Streptococo agalactiae y taponamiento pericárdico Sr. Director:El Estreptococo agalactiae (EA), estreptococo del grupo B de Lancefield, forma parte de la flora habitual del tracto gastrointestinal desde donde coloniza al aparato genitourinario. Las infecciones por EA son frecuentes durante la gestación y el puerperio. En este periodo puede producir infecciones tanto en la mujer (infección de la vía urinaria, endometritis, corioamnionitis, y bacteriemia periparto) como en el feto (sepsis, neumonía, meningitis, etc.). Las manifestaciones en adultos fuera de estas dos situaciones son menos frecuentes. Recientemente hemos tenido oportunidad de atender a una paciente con bacteriemia por EA que cursó con afectación multiorgánica incluyendo taponamiento pericárdi-co.Mujer de 64 años con antecedentes de hipertensión arterial, diabetes mellitus tipo 2 diagnosticada dos meses antes y en tratamiento dietético, había tenido lesión ulcerosa en pierna izquierda resuelta una semana antes del inicio de su actual sintomatología. Acudió a otro hospital por fiebre de 4 días de evolución que se acompañó de astenia, malestar general y dolor abdominal, todo ello agravado el día del ingreso. Exploración física: tensión arterial 120/70 mmHg. Auscultación cardiopulmonar: taquicardia sinusal. El abdomen era blando con dolor difuso. Analíticamente: glucosa 422 mg/dl, función renal e ionograma normales, proteína C reactiva 23 mg/dl, bilirrubina total 2,46 mg/dl, GOT 37 U/l, GPT 72 U/l, amilasa 17 U/l. Hemograma: hemoglobina 11,4 g/dl, leucocitos 10.800/ul (87% PMN), plaquetas 125.000/ul. Estudio de coagulación normal. Radiografía de torax normal. Ecografía abdominal: hígado de estasis. Se realizaron hemocultivos y se inició tratamiento endovenoso con amoxicilina/clavulánico. Éstos fueron positivos a las 48 horas para EA sensible al antibió-tico pautado. Setenta y dos horas tras el ingreso la enferma presentó signos de insuficiencia cardiaca con inestabilidad hemodinámica. Se realizó TAC toraco-abdominal y un ecocardiograma que demostraron la presencia de derrame pericárdico y pleural en tercio inferior de ambos hemitorax, por lo que se remitió a nuestro centro.Al ingreso debido a la presencia de taponamiento cardiaco precisó atención en la Unidad de Medicina Intensiva (UMI), realizándose pericardiocentesis de forma urgente; se obtuvo un líquido hemático con 180.000 hematies, 35.500 leucocitos (95% PMN), glucosa 128 mg/dl, proteínas 5,2 g/dl, LDH 2.933 u/l, ADA 3 U/l. Analítica en UMI: se objetiva hiperglucemia leve, función renal y hepática normales e hipoalbuminemia. Hemograma: anemia leve normocítica y leucocitosis con neutrofilia. Estudio de coagulación normal. El análisis del líquido pleural fue compatible con un trasudado. Tras su estabilización clínica la paciente fue trasladada a Medicina Interna, donde de se realizaron los siguientes estudios: serología a virus hepatitis B y C, VIH, toxoplasma, micoplasma, coxiella, legionella y chlamydia negativas; estudio de autoinmunidad negativo, hormonas tiroideas normales. Panhipopituitarismo secundario a traumatismo craneoencefálico Sr. Director: Presentamos el caso de un varón con panhipopituitarismo de clínica lentamente evolutiva a partir de un traumatismo craneoencefálico. Varón de 68 años, que ocho años antes del ingreso, sufre traumatismo craneoencefálico que precisa ingreso en Neurocirugía, donde se realiza TAC que evidencia hematoma orbitario izquierdo, que se resuelve espontáneamente. Desde entonces y de forma progresiva nota involución testicular, sin otra sintomatología acompañante, salvo trastornos de conducta, por lo que es diagnosticado, dos años más tarde, de esquizofrenia. Ingresa en nuestro servicio, por clínica de 15 días de evolución, de episodios de desorientación, detectándose en varias ocasiones, cifras bajas de glucemia, por lo que ingresa para estudio. El paciente niega cefalea, trastornos visuales, polidipsia, u otra sintomatología. En la exploración física, destacaba, cifras tensionales bajas (100/70), discreta palidez cutáneo-mucosa, ausencia de vello axilar y genital, y atrofia testicular, siendo la exploración neurológica, normal. Las pruebas complementarias arrojaron los siguientes datos: Hemograma normal, con VSG 96; glucosa 55 mg/100, cloro 100 mmol/l, sodio 131 mmol/l, potasio 5,1 mmol/l, colesterol total 233 mg/100 y triglicéridos 196 mg/100, siendo el resto normal. Insulina inmunorreactiva 8,4 mcU/ml, anticuerpos antiinsulina negativos, y péptido C normal. Dados los hallazgos exploratorios y analíticos se solicita estudio hormonal, detectándose un cortisol < 1 mcg/100, ACTH 29 mcg/100, T4 total 2,8 mcg/100, T4 libre 0,3 ng/100, T3 total 1 ng/ml, TSH 1,6 mcU/100, FSH 0,09 mU/ml, LH < 0,1 mU/ml y prolactina 5,9 ng/ml. Con el diagnóstico sindrómico de hipopituitarismo, se realizaron pruebas de imagen, mostrando, la radiografía de cráneo, una silla turca mínimamente aumentada de tamaño, sin erosiones en su pared. EL TAC craneal fue informado como normal, por lo que se realiza RMN, que revela una silla turca normal, así como el tallo y la glándula hipofisaria, no observándose masas a ese nivel, silla turca vacía, alteraciones en las regiones para y suprasellares ni en el quiasma óptico. El hipopituitarismo es la consecuencia de la destrucción hipofisaria y/o hipotalámica, que produce una insuficiencia de los hormonas periféricas reguladas por la hipófisis. Las causas son múltiples, siendo con mucho, las más frecuentes, el adenoma cromófobo en el adulto, y el craneofaringioma en el niño (1), seguidos de alteracio-nes vasculares, infecciosas, infiltrativas e inflamatorias. Con menos frecuencia, se debe a agentes físicos, principalmente por radiaciones y cirugia, siendo inusual las secundarios a traumatismos. La patología endocrina más común, asociado a traumatismo cráneo-encefálico, es la afectación del lóbulo posterior de la hipófisis (2); sin embargo, revisada la bibliografía, aunque conocida, han sido publicados relativamente pocos casos de fallo postraumático de la adenohipófisis (3).
