Este estudo descreveu a linguagem utilizada no dia-a-dia por enfermeiros em unidade de terapia intensiva cardíaca. Apoia-se em uma abordagem qualitativa, descritiva, que utilizou a etnometodologia, envolvendo os conceitos de prática, indicialidade e noção de membro. Para a coleta de dados, foram realizadas observação participante e fotografia para captação da imagem gestual. Os resultados apontam a utilização de palavras científicas, siglas e abreviações, silêncio, gestos e palavras com significado distinto. Essa linguagem é totalmente entendida pelos membros do grupo e se apresenta de forma natural pelos atores sociais; no entanto, não é de domínio do cliente, podendo ocasionar falsas interpretações. Dessa forma, este estudo propõe uma reflexão sobre a importância de utilizar-se no cotidiano uma linguagem compreendida não só pelos profissionais, mas também pelo cliente, para que esta seja uma ferramenta do cuidar/cuidado de enfermagem.
Objectives:To estimate the prevalence of human papilloma virus in women with human immunodeficiency virus accompanied the referral center for sexually transmitted diseases in the city of Caxias (MA). Methodology: study performed in documentary form, descriptive, exploratory, retrospective study with a quantitative approach. Data were collected during the months of December 2012 to March 2013. Thus, the medical records of patients as well as a pre-coded and pre-tested semi-structured form was used. For data analysis, we used descriptive and univariate statistics. Results: 180 women participated in this study, most of whom were married (80.00%). Regarding risk factors, 60% were smokers, 40% of drinkers, 100% heterosexual and 80% were not using contraception. As to diagnosis, only anoscopy examination was not performed. In the case of opportunistic infections other than human papillomavirus found other types of sexually transmitted diseases in 100% of the sample. Furthermore, 100% of the customers surveyed presented with cervical intraepithelial neoplasia. Conclusion: In view of the results, it is suggested that the women coinfection infected by the human papilloma virus and human immunodeficiency virus women are vulnerable to the develop ment of cervical cancer. This reinforces the need for strengthening the gynecological screening and greatly disclosure of specific monitoring protocol for this population. Descriptors: co-infection. Cancer of the uterus. RESUMO Objetivos: Estimar a prevalência do vírus papiloma humano em mulheres com o vírus da imunodeficiência humana acompanhadas no centro de referência em doenças sexualmente transmissíveis no município de Caxias do Maranhão. Metodologia: estudo desenvolvido de forma documental, descritivo-exploratório e retrospectivo com abordagem quantitativa. Os dados foram coletados durante os meses de dezembro de 2012 a março de 2013. Para tanto, foi utilizado o prontuário das pacientes como também um formulário semiestruturado pré-codificado e pré-testado. Para análise dos dados, utilizou-se estatística descritiva e univariada. Resultados: Participaram dessa pesquisa 180 mulheres, que em sua maioria eram casadas (80,00%). A respeito dos fatores de risco 60% eram tabagistas, 40% etilista, 100% heterossexual e 80% não usavam contracepção. Quanto ao diagnostico, apenas o exame de anoscopia não era realizado. Em se tratando das infecções oportunistas foram encontrados além do papiloma vírus humano outros tipos de doenças sexualmente transmissíveis em 100% da amostra. Além do mais, 100% das clientes investigadas apresentavam neoplasia intraepitelial cervical. Conclusão: Ante os resultados, sugere-se que as mulheres infectadas pela coinfecção do papiloma vírus humano e do vírus imunodeficiência humana são vulneráveis ao desenvolvimento do câncer de colo de útero. OBJETIVO: avaliar a resistência ao cisalhamento de braquetes metálicos (American Orthodontics) utilizados na técnica lingual, colados em facetas de cerâmica. MÉTODOS: foram utilizados 40 corpos de prova divididos em quatro grupos de 10, de acordo com o material de colagem e do preparo da porcelana: Grupo I -resina Sondhi Rapid-Set e ácido fluorídrico; Grupo II -resina Sondhi Rapid-Set e óxido de alumínio; Grupo III -resina Transbond XT e ácido fluorídrico; e Grupo IV -resina Transbond XT e óxido de alumínio. Previamente à colagem, os braquetes foram preparados com base de resina de carga pesada (Z-250) e as facetas de cerâmica receberam aplicação de silano. O teste de cisalhamento foi realizado por uma máquina de ensaios Kratos à uma velocidade de 0,5mm/min. RESULTADOS: os resultados obtidos foram analisados estatisticamente através do teste de Tukey (p<0,05) e demonstraram diferença significativa entre os grupos I (2,77MPa) e IV (6,00MPa), e entre os grupos III (3,33MPa) e IV. CONCLUSÕES: concluiu-se que a colagem de braquetes da técnica lingual em superfície de cerâmica foi mais resistente ao cisalhamento com o uso de óxido de alumínio associado às duas resinas utilizadas neste estudo, porém a resina Transbond XT apresentou maior resistência que a Sondhi Rapid-Set.
A 62 year old man arrived in PS for typical chest pain radiating to the jugular and associated with sweating for about three hours. History of arterial hypertension and untreated dyslipidemia, family history of CVD. ECG: sinus rithm at rate of 50 bpm with first degree AVB. Inferior ST elevation in inferior leads and ST depression in V2–V3. Echocardiogram: depressed systolic function (EF 48%) in relation to hypo–akinesia of the mid–basal segments of the inferior–posterior–lateral wall. Diagnosed with STEMI inferior–posterior, was sent to the cath lab to undergo to emergency coronary angiography, with findings of non–significant atheromas on IVA and Cx. Multiple unsuccessful attempts to search for the right coronary artery and failure to visualize the aortography in two complementary projections. During the procedure hemodynamic and electrical instability, worsening angor, marked hypotension and extreme bradycardia up to complete AVB with asystole phases. Inotropics and atropine were started, which proved ineffective, and it was therefore decided to proceed with the implantation of a temporary pacemaker. During the placement, the patient presented cardiac arrest from VF (likely an irritating effect from the passage of the lead through the tricuspid) treated with effective DC–shock. Given the technical difficulties in proceeding with mechanical revascularization and given the patient‘s instability, the decision was made to postpone PCI by attempting systemic thrombolysis. Early efficacy of thrombolysis with disappearance of precordial pain, improvement of echo motion and reduction > 50% of the ST–segment elevation to the rare spontaneous QRS displayed at the transient shutdown of the temporary PM. Subsequent emergence of AF at rate of 116 bpm. Approximately 12 hours after thrombolysis: complete ST resolution in absence of Q waves. AngioTC of the coronaries was performed which showed regularl origin of Cdx from the right coronary sinus but higher up and lateralized to the left, with severe lesion in the first segment. So the patient underwent falicitated PCI with detection of ostio–proximal Cdx thrombotic lesion effectively treated with PTCA and drug–eluting stent placement with good final angiographic result. Control echo: preserved global left ventricular motion (FE 55%), hypokinesia of the basal segment of the inferior wall. The remaining stay decurred without any complications and the patient was discharged with indications for a close follow–up.
