This article is part of the evaluation study on the Project for Expansion and Consolidation of the Family Health IntroduçãoA Estratégia Saúde da Família busca romper com paradigmas cristalizados e incorpora novo pensar e agir na perspectiva de mudança e conversão do modelo assistencial. Dessa forma, possibilita a entrada de cenários, sujeitos e linguagens no âmbito da atenção à saúde com potenciais para reconstrução das práticas. Nessas, o cuidado deve considerar o princípio da integralidade e o usuário como protagonista. Pressupõem ainda a presença ativa do outro e as interações subjetivas, ricas e dinâmicas, exigindo ampliação dos horizontes da racionalidade que orienta tecnologias e agentes das práticas 1 .Para Merhy 2 , o serviço de saúde, ao adotar práticas centradas no usuário, faz-se necessário desenvolver capacidades de acolher, responsabilizar, resolver e autonomizar. Nesse sentido, o trabalho em saúde deve incorporar mais tecnologias leves que se materializam em práticas relacionais, como, por exemplo, acolhimento e vínculo.
Este estudo objetiva avaliar a satisfação do usuário quanto ao cuidado do Programa de Saúde da Família (PSF) de dois pequenos municípios do Rio Grande do Norte. Foi realizado um Estudo de Caso entre outubro e dezembro de 2007 utilizando 4 grupos focais com 60 usuários. O método de análise foi o de conteúdo, com as seguintes categorias: concepções sobre necessidades de saúde e PSF; integralidade do cuidado (acolhimento, vínculo, visita domiciliar), ações de promoção da saúde, facilidades e dificuldades em relação aos atendimentos, e participação social. Os resultados mostraram: percepção das necessidades de saúde como assistência; desconhecimento do programa; satisfação pela ampliação do acesso; melhoria da qualidade; vínculo; acolhimento; e visita domiciliar. O motivo de insatisfação foi a presença descontínua das equipes e a falta de centros de saúdes. Ações de promoção e controle social eram praticamente inexistentes, direcionando as reivindicações aos políticos locais. Concluiu-se que não houve conversão, e sim reprodução de um modelo de atenção simplificada, não articulado nem com promoção nem com saúde como direito social.
Este artigo apresenta parte dos resultados referentes à pesquisa avaliativa Estudos de Linha de Base, desenvolvida em 21 municípios com mais de 100 mil habitantes em três estados do Nordeste do Brasil. O objetivo geral foi avaliar experiências de implementação do Programa Saúde da Família (PSF), com foco nas induções do PROESF. Foi realizada Análise de Implantação utilizando-se como método o estudo de caso. Na análise utilizaram-se as dimensões: político-institucional, organização da atenção e cuidado integral. Como avanços destacam-se: priorização do PSF em áreas de risco; aprendizado institucional, qualificação dos gestores e equipes; centrais de regulação, vínculo e percepção positiva sobre PSF. Em relação aos desafios observaram-se: fortalecimento da capacidade formuladora local, alocação dos recursos na Atenção Básica, desprecarização do trabalho, efetivação de rede de atenção, fortalecimento da participação social, qualificação do monitoramento e avaliação para tomada de decisão; acolhimento; filas para exames, consultas e internação; implementação do trabalho em equipe e atividades promocionais e intersetoriais.
Este artigo objetiva discutir avanços e desafios do trabalho em equipe multiprofissional na reabilitação infantil. A pesquisa desenvolveu-se num centro de referência estadual, em Natal, RN, Brasil, entre maio e julho de 2010. O método utilizado foi o estudo de caso, com entrevistas semiestruturadas, numa amostra qualitativa de 20 profissionais de saúde de nível superior. Contemplaram-se as categorias: concepção de trabalho em equipe; interação nesta e com família e escola; intercâmbio de conhecimentos e interdisciplinaridade; flexibilidade da divisão do trabalho; integralidade e qualidade da assistência; valorização do trabalho; limites profissionais; e relações interpessoais. Aplicou-se a análise de conteúdo. Os resultados mostraram busca da integralidade e objetivos comuns. Como desafios destacaram-se: falta de projetos assistenciais, protocolos e discussões; dificuldades de interação, integração e delimitação profissional; rigidez na divisão de trabalho. Conclui-se que o trabalho organiza-se numa dimensão instrumental voltada à integralidade e à resolutividade, mas sem integração e reconhecimento mútuo de atuação em equipe.
Atualmente, o PSF convive com práticas do modelo biomédico e outras marcadoras de mudança. Este artigo aborda experiências inovadoras de cuidado baseada em pesquisa realizada em Natal no período de 2005 a 2007. Teve como objetivo compreender os sentidos da integralidade, discutir o papel da criatividade e sensibilidade dos profissionais e analisar as possibilidades de institucionalização e sustentabilidade. Utilizou análise documental; entrevista semi-estruturada com os protagonistas, observação das unidades e experiências. Dentre as concepções de integralidade encontradas destacam-se: promoção da saúde como mudança do estilo de vida; aproximações dialógicas entre saber científico e popular; fortalecimento da participação popular, auto-cuidado. Houve reconhecimento social expressos em ganho de prêmios, publicações e peças teatrais. Nenhum dos projetos contava com financiamento. Conclui-se que as experiências têm uma vocação para promoção e cuidado, mas ainda não têm sustentabilidade financeira e política.
