Objetivo: Discutir e revisar sobre a importância do cardiologista no reconhecimento precoce e na abordagem da insuficiência cardíaca como doença cardíaca terminal. Revisão bibliográfica: As doenças cardiovasculares são a causa número um de morte global, sendo a insuficiência cardíaca o protótipo da doença cardíaca avançada. Segundo a Organização Mundial de Saúde foi incluído um grupo de doenças cardiovasculares como primeira demanda para cuidados paliativos no mundo, sendo que, a insuficiência cardíaca representa um desafio na avaliação prognóstica, uma vez que muitos pacientes morrem subitamente, mesmo em classes funcionais iniciais. Muitos desses pacientes são avaliados em um momento tardio de sua doença, e a grande maioria não é abordada pelo seu cardiologista assistente. Considerações finais: O conhecimento técnico da doença, aliado a todo o trabalho precoce com o paciente e a família podem proporcionar um tratamento otimizado e um adequado controle de sintomas em todos os momentos da doença, podendo diminuir o número de mortes precoces e aumento na expectativa de vida.
INTRODUÇÃO: A fluoroscopia é procedimento primário de imagem utilizado para manipulação de cateteres principalmente em cirurgias menos invasivas, e utiliza de doses por ionizantes que levam a lesões cutâneas, ocasionando dermatites por radiação e como consequência decorre diagnóstico de radiodermite por exposições acumulativas a estas doses em procedimentos realizados. OBJETIVO: relatar caso de paciente portadora de fibrilação atrial paroxística e submetida à ablação percutânea por cateter de crioablação, além de cateterismo cardíaco e angioplastia percutânea de artéria femoral direita, posteriormente diagnosticada com radiodermite relacionada à fluoroscopia. CASO CLÍNICO: paciente de 60 anos portadora de fibrilação paroxística submetida à ablação percutânea por cateter, com fístula arteriovenosa femoral, com necessidade de arteriografia e angioplastia, evoluindo após 15 dias com eritema e necrose dérmica de contornos regulares. CONCLUSÃO: após evidencias do relato de caso clínico nota-se a importância da análise de procedimentos guiados por fluoroscopia, quanto benefícios e riscos a pacientes e profissionais, por se tratar de diagnóstico raro e inesperado.
Objetivo: Abordar as manifestações ecocardiográficas de Doenças Cardiovasculares (DCV) na infecção pelo Vírus da Imunodeficiência Humana (HIV), com ênfase nos dados que contribuem para o diagnóstico precoce e antecipação de complicações. Revisão bibliográfica: Atualmente, com a moderna Terapia Antirretroviral (TARV) e prolongamento da expectativa de vida da Pessoa Vivendo com HIV (PVHIV) com carga viral controlada, a atenção e o cuidado têm sido direcionados ao combate de doenças crônicas e complicações secundárias, não infecciosas. As disfunções cardíacas nos pacientes portadores de HIV ocorrem com frequência variável e diversidade de apresentações clínicas e constituem uma das causas mais comuns de morte nesta população. A complexa e multifatorial patogênese das complicações cardiovasculares associadas ao HIV envolve fatores de risco tradicionais para DCV, efeitos deletérios do vírus, status inflamatório e distúrbios metabólicos relacionados a TARV. A maioria das alterações detectadas pelo ecocardiograma persistem durante o acompanhamento, de forma assintomática, sugerindo um longo período de alterações cardíacas subclínicas. Considerações finais: A ecocardiografia demonstra ser uma importante ferramenta no seguimento de pacientes com HIV para diagnóstico primário e detecção precoce do acometimento cardiovascular, permitindo o tratamento em tempo oportuno e melhora da qualidade e expectativa de vida dos pacientes.
