Resumo O estudo que deu origem a este artigo teve por objetivo analisar como o agente comunitário de saúde avalia seu contexto de trabalho e os possíveis fatores que contribuem para seu sofrimento/adoecimento. Tratou-se de um estudo transversal, descritivo, de abordagem quantitativo-qualitativa, com agentes de saúde de 13 municípios da região central do estado do Rio Grande do Sul, no ano de 2014. Utilizaram-se para a coleta de dados um questionário sociodemográfico, o 'Inventário de trabalho e risco de adoecimento' e entrevista. Os dados foram examinados por meio da análise de conteúdo. As situações de sofrimento relacionaram-se ao não reconhecimento profissional, ritmos excessivos, sobrecarga de trabalho, conflitos com a comunidade, não resolutividade das ações, indisponibilidade de materiais e insumos somados à localização geográfica da unidade de saúde e das residências dos usuários, tipo de estratégia e gestão da saúde. Concluiu-se que o contexto de trabalho do agente de saúde pode resultar em sofrimento/adoecimento desses trabalhadores. Palavras-chave agentes comunitários de saúde; trabalho; doença.Abstract The aim of this study was to analyze how community health agents evaluate the work context and possible factors that contribute to their suffering/illness. This was a cross-sectional, descriptive, quantitative-qualitative study carried out with health agents from 13 municipalities in the central region of the state of Rio Grande do Sul, Brazil, in 2014. The 'Work and Risk of Illness Inventory' sociodemographic questionnaire and interview were used to collect data. The data were analyzed through content analysis. The situations of suffering were related to the lack of professional recognition, an excessive pace, work overload, conflicts with the community, lack of resolution of actions, unavailability of materials and inputs, in addition to the geographic location of the health unit and of the users' homes, type of health strategy, and management. It was concluded that the health agent's working environment can cause suffering/illness among these workers.
Resumo O estudo teve como objetivo investigar o custo humano físico, cognitivo e social no trabalho do agente comunitário de saúde (ACS) e sua relação com possíveis danos à sua saúde. Estudo transversal, descritivo, quantitativo-qualitativo, com 251 ACS de municípios da 13ª Coordenadoria Regional de Saúde-RS. Os dados foram coletados em entrevistas, questionário sociodemográfico e “Inventário de Trabalho e Riscos de Adoecimento” (ITRA). Utilizaram-se para a análise dos dados a média dos escores fatoriais e a Análise de Conteúdo. Todos os fatores relativos aos custos humanos apresentaram avaliação moderada/crítica. Quanto aos danos à saúde, os danos físicos receberam avaliação moderada/crítica, sendo os demais avaliados como positivos/suportáveis. Apesar disso, foi relatada a presença de transtornos psíquicos, em um discurso de banalização do sofrimento advindo do trabalho.
Introduction: Pressure injury is 1 of the most common pressure related injuries in patients admitted to the intensive care unit. In individuals with darker skin tones, skin assessment protocols appear to be less effective, resulting in early damage from pressure. Bedside assessment measures using ultrasound and infrared thermography (IRT) have been studied to identify pressure injuries. Patient concerns: A 58-year-old dark-skinned male was admitted to the intensive care unit due to an ischemic stroke. Diagnosis: The visual evaluation of the skin took place on the second day after admission to the intensive care unit (before 48 hours). The patient had a whitish erythema on the left heel (LH) and a large bloody blister on the right heel. There were no signs of color change on the sacrum skin. Interventions: We performed 3 skin evaluations of the sacrum and calcaneus using ultrasound and IRT. Outcomes: Changes in the temperature of the target regions (sacrum, right heel and LH) were observed. The right heel showed higher mean temperatures than the LH in all evaluations. In the first evaluation of the sacrum region, the average temperature was lower (-1.3°C) than in the second and third evaluation (1°C). In the calcaneus, the mean temperature range (right heel - LH) showed a difference of (3.5°C) in the first evaluation, a difference of (1.4°C) in the second evaluation, and a difference of (1.7°C) in the third evaluation. Ultrasound images of the selected regions showed abnormal tissue patterns - edema - since the first evaluation. Conclusion: These findings indicate that the regions with deep tissue injury on ultrasound evaluation were compatible with the regions of abnormal temperatures in the IRT. IRT could identify regions of pathological process, which could be confirmed by abnormal ultrasound findings. Well-designed, randomized research with a larger sample could verify if the combination of these assessment techniques could be used as a potential method for early detection and evaluation of pressure injuries.
O objetivo do estudo foi investigar as condições de trabalho do Agente Comunitário de Saúde (ACS) e as modificações necessárias para o aprimoramento das mesmas. Estudo transversal, descritivo do tipo quantitativo-qualitativo, com 251 ACS de 34 equipes de Estratégias de Saúde da Família e de 5 Estratégias de Agentes Comunitários de Saúde dos 13 municípios da 13ª Coordenadoria Regional de Saúde/RS. Os dados foram coletados com o formulário "Inventário de Trabalho e Risco de Adoecimento" e analisados obtendo-se o escore fatorial dos fatores com o cálculo da média dos itens. Para o estudo qualitativo, cujos dados foram analisados por meio da Análise de Conteúdo, 16 sujeitos participaram de entrevistas, em uma amostragem intencional por município e tipo de estratégia de saúde. Evidenciou-se uma avaliação moderada/crítica no fator condições de trabalho. Os ACS desejam a modificação do ambiente físico do local de trabalho, apontaram a falta de material de apoio e educativo para realização das atividades, de auxílio financeiro e de transporte para as visitas domiciliares. Infere-se que os fatores citados podem ser considerados como dificultadores para a execução do trabalho dos ACS na região estudada.
