RESUMO: Objetivou-se identificar os erros na assistência de enfermagem em um Centro de Terapia Intensiva, de acordo com a equipe de enfermagem, e discutir os principais à luz da Teoria do Erro Humano. Estudo transversal cujo cenário foi um Centro de Terapia Intensiva de um hospital do estado do Rio de Janeiro -Brasil. Participaram 36 profissionais da equipe de enfermagem. Os dados foram coletados em entrevista estruturada, no período de julho a setembro de 2013, e analisados através EpiInfo™. Dentre os principais erros na assistência de enfermagem destacam-se: erros de medicação, não elevação das grades do leito, perda de catéteres, sondas e drenos, citados por 83% dos entrevistados; extubações acidentais (72%); e higienização inadequada das mãos (67%). A gravidade dos erros identificados comprometem a assistência e a recuperação da clientela. Enfatiza-se a necessidade de medidas de notificação e prevenção, estimulando-se a cultura de segurança e aumento da qualidade assistencial. CARACTERIZAÇÃO DE ERROS ERROR CHARACTERIZATION IN INTENSIVE CARE NURSINGABSTRACT: This study aimed to identify errors in nursing care in an intensive care unit, according to the nursing staff, and discuss the main ones according to Human Error Theory. This was a cross-sectional study, and the scenario was an intensive care unit of a hospital in Rio de Janeiro state -Brazil. Participants were 36 professionals of the nursing team. Data were collected in structured interviews, from July to September 2013 and analyzed using Epi InfoTM. The major mistakes in nursing care included: medication error, not lifting the bed railings, loss of catheters, probes and drains (cited by 87% of respondents); accidental extubation (72%); and inadequate hand hygiene (67%). The severity of the errors identified involve the care and recovery of clientele. It emphasizes the need for notification and for prevention measures, contributing to the culture of safety and increasing care quality.
RESUMO: Estudo documental, desenvolvido em hospital privado no município do Rio de Janeiro, que objetivou analisar o tempo de espera para a realização do primeiro eletrocardiograma (ECG) dos pacientes, com suspeita de síndrome coronariana aguda, na sala de emergência, e discutir suas implicações para o cuidado gerenciado nesta clientela. Foram analisados 116 exames de eletrocardiogramas, do período de janeiro e fevereiro 2010; o tempo médio do ECG inicial foi de 22 minutos. Nos pacientes com infarto agudo do miocárdio, com supradesnível do seguimento s-t (IMACSST), esta média foi de 7,5 minutos. Quanto ao laudo, 2,5% tiveram diagnóstico de IAMCSST; 7,1% de infarto agudo do miocárdio sem supradesnível do segmento s-t e 16,6% foram diagnosticados com angina instável. Conclui-se que o ECG inicial em tempo adequado contribui para a identificação e tratamento precoce dos pacientes com IAMCSST, porém ainda necessita de ajustes para maior adesão da equipe de saúde aos tempos preconizados. A cross-sectional study was conducted at a private hospital in Rio de Janeiro municipality. In January and February 2010, 116 electrocardiograms were performed, and mean time to initial EEG was 22 minutes. In patients with acute ST segment elevation myocardial infarction (STEMI), mean time was 7.5 minutes. In the physicians' reports, 2.5% were diagnosed as STEMI; 7.1% as non-ST segment elevation myocardial infarction, and 16.6% as unstable angina. It was concluded that timely initial ECG contributes to the identification and early treatment of patients with STEMI. However, some adjustments are still needed to improve health teams' adherence to recommended times. Objective:To describe the application of positive deviance as a strategy to prevent and control bloodstream infections. Method: An intervention study with nursing and medical team members working in an intensive care unit in a university hospital, between June and December 2014. The four steps of the positive defiance methodology were applied: to define, to determine, to discover and to design. Results: In 90 days, 188 actions were observed, of these, 36.70% (n=69) were related to catheter dressing. In 81.15% (n=56) of these dressings, the professionals most adhered to the use of flexible sterile cotton-tipped swabs to perform antisepsis at catheter entry sites and fixation dressing. Conclusion: Positive deviance contributed to the implementation of proposals to improve work processes and team development related to problems identified in central venous catheter care. Objetivos: caracterizar as condições sóciodemográficas e de hábitos de vida de uma clientela masculina e discutir sobre gerenciamento do cuidado de enfermagem cardiovascular em nível ambulatorial. Metodologia: é um estudo transversal, descritivo, exploratório com abordagem quantitativa, amostra do tipo de não probabilidade. Resultados: caracterizaram-se homens com mais de 40 anos, aposentados, pensionistas ou desempregados, com baixa renda e escolaridade, padrão de sono insatisfatório e 52% hipertensos. Discussão: observou-se que os fatores de risco são modificáveis, podendo-se corrigi-los através da educação em saúde e mudanças no estilo de vida. Conclusão: são grandes os desafios para o gerenciamento do cuidado de enfermagem na perspectiva de gênero oferecendo importantes discussões sobre promoção e prevenção de agravos à saúde.
