AIM:To compare the ultrasound, cytological and histopathological fi ndings of thyroid nodules undergone fi ne needle aspiration biopsy and to determine the effi ciency of fi ne needle aspiration biopsy. METHODS:The records of 1420 patients diagnosed with a thyroid nodule and undergone fi ne needle aspiration biopsy between January 1 and June 30, 2010 were evaluated retrospectively. The relation and consistency among the ultrasound, cytological and histopathological fi ndings were analyzed. RESULTS:The study included 1254 (88.3%) female and 166 (11.7%) male patients. The mean age of the patients was 49.48 years and 10.5% of the patients were operated. The mean nodule sizes of the patients managed with or without surgery were 25.5±14.20 mm and 17.62±9.69 mm, respectively. Surgical management option was negatively correlated with the size of the examined nodule (p<0.05). The mean size of the benign and malignant nodules were 27.01 and 20.68 mm, respectively, and the difference was signifi cant (p<0.001). CONCLUSION:Fine needle aspiration biopsy is a useful diagnostic tool for the evaluation of thyroid nodules and has a high consistency with ultrasound and histopathological fi ndings. In the majority of the patients it helps to determine the next clinical management option. ÖZET AMAÇ: İnce iğne aspirasyon biyopsisi yapılan tiroit nodüllerinin ultrasonografi, sitoloji ve histopatoloji bulgularını karșılaștırmak ve ince iğne aspirasyon biyopsisinin etkinliğini belirlemek.YÖNTEM: Radyoloji Kliniği'nde, 01 Ocak ve 30 Haziran 2010 tarihleri arasında tiroit nodülü saptanan ve ince iğne aspirasyon biyopsisi yapılan 1420 hastanın kayıtları retrospektif olarak incelendi. Ultrasonografi, sitoloji ve histopatoloji bulguları arasındaki ilișki ve uyumluluk araștırıldı.
AIM: To determine the diagnostic value of fi ne needle aspiration biopsy (FNAB) of parotid masses in the basis of postoperative histopathological results. METHODS: The study was conducted in Ankara Atatürk GirișTükürük bezi tümörleri, tüm baş boyun tümörlerinin %3-12'sini ve tüm vücut tümörlerinin ise %2-3'ünü oluşturmaktadır. Bu tümörlerin %80'i parotis bezinden köken almaktadır 1 . Parotis bezi tümörlerinin yaklaşık %80'i benign, %20'si ise malign yapıdadır 1-3 . Parotis tümörlerinde preoperatif değerlendirme, parotise komşu yapılardan köken alan lezyonların ayrılmasında ve lezyonlara yönelik farklı tedavi yaklaşımları olması nedeniyle oldukça değerlidir. Doğru değerlendirme ile medikal tedavi, klinik takip ya da cerrahi tedavi seçeneklerinden birine karar verilmesi yanı sıra, preoperatif değerlendirme bezin fasial sinir ile yakın komşuluğu nedeniyle cerrahi planlama ve gereksiz cerrahiyi engellemek açısından da önemlidir 4,5 . Preoperatif patolojinin bilinmesi cerrahi planlamada da kolaylık sağlamaktadır. Operasyon öncesi değer-lendirme aşamasında ince iğne aspirasyon biyopsisi (İİAB) sıklıkla kullanılmaktadır.Bu çalışmada, parotis bezi kitlelerinin histopatolojik tanıları ile operasyon öncesi yapılan İİAB bulguları karşılaştırılarak İİAB'nin duyarlılık, özgüllük, pozitif tahmin değeri (PPV) ve negatif tahmin değerlerinin (NPV) ve etkinliğinin belirlenmesi amaçlandı.

Genellikle kontamine sudan içen ya da akarsularda yüzen kişilerde görülür. Su içme esnasında vücuda alınan sülükler nazal kavite, orofarenks, hipofarenks, larenks, trakea ve özefagusa tutunabilirler 2 . Larenks mukozasına tutunmaları daha nadir görülür 3,4 . Tutundukları bölgeye hirudin denilen antikoagulan bir madde salgılarlar ve kan emmeye baş-larlar. Birkaç hafta içerisinde kan emerek büyürler. En önemli belirtileri hemoptizi veya epistaksistir. Ayrıca larengeal sülükler özellikle çocuklarda inspiratuar stridora, öksürüğe ve larengeal spazma yol açabilirler 5 .Bu olgu sunumunda nadir olarak görülen epiglot larengeal yüzüne tutunmuş, serbest kalan kısmı rima glottis arasında hareket eden canlı sülük enfestasyonunu sunuyoruz. Olgu SunumuAltmış dört yaşında, bir erkek hasta kliniğimize hemoptizi, ses kısıklığı, boğazda takılma, yabancı cisim hissi ve solunum sıkıntısı yakınmalarıyla başvurdu. Anamnezinden çoban olduğunu ve gece susuzluğunu gidermek için doğal bir kaynak suyundan içtiğini ve yakınmalarının bundan sonra başladığı öğrenildi.Bir hafta sonra bu yakınmaların geçmemesi üzerine başvurduğu aile hekimi tarafından üst solunum yolu enfeksiyonu tanısıyla tedavi verilmiş ancak aldığı bu medikal tedaviden fayda görmemiş. Yakınmalarının artması üzerine hasta polikliniğimize başvurmuş.Kulak burun boğaz muayenesinde; indirekt laringoskopide epiglot larengeal yüzde yeşil-kahverengi görü-nümde, serbest kısmı rima glottise doğru uzanan hareketli (canlı) yabancı cisme rastlandı (Şekil 1). Diğer larengeal yapılar doğal izlendi. Muhtemel sülük olan GirișSülükler (hirudinea) tabiatta parazit olarak bulunan, eski çağlardan beri tedavi amaçlı da kullanılabilen canlılardır. Kan emici olan sülük değişik renklerde ve deği-şebilen uzunluklarda olan (1 cm ile 20 cm) hermafrodit bir parazittir. Bazı sülükler denizlerde, çoğu türü ise tatlı sularda yaşar. Yüzeyinde bitki bulunan havuz, göl ve hafi f akan akarsu kenarlarını tercih ederler 1 .
In this report, with regard to the diagnosis and treatment, we aimed to share our case in whom there was symptomatic multiple mucoceles in a minor salivary gland, which were localized in the nasopharynx in the left rosenmüller fossa and in the aryepiglottic fold in the hypopharynx, which is very rarely encountered. CaseA 45-year-old female patient presented to our clinic with the complaints of intermittent difficult breathing through the left side of the nose and the feeling of food getting stuck in the throat during swallowing, which had been presented for one year. The physical examination performed on the patient revealed normal findings, and on the endoscopic examination of the nasopharynx and the larynx, a cystic mass was observed in the nasopharynx filling the left rosenmüller fossa and in the left aryepiglottic fold neighboring the epiglottis (Figure 1, 2). The laboratory findings were normal and the patient's past medical history was unremarkable.Under general anaesthesia, using the endoscopic method, the cystic mass filling the left rosenmüller fossa in the nasopharynx was marsupialized, and in laryngoscopy, the cystic mass in the left aryepiglottic fold neighboring the epiglottis was totoally excised under microscopy. The pathology of the excision material was reported as a salivary gland mucocele (Figure 3). No recurrence was observed on the follow-ups performed for two years.
