The article presents a review of the literature, visited various forms of dislocations of the shoulder joint in children, the methods of diagnostics and treatment.
ФГБУ «ФЦТОЭ» Минздрава России, Смоленск Представлен анализ лечения 15 пациентов 11-17 лет с травматической формой нестабильности плечево-го сустава, которая у детей наблюдается в форме первичного травматического вывиха плеча и привычного вывиха. Выявлены причины развития привычного вывиха плеча: повреждение губы гленоида по типу Банкарта, SLAP-повреждение, дефект Хила -Сакса, перелом суставного отростка лопатки, 3-й тип соот-ношения и ретровесия головки плечевой кости, а также дефекты лечения при первичном вывихе плеча. Хирургическое лечение выполнено 7 пациентам с привычным вывихом плеча (7 суставов). Неудовлет-ворительный результат, обусловленный 3-м типом соотношения головки плечевой кости и суставного от-ростка лопатки, наблюдался у 1 пациента (1 сустав). Ключевые слова: вывих, плечо, ребенок.

ВведениеПереломы шейки бедренной кости составляют 0,5 % общего числа всех переломов у детей. Дан-ные о частоте возникновения повторных пере-ломов шейки бедренной кости в отечественной и зарубежной литературе нами не были найдены. Наиболее часто происходят в возрасте от 5 до 14 лет и значительно реже встречаются в возрас-те от 2 до 4 лет и от 15 до 17 лет [1]. По данным ВОЗ, в 1990 г. около 1,3 млн случаев переломов проксимального отдела бедра зарегистрировано во всем мире, половину из которых составили внутрисуставные переломы [2].Более чем в 80 % случаев причина перелома шейки бедренной кости -тяжелая травма, одна-ко у 15 % пациентов перелом происходит при не-адекватной травме в случае физиологически нор-мальной активности ребенка, т. е. при падении и прыжке с высоты от 0,5 до 1,5 м, при выполне-нии «шпагатов» и «полушпагатов», при падении во время бега или игры и при прямых ударах уме-ренной силы в область большого вертела [1].Мировой практикой подтверждена высокая эффективность оперативного метода лечения переломов проксимального отдела бедра по срав-нению с консервативным. Хирургическое вме-шательство, которое показано более чем в 80 % случаев, заключается в открытой или закрытой репозиции костных фрагментов и их фиксации различными конструкциями. Несмотря на послед-ние достижения, достаточно высоким остается процент неудовлетворительных исходов лечения переломов проксимального отдела бедра. Это об-условлено отсутствием сращения, вторичным сме-щением, развитием последующего аваскулярного некроза головки бедра, ложных суставов. Ослож-нения ведут к повторному оперативному вмеша-тельству и в большинстве случаев -тотально-му эндопротезированию тазобедренного сустава в более старшем возрасте [2].Интерес к переломам шейки бедренной кости у детей обусловлен не их частотой, а частотой возникновения возможных осложнений, наиболее грозным из которых является аваскулярный не-кроз головки бедренной кости [1,[3][4][5]. Так, в за-висимости от типа перелома развитие аваскуляр-ного некроза головки бедренной кости колеблется от 10 до 100 % [6,7]. Клиническое наблюдение6 мая 2013 г. в детское отделение травматоло-гии и ортопедии ФГБУ «ФЦТОЭ» (г. Смоленск) для проведения хирургического лечения был пе-реведен пациент М., 15 лет, с диагнозом «Закры-тый перелом шейки левой бедренной кости со смещением фрагментов» (из Гагаринской ЦРБ). Из анамнеза известно, что травма произошла 2 мая 2013 г. -падение с турника. Был госпитализиро-ван в отделение травматологии Гагаринской ЦРБ. Проводилась симптоматическая терапия.
