The objective: to study the effect of short course chemotherapy regimens on treatment outcomes in children with drug resistant tuberculosis.Subjects and methods. In 2017-2019, 31 children at the age from 3 to 17 years old, received short course chemotherapy which lasted for 12-15 months. Children of both genders were enrolled in the study, they all were new pulmonary tuberculosis cases with multiple drug resistance or at risk of MDR, with no history of previous treatment with reserve anti-tuberculosis drugs, and without severe concomitant diseases. Before the treatment was prescribed, all children underwent lung computed tomography additionally to general clinical and laboratory tests.Results. The chemotherapy regimen for each child consisted of 4-6 drugs selected individually with the consideration of resistance pattern of the child or suspected index case. In all cases, the combination of drugs included fluoroquinolones (levofloxacin or moxifloxacin). Amikacin (67.7%), aminosalicylic acid (80.6%) and prothionamide (74.2%) were frequently prescribed. 54.8% of children received pyrazinamide and 48.4% – cycloserine. Given the limited lesions, only 16.1% of children received linezolid and 9.7% of children received bedaquiline. The main chemotherapy course made 13.2 ± 0.5 months (from 12 to 15 months depending on the form of tuberculosis and changes during treatment). The duration of the intensive phase made 4.8 ± 0.3 on the average. 2 (6.5 ± 4.4%) of 31 children developed adverse events requiring the cancellation of the drugs causing them.Conclusion. This study has demonstrated satisfactory tolerability and good efficacy of these short course regimens for treatment of multiple drug resistant tuberculosis. No relapses of tuberculosis were reported.
Цель исследования: оценить частоту развития гепатотоксических реакций при проведении гепатопротективной терапии по результатам генетического исследования SNPs rs1799931, rs1799930 и rs1041983 гена N-ацетилтрансферазы2 (NAT2). В исследовании приняли участие 147 пациентов в возрасте от 5 до 60 лет. В основной группе у 71 пациента проводили профилактику гепатотоксических реакций с учетом генетического исследования, а 76 пациентов (у которых профилактика гепатотоксических реакций проводилась без учета генетического обследования) составили контрольную группу. Основой схемы лечения у пациентов обеих групп были препараты изониазид в дозе 10 мг/кг/сут и рифампицин в дозе 10 мг/кг/сут. В качестве предикторов нежелательных побочных реакций (НПР) по гепатотоксическому типу, которые могут возникнуть при применении указанных противотуберкулезных препаратов первого ряда у больных туберкулезом, мы рассматривали наличие SNPs (олигонуклеотидных последовательностей) АА rs1799931 и SNPs AA и AG rs1799930, которые представляют ген NAT2, а также TT или CT SNPs rs1041983 гена NAT2. Пациентам, имеющим указанные типы мутаций в генах NAT2, в качестве профилактики назначались гепатопротекторы (урсодезоксихолиевая кислота) в дозе 10 мг/кг/сут (максимальная суточная доза-750 мг). При проведении противотуберкулезной терапии впервые выявленных больных туберкулезом с применением противотуберкулезных препаратов первого ряда (изониазид, рифампицин, пиразинамид) могут возникать НПР по гепатотоксическому типу, что часто требует отмены рифампицина и (или) изониазида. Использование в качестве предикторов НПР по гепатотоксическому типу наличия SNPs АА rs1799931 и SNPs AA и AG rs1799930, а также присутствие SNPs TT или CT rs1041983, определяющих активность фермента NAT2, статистически значимо снижает частоту развития НПР по гепатотоксическому типу при приеме противотуберкулезных препаратов. The objective: in the experiment, to study specific parameters of manifestations of hepatotoxic reactions to the combination of anti-tuberculosis drugs containing isoniazid and rifampicin in rats with different acetylation phenotypes. Subjects and methods. Liver damage was modeled in rats, old females, belonging to the phenotype of rapid acetylation, and in mature males, belonging to the phenotype of slow acetylation, with a combination of basic anti-tuberculosis drugs, which were administered for 14 days. Hepatotoxic reactions were assessed by clinical observations of rats, biochemical parameters and morphological changes in the liver, which were compared with intact animals. Results of the study: in female rats (fast acetylators of isoniazid), signs of the cytolytic mechanism of liver damage prevailed (more significant elevation of ALT activity, pronounced duration of thiopental sleep, pronounced hydropic degeneration of hepatocytes with the presence of hepatocyte necrosis, venous congestion and edema of the portal tracts). In males (with the phenotype of slow acetylation of isoniazid), a significant elevation of total and direct bilirubin levels, AST activity and, to a lesser extent, ALT were observed, i.e., signs of a mixed mechanism of liver damage (cholestatic and cytolytic).
