Métodos: se administró un cuestionario de autoevaluación de tipo Likert validado, a los médicos de atención de emergencia; se exploraron variables relacionadas al sexo, edad, especialización, años de práctica, antecedentes legales previos y turnos de trabajo. Resultados: se evaluó a 220 médicos de 8 Servicios de Urgencias, el 82% practicaba medicina defensiva. Los médicos varones mostraron un comportamiento más defensivo (p=0,1). Emergentología, Traumatología y Ortopedia fueron las especialidades con mayor práctica de medicina defensiva (100%). Ginecología y Obstetricia tuvo el mayor número de demandas. Conclusión: la medicina defensiva se presenta en un alto porcentaje en los Servicios de Urgencias pues 82% de los médicos practican algún grado de la misma, siendo mayor en varones y en las especialidades Emergentología y Traumatología y Ortopedia Palabras claves: medicina defensiva, mala praxis, responsabilidad legal, servicio de urgencia en hospital. ABSTRACT Objective:To determine the degree of defensive medicine practiced in the Emergency Services of the Hospital Central of the Instituto de Previsión Social, Asunción-Paraguay. Methods: A validated Likert-type self-assessment questionnaire was administered to emergency care physicians. Variables related to sex, age, specialization, years of practice, previous legal background and work shifts were explored. Results: Two hund1red and twenty physicians from eight Emergency Services were evaluated, 82% practiced defensive medicine. The male doctors showed a more defensive behavior (p=0.1). RESUMENEl síndrome neuroléptico maligno like o crisis acinética es una rara y potencialmente letal complicación del abandono de medicación de los pacientes con enfermedad de Parkinson. Se presenta el caso clínico de un paciente de sexo masculino de 52 años de edad, con antecedente de enfermedad de Parkinson de 10 años de evolución que se encuentra en tratamiento con levodopa-carbidopa. Acude a consultar por disminución del estado de conciencia y rigidez muscular generalizada, relacionado con el abandono de medicación 7 días antes del inicio de los síntomas. We present the clinical case of 52-year man with 10-year history of Parkinson's disease who is being treated with levodopa-carbidopa. He consults for decreased level of consciousness and general muscular rigidity, related to the withdrawal of the medication 7 days before the onset of symptoms. Introducción: El VIH es una enfermedad que afecta el sistema inmunitario y lo destruye gradualmente, provocando de esta forma que para el cuerpo sea más difícil combatir infecciones, lo cual facilita que la persona pueda tener SIDA (síndrome de inmunodeficiencia adquirida Resultados: Referente al sexo se observó una predominancia del sexo masculino en 175 pacientes (53%), la edad predomina el rango de 9 a 39 en 140 pacientes (43%), eran solteros 172 casos (52%), 228 de ellos manifestaron haber terminado solo la secundaria, representado el (59%), se sienten completamente aceptados representando el (37%) de los encuestados, en cuanto a los sentimientos de discriminación en donde a 156 (47%) pacientes les molesta demasiado que lo culpen por su enfermedad, donde también se puede ver una similitud en cuanto a sentimientos por parte de los pacientes. Los valores de calidad de vida se han dado con relación a los dominios en un rango de 66 y 79 con una calificación intermedia a buena. Conclusión: La calidad de vida en cuanto a las medias y desviación estándar de los dominios de CV del WHOQOL-HIV BREF, presentaron puntajes en un rango entre 66 y 79 puntos en las seis dimensiones en donde los pacientes presentaron buenos resultados en especial en lo físico. Referente al sexo se observó una predominancia del sexo masculino, referente a la edad una predominancia del rango de 29 a 39 años, solteros en su mayoría y la procedencia de asunción. Con relación a las asistencias recibidas de necesidades médicas y psicológicas, han manifestado mucha necesidad de tratamiento médico para funcionar normalmente día a día en su mayor parte.
