Objetivo: Fazer uma atualização, com base na literatura recente, sobre a síndrome de Takotsubo, explicando aspectos sobre sua fisiopatologia, diagnóstico diferencial e tratamento. Revisão bibliográfica: A síndrome de Takotsubo é uma doença de importância clínica, caracterizada por insuficiência cardíaca aguda geralmente reversível, desencadeada por intenso estresse emocional ou físico, mas sua prevalência pode estar sendo subestimada devido a confusão diagnóstica com a síndrome coronariana aguda. Com isso, muitos casos de Takotsubo podem estar erroneamente sendo diagnosticados como infarto agudo do miocárdio. Considerações finais: Tendo em vista seu caráter reversível, é indicado que a síndrome de Takotsubo seja considerada como um diagnóstico diferencial em pacientes que apresentam características clínicas de síndrome coronariana aguda. Para isso, a história clínica, além de exames subsidiários, a exemplo do eletrocardiograma e troponina, bem como exames de imagem, tais como ecocardiografia, ressonância magnética cardíaca e angiografia coronariana, devem ser realizados para identificar as alterações típicas do ventrículo esquerdo causadas pela síndrome de Takotsubo.
Objetivo: Discutir e revisar sobre a importância do cardiologista no reconhecimento precoce e na abordagem da insuficiência cardíaca como doença cardíaca terminal. Revisão bibliográfica: As doenças cardiovasculares são a causa número um de morte global, sendo a insuficiência cardíaca o protótipo da doença cardíaca avançada. Segundo a Organização Mundial de Saúde foi incluído um grupo de doenças cardiovasculares como primeira demanda para cuidados paliativos no mundo, sendo que, a insuficiência cardíaca representa um desafio na avaliação prognóstica, uma vez que muitos pacientes morrem subitamente, mesmo em classes funcionais iniciais. Muitos desses pacientes são avaliados em um momento tardio de sua doença, e a grande maioria não é abordada pelo seu cardiologista assistente. Considerações finais: O conhecimento técnico da doença, aliado a todo o trabalho precoce com o paciente e a família podem proporcionar um tratamento otimizado e um adequado controle de sintomas em todos os momentos da doença, podendo diminuir o número de mortes precoces e aumento na expectativa de vida.
Introdução: a tuberculose é causa de mortalidade por um único agente infeccioso, o mycobacterium tuberculosis, que afeta o sistema respiratório e outros sistemas do corpo humano, como o sistema cardiovascular. Ela pode acometer o pericárdio e levar à pericardite tuberculosa, que em sua apresentação típica se dá na forma de pericardite constritiva. Objetivo: relatar o caso de um paciente com pericardite constritiva secundária a uma tuberculose extrapulmonar. Caso clínico: paciente, sexo masculino, 17 anos, sintomas de tuberculose, mas exames não compatíveis, que desenvolveu hepatite medicamentosa com o uso dos antituberculínicos. Após biópsia do pericárdio foi constatada pericardite constritiva por tuberculose extrapulmonar. Foi realizada pericardiectomia em que se constatou mediastinite. Iniciou-se um esquema de antibioticoterapia alternativo devido à hepatite medicamentosa ao uso do esquema terapêutico inicial. O paciente evoluiu com boa resposta a esse esquema terapêutico, sem efeitos colaterais relatados. Conclusão: a pericardiectomia continua sendo o tratamento de escolha para pacientes com sintomas graves de pericardite constritiva causada por tuberculose pulmonar e/ou extrapulmonar. O paciente deste estudo repetiu o ecocardiograma e mostrou boa recuperação da função cardíaca.
A síncope trata-se de um evento frequente e responsável por promover seguidos desafios no ambiente profissional da saúde. Caracteriza-se por formatos que sofrem variações constantes, e os fatores causadores da síncope, em geral, não são capazes de serem representados detalhadamente, mesmo diante de análises clínicas minuciosas – além dos aspectos técnicos, na maioria dos casos, uma análise detalhada para averiguar os fatores causadores da síncope necessita de investimento financeiro elevado. Considerando-se os obstáculos técnicos e financeiros associados à abordagem diagnóstica, determinadas diretrizes científicas foram idealizadas a fim de apoiar, por meio de orientações técnicas, os especialistas na área da saúde referente ao diagnóstico e ao gerenciamento, especialmente, devido ao elevado número de invidíduos com quadro clínico associado à síncope. Ao considerar as principais diretrizes em nível internacional, a presente pesquisa destaca a Diretriz Norte Americana de 2017 e a Diretriz Europeia de 2018. De acordo com o contexto apresentado, essa pesquisa pretende analisar o seguinte problema de pesquisa: quais são as principais características em uma abordagem diagnóstica sincopal? Referente ao objetivo geral, busca-se aferir questões relevantes sobre o diagnóstico da síncope, especialmente, à luz das diretrizes norte americanas e das diretrizes europeias. Por sua vez, os objetivos específicos estão descritos a seguir: verificar aspectos conceituais sobre perda transitória de consciência (PTC) e síncope; investigar questões relevantes referente a abordagem diagnóstica sincopal; analisar características e discrepâncias sobre atendimento e diagnóstico da síncope entre a Diretriz Norte Americana de 2017 e a Diretriz Europeia de 2018. Neste estudo, foi adotado o procedimento metodológico associado à Revisão Bibliográfica Narrativa, também denominado como Revisão de Literatura, em que a pesquisa é qualitativa e de caráter exploratória.
