Los tumores vertebrales de columna tienen una frecuencia muy baja. La incidencia de quiste óseo aneurismático es del 1-4% de todos los tumores. Su ubicación predominante es la columna lumbar, en mayor medida, en el arco posterior. Según estudios publicados, esta enfermedad se comporta de forma agresiva, por la compresión radicular que genera debido a su expansión. Si bien hay bibliografía sobre el tratamiento quirúrgico del tumor y su resección en bloque y embolización, no hay experiencias sobre el tratamiento con anticuerpos monoclonales. Existen muy pocos artículos que corroboren un tratamiento para las recidivas o los quistes óseos aneurismáticos inoperables o intralesionales, los cuales dejan la posibilidad abierta a otras técnicas. Presentamos el caso de una mujer de 38 años con lumbocruralgia. En los estudios, se detecta una tumoración, y la biopsia por punción y las imágenes confirman que se trata de un quiste óseo aneurismático. Se planifica el tratamiento con embolización preoperatoria y exéresis intracompartimental mediante cirugía posterior con liberación del saco dural. A los seis meses, el tumor recidiva. Se indica tratamiento con denosumab por dos años, se realizan controles periódicos y se constata la remisión de los síntomas y la mejoría de las imágenes.
Background: Pyogenic Spinal Epidural Abscess (PSEA) is difficult to diagnose and can have devastating consequences. Magnetic Resonance Imaging (MRI) has high sensitivity and specificity, which are further increased with the use of contrast. There are several classifications of vertebral infectious processes, with emphasis on spondylodiscitis. Objective: To analyze the morphological parameters and their reproducibility; and to analyze different resonance imaging sequences. Methods: Using an image database, a morphological classification of PSEA was planned, with five parameters: Region (R), indicating the upper and lower limits of the abscess; Location (U), indicating whether the abscess is anterior or posterior within the canal; Compromise (C), meningeal or content of the structures; Association (A), discitis, osteomyelitis or both; and Perivertebral (P), anterior, lateral or posterior extravertebral abscess. The first three parameters give an idea of the volume of the PSEA, while the last two give the related infectious foci. Thirty-five cases were analyzed using Kappa’s coefficient. Results: The global intra- and interobserver reproducibility was Kappa 0.81. The results for each parameter were as follows: R=0.95, U=0.92, C=0.66, A=0.70 and P=0.80. The first three give a notion of volume and the last two relate to the presence of vertebral infectious foci outside the canal. T2 weighted MRI with contrast was found to be the most effective imaging sequence. Conclusion: The morphological classification is simple to use, with excellent reproducibility. The parameters with the highest reproducibility were region and location, with values >0.92. The addition of gadolinium contrast increased the sensitivity of the diagnosis; the use of sagittal and axial images in T2-MRI was the most sensitive imaging sequence. Evidence Level III; Original.
Introducción: Las lesiones raquídeas son frecuentes en hombres adultos jóvenes. Reconocer la cinemática ayuda a disminuir la tasa de diagnósticos tardíos, principalmente si hay trastornos de la conciencia. Las fracturas vertebrales pueden ser únicas o múltiples, y asociarse con lesiones extravertebrales. Los objetivos de este estudio fueron analizar la distribución de la lesión según el mecanismo de producción, caracterizar el cuadro neurológico, evaluar el patrón de lesión y la asociación con lesiones extravertebrales, y analizar el tratamiento. Materiales y Métodos: Estudio multicéntrico, prospectivo de pacientes con lesiones vertebrales postrauma, que ingresaron entre el 1 de julio de 2018 y el 30 de junio de 2020. Se analizaron los siguientes parámetros: edad, sexo, cinemática, cuadro neurológico, sector afectado, patrón de lesión, lesiones extravertebrales asociadas. Resultados: Se evaluó a 281 pacientes (60% hombres) con 400 lesiones vertebrales y 118 extravertebrales que, en 62 casos, conformaban un cuadro de politraumatismo; 147 con trauma por caída de altura y 98, por accidente de tránsito. El cuadro neurológico más observado fue ASIA E (8 casos), no determinado al ingreso. El sector T2-L5 fue el más afectado, en su mayoría, por lesiones por compresión. Las lesiones extravertebrales más frecuentes fueron el trauma de cráneo y de tórax; hubo un caso de SCIWORAy un óbito temprano. Conclusiones: Los sectores raquídeos más afectados fueron: el toracolumbar, el torácico y el lumbar; las lesiones suelen deberse a caídas de altura y suelen ser únicas, sin cuadro neurológico. El tratamiento se decide según la estabilidad y el cuadro neurológico.
Bone mineral density is a crucial factor in the success or failure of osteosynthesis in spine surgery; it shows the onset of osteoporosis and related complications. Its evaluation is verified by dual-energy X-ray absorptiometry (DEXA) and Hounsfield Unit (HU) measurement by CT scan. Objective: Determine the use of HU in surgical planning; compare utility in diagnosing osteoporosis by DEXA; and evaluate sensitivity in predicting complications. Method: A systemic literature review was conducted on PubMed, in line with PRISMA methodology. Including those who justified the use of pre-surgical planning, compared HU/DEXA, and assessed complications. For the statistical analysis, the χ2 was used. Results: 57 articles were identified by selecting nine that met the inclusion criteria. In patients undergoing spinal surgery for fixation and fusion for degenerative pathology, HU measurement showed a prevalence of osteoporosis of 58.5% (sensitivity 93.26%; specificity 90.22%), osteoporosis-associated complications of 24.5%, proper diagnosis of 71.98%, and screw release rate of 82.31%. Conclusions: UH measurement for the diagnosis of osteoporosis turns out to be more sensitive, specific, and predictive compared to DEXA, especially in elderly patients; it represents a useful tool in planning spinal surgery, minimizing the risk of complications such as screw release, fractures, pseudoarthrosis, subsidence of intersomatic devices, and kyphosis of the proximal junction. Introducción: El absceso epidural espinal piógeno es la localización de una colección infecciosa dentro del canal raquídeo, por fuera del saco dural. El diagnóstico es difícil y las consecuencias son devastadoras a causa de la compresión o el compromiso vascular. La resonancia magnética tiene una alta sensibilidad y especificidad diagnósticas que aumentan si se administra un medio de contraste. Los objetivos de este estudio fueron: determinar la reproducibilidad inter- e intraobservador, y discriminar los distintos parámetros y diferencias entre especialidades. Materiales y Métodos: Veintisiete observadores independientes evaluaron 5 parámetros: región, ubicación, compromiso, asociación y perivertebral, absceso extravertebral anterior, lateral o posterior. Se analizaron 35 casos en tres oportunidades, se utilizó el coeficiente kappa. Resultados: El nivel de acuerdo global intra- e interobservador global fue kappa 0,76; los valores obtenidos fueron: región 0,94; ubicación 0,88; compromiso 0,55; asociación 0,67 y perivertebral 0,77. Los tres primeros parámetros dan noción de volumen y los dos últimos reflejan la presencia de focos infecciosos vertebrales por fuera del conducto. Conclusiones: La clasificación morfológica propuesta es de uso simple y tiene una muy buena tasa de reproducibilidad intra- e interobservador. Los parámetros con mayor reproducibilidad son: región y ubicación (>0,87).
