Objective To build a theoretical model that represents the experience of family caregivers of elderly people during the COVID-19 pandemic. Method Qualitative study that used the Grounded Theory as a methodology, carried out in Minas Gerais State in August 2020. 16 caregivers were interviewed. Results Uncomfortable emotions emerged and drove the caregivers’ actions to: adopt preventive measures to protect the elderly from contracting the coronavirus; guarantee their healthcare; and make them aware of the pandemic. However, when these emotions manifested in an exacerbated manner, they needed to be alleviated so that they could adopt such initiatives. Conclusion The emotions felt by caregivers can compromise their health and quality of life. Therefore, health professionals need to create strategies to ensure that they are well assisted, enabling care through telehealth. Guidance on the pandemic for caregivers could reflect better care for the elderly people.
Objetivo: compreender a experiência de cuidadores familiares no cuidado de idosos com demência em ambiente domiciliar. Métodos: foi realizada uma busca sistemática por estudos qualitativos e análise temática para sintetizar os resultados. Resultados: foram incluídos nove estudos. Emergiram quatro temas analíticos: Reconhecendo a demência e desvendando o cuidar; “Você está sozinha”; Limitações e desconhecimentos no cuidar com demência; “É um último recurso (colocação em instituição de longa permanência)”. A perda de autonomia dos idosos gerou sentimentos de luto e solidão. O conhecimento limitado sobre demência suscitou em um cuidado inábil. Os cuidados temporários eram percebidos como uma solução frente à necessidade de ajuda, porém, a institucionalização foi abordada como uma última alternativa. Conclusão: os resultados obtidos são importantes para fornecer subsídio para a criação de políticas públicas mais humanizadas que considerem a integralidade no cuidado, auxiliar profissionais de saúde na prestação de cuidados aos idosos com demência e sua família.
In this review, the main goal was to answer the question: What are the family caregivers’ experiences in the process of providing care to older people? To do so, a systematic literature review was carried out by consulting the MEDLINE, Cochrane, and Lilacs databases. Two reviewers selected the studies independently, and discrepancies were sorted out by a third reviewer. The Critical Appraisal Skills Programme was used to evaluate the methodological quality of the included studies, and thematic analysis was applied to synthesize the results. The analytical themes addressed in the synthesis were developed through researchers’ interpretations and new constructs. Confidence in the findings was reviewed using GRADE-CERQual. The study adhered to Enhancing Transparency in Reporting the Synthesis of Qualitative Research guidelines. Eighteen studies were included in the synthesis. Three analytical themes emerged: "Mixed feelings", "You have no life. Your own life gets put on hold”, and "The need for increasing coping strategies". Several conflicting feelings emerged in the process of care, showing the complexity of the caregivers’ experience. Either way, the caregivers put their lives on hold to provide care and sought coping strategies, such as formal and informal support and spirituality. Therefore, both health professionals and policymakers must ensure comprehensive care for the older adults and their families, so that caregivers do not compromise their own lives and health in order to take care of others.
Objective: to describe the implementation and to assess the effectiveness of a pilot integrated qualification program to improve the medication use in a long-term care facility (LTCF). Methods: This was a type 1 hybrid effectiveness-implementation study. A pilot integrated qualification program to improve the medication use in a LTCF was carried out by implementing a new drug distribution system and a comprehensive medication management (CMM) service according to the following four steps: I) implementation of the drug distribution system followed by the evaluation of the health team's opinion; II) prescription review with the identification of potential drug therapy problems (PDTPs); III) provision of the CMM service according to the framework of Pharmaceutical Care practice within one year; and, IV) evaluation of the effectiveness of the program through the comparison of clinical and laboratory parameters (blood pressure, glycated hemoglobin and lipid fractions) using the t-test or Wilcoxon signed-rank test. Results: In step I, the distribution system was fully outsourced to a company that furnished all solid oral dosage forms in individual boxes containing a plastic coil with multiple envelopes for 30 days. In step II, 180 PDTPs were identified, and all patients presented with at least one of them. In step III, after the first assessment of the CMM Service, 43 actual drug therapy problems (DTPs) were identified. After one year of service provision, 96 DTPs were identified and 75.8% of them were resolved (n=72). In step IV, a statistically significant difference was observed between the initial and final minimum and maximum systolic and diastolic blood pressure (p<0,05). Conclusions: The pilot integrated qualification program had a positive impact on the clinical parameters. The global population is rapidly aging, making this type of study important to exemplify a multifaceted strategy to improve the quality of drug therapy for institutionalized patients.
RESUMO Objetivo Construir um modelo teórico que representa a experiência de cuidadores familiares de idosos durante a pandemia da COVID-19. Método Estudo qualitativo que utilizou como metodologia a Teoria Fundamentada nos Dados, realizado em Minas Gerais em agosto de 2020. Foram entrevistadas 16 cuidadoras. Resultados: Emoções desconfortantes emergiram e impulsionaram as ações das cuidadoras em prol de: adotar medidas preventivas para proteger os idosos de contraírem o coronavírus; garantir a assistência à saúde do mesmo; e, conscientizá-lo sobre a pandemia. Todavia, quando essas emoções se manifestavam de formas exacerbadas, elas precisaram ser amenizadas para que elas conseguissem adotar tais iniciativas. Conclusão As emoções sentidas pelas cuidadoras podem comprometer sua saúde e qualidade de vida. Portanto, os profissionais de saúde precisam criar estratégias para garantir que elas estejam bem assistidas, viabilizando atendimentos por meio de telessaúde. A orientação sobre a pandemia aos cuidadores poderá refletir em um melhor cuidado aos idosos.
Resumo A divisão sexual do trabalho ocasiona iniquidade entre gêneros, bem como situações de violência direcionadas às mulheres no ambiente laboral. Na área farmacêutica, a maioria feminina e a inserção em múltiplos cenários, que envolvem contato com público e parcerias profissionais diferenciadas, potencializam a exposição à violência. Assim, o objetivo deste estudo foi analisar situações de violência vivenciadas por farmacêuticas no ambiente laboral. Para tal, utilizou-se método qualitativo, com análise de conteúdo, realizada no software NVivo®, de uma survey on line, respondida por farmacêuticas (n=381) registradas no Conselho Regional de Farmácia de Minas Gerais, Brasil. Como resultado, identificou-se quatro categorias: ‘Convivendo com o desrespeito, ameaças e vulnerabilidade’, ‘O sexismo e o machismo estrutural que cala, diminui e atordoa’, ‘A discriminação contra a mulher como obstáculo à equidade’ e ‘Assédio sexual e objetificação da mulher’. Emergiram diversas expressões de violências no ambiente laboral provenientes de múltiplos agressores. Farmacêuticas reconheceram prejuízo profissional e pouco reconhecimento pela capacidade técnica. Conclui-se que os relatos expuseram a falta de proteção da integridade farmacêutica no ambiente de trabalho. Espera-se que esses resultados tragam à luz a desigualdade de gênero no trabalho farmacêutico, com destaque para situações de violência, ampliando a discussão e proporcionando evolução dessa profissão majoritariamente feminina.
Introdução: A terapia antirretroviral (TARV) tem impacto na melhora da expectativa de vida das pessoas vivendo com HIV (PVHIV), embora alguns fatores possam dificultar a continuidade do tratamento como a complexidade da farmacoterapia, interações medicamentosas, efeitos adversos, falhas terapêuticas e não adesão. Objetivo: Reunir os estudos que abordam serviços farmacêuticos de acompanhamento farmacoterapêutico oferecidos a PVHIV para descrever os resultados clínicos alcançados com a oferta desse serviço a esses pacientes. Métodos: Trata-se de uma revisão integrativa da literatura. As buscas na literatura foram realizadas nas bases de dados eletrônicas PubMed e LILACS, usando os descritores Medical Subject Headings (MeSH): “HIV”, “pharmaceutical services”, “medication therapy management”, “pharmaceutical care”. Um total de 354 artigos foi recuperado, sendo nove artigos incluídos, pois abordavam o serviço de acompanhamento farmacoterapêutico a PVHIV. Resultados: Foi possível observar que os farmacêuticos que ofereceram o serviço promoveram melhor adesão à TARV, a identificação e resolução de problemas relacionados ao uso de medicamentos e realizaram intervenções de educação em saúde. Conclusão: Existem diversos serviços farmacêuticos com o potencial de trazer benefícios clínicos a essa população, com destaque para serviços de acompanhamento farmacoterapêutico. Porém, ainda são necessários mais estudos que avaliem de forma robusta o impacto clínico, humanístico e econômico desses serviços.
