La amiotrofia monomiélica es un trastorno neurológico poco común que produce atrofia de una extremidad. La afección de la extremidad superior es más frecuente que la extremidad inferior, esta entidad clínica predomina en el sexo masculino. Se presenta caso clínico de Varón, 52 años de edad, referido para evaluación por debilidad y atrofia de mano derecha. Su historia comenzó cerca de un año antes, con paresia inducida por el frio, parestesias del ante brazo seguido de debilidad en su mano que progresó hacia su antebrazo y brazo derecho. El diagnostico luego de la exploración física más los exámenes de imágenes, confirman que hay un signo de afectación de la neurona motora inferior (monomélica), y como posibilidad lejana una afectación C7-C8-T1. No se encuentran signos de afectación al plexo. En base a la literatura consultada, se desprende que la Amiotrofia Monomiélica es una enfermedad de progreso lento, que afecta a hombres después de los 30 años, aseveración que va acorde con la edad del paciente, expuesto en el caso clínico presentado en esta investigación, así como los síntomas presentados en cuanto a la disminución de fuerza en las extremidades superiores, que le puede dar un carácter de degenerativa. Tiene muy pocas opciones de tratamiento, ya que no es una enfermedad que genere dolor, sin embargo, dentro de la literatura se indica como opción terapéutica la rehabilitación física de leve a moderada, existiendo todavía muchas ambigüedad y discusión en cuanto a su efectividad en la fuerza muscular.
Se presenta el caso de una paciente femenina 44 años de edad, con diagnóstico de rotura aneurismática de carótida derecha en relación con arteria comunicante posterior asociado a hipoplasia de arteria cerebral posterior ipsilateral. A inicios del mes de febrero del año en curso mientras dormía, en forma súbita presentó cefalea holocraeana acompañado de náuseas y vómito por varias ocasiones sin pérdida de la conciencia. En horas de la mañana ante la persistencia de su cefalea, que no había cedido al tratamiento analgésico, acudió a consulta particular; el estudio tomográfico cerebral demostró hemorragia subaracnoidea en las cisternas selar, Silviana derecha e interhemisférica anterior. En las siguientes tres horas se sometió a angiotac que demostró la presencia de aneurisma de la arteria carótida derecha en relación con la arteria comunicante posterior, el flujo sanguíneo de la arteria comunicante posterior proveniente de la carótida atravesaba el aneurisma; siendo el motivo de no ser tributario de cirugía temprana. En el trascurso la paciente se mantuvo orientada, sin déficit motor ni sensitivo y estable hemodinámicamente, decidiéndose tratamiento ulterior en 15 días con diversor de flujo a nivel de la arteria carótida, iniciando anti agregación con clopidogrel y ácido acetil salicílico (ASA). En condiciones estables egreso a su domicilio por el tiempo descrito para tratamiento definitivo. Trascurrido los 15 días, bajo anestesia general se procedió a la colocación del dispositivo Pipeline flex (PED) a nivel de arteria carótida derecha vía endovascular; no se presentó ninguna complicación y se concedió alta domiciliaria con control en seis meses. Al momento evolución satisfactoria sin ningún déficit.
Las afecciones coronarias son un tema común tratado por la sociedad, debido a que las funciones del corazón repercuten considerablemente en el funcionamiento del cuerpo humano. El corazón se monitorea desde el vientre materno hasta la longevidad del ser humano. La insuficiencia cardíaca (IC) es un síndrome clínico caracterizado por diferentes signos y síntomas típicos, causados por una reducción del gasto cardíaco o un aumento de las presiones intracardíacas, debido a anomalías tanto estructurales como funcionales en el sistema cardiovascular. En la práctica clínica, el reconocimiento de los casos clásicos suele ser sencillo, pero englobar todos los conceptos que abarca este síndrome en una definición es complicado. Antes de que se produzcan signos y síntomas comunes, los pacientes pueden presentar la anomalía cardíaca subyacente de forma asintomática. Recientemente, con la propagación pandémica del virus SARS-COV-2 y la enfermedad que ocasiona COVID-19 se centra la mirada a las personas con IC puesto que la ciencia demuestra una relación directa entre comorbilidades como ésta y la gravedad de la enfermedad por contagio del virus. Por tal motivo se ha de rescatar los últimos aportes científicos publicados en el tema a fin de impulsar y sintetizar los temas generales asociados al diagnóstico y tratamiento de la IC. Se trata de una investigación descriptiva, transversal, bibliográfica que concreta una revisión de las publicaciones científicas de documentos académicos de valor reconocido por la sociedad médica como PUBMed, Scielo, Elseiver y las bibliotecas virtuales direccionadas a través de Google Académico. La prevención y control de las IC de una población representa un aporte para la calidad de vida general de la población. En la historia contemporánea aun estamos en pleno desarrollo de las consecuencias cardiovasculares que ha dejado la pandemia por la COVID-19, todavía no hay consenso medico respecto a las consecuencias durante la enfermedad. Se insiste en la actualización y monitoreo de las distintas manifestaciones que engloban la IC para cumplir con el llamado de las ciencias médicas y las responsabilidades adquiridas como elementos imprescindibles de la salud humana.
