Giant Cell Tumor (GCT), a benign tumor with local aggression, corresponds to 5% of primary tumors. Fifteen percent of these are located in the sacrum. En bloc resection is an effective treatment, but when it cannot be performed, Denosumab may be indicated as an alternative treatment. The objectives of this work are: to justify the indication; determine the best dose and time of use; and recognize the need for post-treatment surgery. Methods A systematic search of clinical trials. Twenty-five articles were selected, ten of which met the inclusion criteria. The use of Denosumab is justified in advanced stages, with a dose of 120 mg administered subcutaneously, every 7 days in the first month and then maintained every 4 weeks, for 2.5 to 13 months. Adverse events are mild and can be observed in 84% of patients. With Denosumab, surgery may be less aggressive or even unnecessary. The bibliography justifies the indication of Denosumab in advanced stages of GCT, with a dose of 120 mg administered subcutaneously; there is no consensus as to the maintenance dose, which is a weekly dose in the first month and then every four weeks for 2.5 to 13 months. Introducción: La acrobacia en tela es una práctica circense con muchos adeptos en la población urbana. Consiste en sostenerse en altura tomado por dos extremos de tela, para realizar posturas fijas y cambiar entre ellas mediante deslizamiento y caídas. La práctica profesional no está libre de lesiones. Objetivo: Conocer la naturaleza de la lesión, reconocer los factores predisponentes de lesión y las medidas de prevención utilizadas, evaluar la cinemática, clasificar la lesión y analizar el tratamiento. Materiales y Métodos: Se incluyeron seis pacientes. Se evaluaron la estructura física personal, la cinemática de la caída, los sistemas de protección, la clasificación de las fracturas según la nueva clasificación AO, el cuadro neurológico, el tratamiento y las complicaciones. Resultados: La muestra incluyó a 6 mujeres, con un promedio de edad de 24 años y un índice de masa corporal de 19,29. Caída de 3,08 m de altura, cuatro con colchoneta <5 cm. Las lesiones principales fueron: 6 A1, 4 B2 y 1C. No se observaron déficits neurológicos. Cuatro pacientes fueron sometidas a cirugía. Conclusiones: La práctica de acrobacia en tela es una actividad circense de reciente aparición global. Las mujeres están más expuestas a las lesiones. El entrenamiento riguroso y el empleo de elementos de seguridad son necesarios para evitar las lesiones. No se observó un patrón único de lesión, la causa fue multifactorial. El tratamiento indicado depende de la lesión vertebral.
Introduction The upper thoracic spine differs from other regions of the spine for extra stability given by the complex rib sternum. Isolated fractures of the sternum are considered harmless. The association between sternum and spine fracture was first described by Fowler and more recently by Park. This association may increase the instability of the spinal injury. The objectives of this presentation are as follows: (1) analyze the kinematics causing this association; (2) describe the impact of the injury; (3) and evaluate the treatment performed. Patients and Methods Four cases are analyzed, quantifying displacement, and angulation of the sternum, and the characteristics of the spinal injury and deformity. Results Most of vertebral lesions were classified as type A, the most affected region was T5 and T10. Three patients with neurological symptoms ASIA E. The majority of the sternum injuries were caused by direct trauma. Conclusion The association of sternum and spine fracture was seen in patients who had suffered high-energy trauma during a car accident. We found no relationship between the angulation of the sternum and its translation to the degree of kyphosis in the thoracic spine. It is important to do good radiographic studies involving the sternum to suspect this relationship.
Introducción: La espondilitis anquilosante es un trastorno inflamatorio progresivo que afecta el esqueleto axial, inclusive las articulaciones sacroilíacas; el riesgo de sufrir una fractura se cuadruplica (10% a los 10 años de enfermedad), la tasa de demora del diagnóstico es alta. La rigidez y la osteoporosis son factores clave para sufrir estas lesiones. La tomografía computarizada y la resonancia magnética cumplen un rol diagnóstico fundamental. La descompresión y la fijación quirúrgica es el tratamiento de elección actual. Se presenta una serie de casos con el objetivo de considerar las dificultades diagnósticas, describir las lesionesy la decisión terapéutica, analizar la presentación de complicaciones y realizar una actualización bibliográfica. Materiales y Métodos: Estudio multicéntrico retrospectivo de una serie de casos con 6 pacientes. Resultados: Seis hombres, edad promedio 58.1 años. Cuatro habían sufrido una caída desde la posición de pie. El tiempo promedio hasta el diagnóstico fue de 12.8 días. Los sectores más afectados fueron el torácico y el lumbar, con un mecanismo predominante en hiperextensión. Cuatro pacientes recibieron tratamiento quirúrgico. Conclusiones: Los pacientes con espondilitis anquilosante tienen un riesgo más alto de sufriruna fractura por traumas de baja energía. La demora para llegar al diagnóstico fue de 12.8 días. La cirugía con fijaciones largas y liberación por vía posterior es el tratamiento más utilizado. No se observaron complicaciones después del tratamiento. Nivel de Evidencia: IV
