEI) sobre habilidades verbais de falante e de ouvinte em três crianças de 8 a 12 anos com deficiência auditiva pré-lingual e implante coclear, utilizando três conjuntos de estímulos não-convencionados. Os repertórios-alvo do ensino foram: ecoico, discriminação auditivo-visual e tato. A Fase 1 consistiu na avaliação inicial dos repertórios de falante (tato e textual) e ouvinte (discriminação condicional auditivovisual). A Fase 2 estabeleceu treino de ouvinte encadeado com palavra ditada pela experimentadora aos estímulos visuais apresentados como modelo e com exigência de resposta ecoica dos participantes, além do treino de falante (tato) com o Conjunto de Estímulos 1. Na Fase 3 foram realizados treino de ouvinte e de falante utilizando-se o procedimento de MEI com o Conjunto de Estímulos 2. A Fase 4 consistiu em avaliação da integração dos repertórios após o MEI. A Fase 5 consistiu na reaplicação da Fase 3 com o Conjunto de Estímulos 3. Em seguida, os testes da Fase 1 foram reaplicados. Os participantes mostraram aquisição das discriminações auditivo visuais (ouvinte) e aumentos sistemáticos nos repertórios de ecoico e tato (falante) com estímulos específicos nas fases de ensino. No pós-teste mantiveram 100% de ecoicos corretos, e a porcentagem de tatos aumentou em relação ao pré-teste, mas ocorreram variações individuais (respectivamente 44,4, 55,5, e 77,7 % para cada um dos três participantes). ABSTRACTThis study aimed at verifying the effects of the Multiple Exemplar Instruction (MEI) procedures on verbal skills of speakers and listeners in three children from 8 to 12 years old with pre-lingual hearing impairment and cochlear implant by using three sets of non-conventional stimuli. The target-repertoires of the study were: echoic, auditory-visual discrimination, and tact. Phase 1 was the initial repertoire assessment of the speakers (tact and textual) and listeners (auditory-visual conditional discrimination). Phase 2 established the listener training chained by the experimenter's dictated word to the visual stimulus used as sample and the demand of echoic response from the participants, in addition to the speaker training (tact) with the Stimuli Set 1. In Phase 3 were performed the training of the listener and the speaker using the MEI procedure with the Stimulus Set 1. Phase 4 consisted in the assessment of repertoire's integration after the MEI. Auditory Neuropathy Spectrum Disorder (ANSD) is characterized by a difficulty in establishing verbal behavior in the speaker and listener modalities due to an asynchrony in the nervous conduction of sound stimuli, leading to hearing loss. Cochlear implants are an alternative that stabilizes sound detection; however, discriminating and recognizing what is heard and establishing relations to speech requires learning. A procedure with recognized efficacy in establishing verbal repertoires in children with little or no language is Multiple Exemplar Instruction (MEI). The objective of this study was to verify replicability of results obtained from MEI with other populations for teaching and integrating listener and speaker behaviors on a six year-old child with ANSD and bilateral cochlear implants. The study was conducted with three sets of stimuli and consisted in a rotated presentation of listener responses, based on figure selection from dictated words, and of speaker responses, after a dictated word (echoic) and figure naming (tact). Repeated probes evaluated performance on the set of stimuli. Even though there is some variability in responding, selection responses were established first and speaker responses occurred gradually throughout successive sessions. The conditions under which a speaker's repertoire may be refined, and generalization of these results to other participants with ANSD should be further investigated. O implante coclear tem sido uma alternativa que estabiliza a detecção sonora, porém, discriminar e reconhecer o que se ouve e estabelecer relações com a fala irá requerer aprendizagem. Um procedimento de reconhecida eficácia no estabelecimento de repertórios verbais em crianças com pouca ou nenhuma linguagem é o Multiple Exemplar Instruction (MEI). O objetivo deste estudo foi verificar a replicabilidade dos resultados obtidos com o MEI com outras populações no ensino e integração entre os repertórios de ouvir e de falar em uma criança de seis anos com DENA e implante coclear bilateral. O ensino foi conduzido com três conjuntos de estímulos, e consistiu na apresentação rotativa de respostas de ouvinte, baseadas na seleção de figuras mediante palavra ditada e, de falante, após uma palavra ditada (ecoico) e mediante a nomeação de uma figura (tato). Repetidas sondas avaliaram o desempenho nos conjuntos de estímulos. Ainda que com alguma variabilidade no responder, as respostas de seleção foram estabelecidas primeiro e as respostas de falante, gradativamente, ao longo de sucessivas sessões. As condições sob as quais o repertório de falante pode ser refinado e a generalização dos resultados para outros participantes com DENA devem ser investigadas.
Children with impaired hearing and cochlear implants produce better speech intelligibility when it is under control of textual stimuli rather than pictures. When teaching promotes equivalence relations between stimuli, it may also promote transfer of stimulus control of vocalization responses from text to pictures, with improvement in picture naming (tact) performance. This study verified the effects of a reading curriculum based on the equivalence relations paradigm on the speech intelligibility of a cochlear implanted child through systematic picture naming probes during intervention. Results showed improvement in speech intelligibility during picture naming trials after exposure to the curriculum, with a higher percentage of correct responses.
