. Positioning of the femoral tunnel for arthroscopic reconstruction of the anterior cruciate ligament: comparative study of 2 techniques. Clinics. 2007;62(5):613-8. OBJECTIVE:To compare the accuracy of positioning of the femoral tunnel in reconstructing the anterior cruciate ligament by means of 2 techniques: tibial tunnel and anteromedial portal. METHOD: Femoral tunnels were drilled in 20 knees from human cadavers by means of arthroscopy. Group I had the femoral tunnel drilled via a trans-tibial tunnel, and Group II via the anteromedial portal. Four variables were measured: A) posterior wall thickness; B) tunnel positioning at the notch; C) tunnel inclination in relation to the femoral axis; and D) distance between the wire guide exit and the lateral epicondyle. RESULTS: As above, respectively, A) 2.23 mm for group I and 2.36 mm for group II (P =.54); B) 25.5° for group I and 30.0° for group II (P =.23); C) 23.9° for group I and 32.0° for group II (P =.02); D) 7.8 cm for group I and 3.9 cm for group II (P <.001). CONCLUSIONS: Both techniques achieved the desired positioning for the femoral tunnel entrance and satisfactory thickness for the posterior cortex. Drilling via the anteromedial portal may provide greater protection against rupture of the posterior wall.
Objective: The postoperative mechanical axis in 36 cases of total knee arthroplasty using navigated and mechanical alignment systems was evaluated and compared. All the operations were performed over the same period by the same surgeon and team, who were beginners in the navigated technique and experienced in the mechanical technique. Methods: Between 2008 and 2010, 36 total knee prostheses were performed and compared: 25 navigated and 11 mechanical procedures. Any deviation from the axis measured on the panoramic postoperative X-ray was considered positive. Results: The navigated prostheses produced a mean axis deviation of 1.32 degrees and standard deviation of 1.57 degrees and the mechanical, respectively 3.18 and 2.99 degrees. There was better alignment with a tendency towards a statistical difference favoring the navigated technique. Conclusion: There were significantly more cases with axis deviation greater than three degrees using the mechanical technique. The navigated technique was incorporated by this team without additional complications and, even without experience of navigated surgery, the initial cases achieved better alignment than with the mechanical technique, and a significantly smaller number of outliers from the three-degree safety zone.
Proximal and distal realignments produce better results than isolated proximal realignment in patients with joint degeneration or with greater duration of disease. The realignment surgery does not produce good results in patients with advanced disease.
RESUMOO objetivo deste trabalho é verificar se o ângulo de inserção das âncoras de sutura interfere na sua resistência quando submetidas à uma força de tração constante.Foram realizados 20 ensaios com âncoras metálicas de sutura inseridas em 4 angulações diferentes (30°, 45°, 60°, 90°), em osso de cadáver humano (fêmur distal) e mais 20 ensaios em osso artificial (Sawbone TM ). Os testes foram realizados na Máquina Universal de Ensaios modelo Kratos, sendo as âncoras tracionadas a 90° em relação ao seu plano de inserção na superfície óssea.Os valores encontrados no osso de cadáver humano não foram estatisticamente significantes, ou seja, o ângulo de inserção não influiu na resistência à tração. No osso artificial verificou-se que houve maior resistência na inserção a 90 o , com relevância estatística em relação aos demais ângulos testados.Descritores: Âncoras; Biomecânica; Resistência à tração INTRODUÇÃODevido às facilidades técnicas que proporcionam, as âncoras de sutura vêm sendo muito utilizadas nas cirurgias ortopédicas, tanto abertas como artroscópicas, permitindo diminuição do tempo cirúr-gico e consequentemente menor morbidade do ato operatório (7,8,15,17) . Devido ao aumento do seu uso, principalmente nas cirurgias de ombro (reparação do manguito rotador e instabilidade glenoumeral), a soltura desses implantes é uma complicação que pode ocorrer no pós-operatório precoce e mais agressivo. Vários trabalhos retratam estudos biomecânicos referentes à resistência às forças de SUMMARYThe suture anchors insertion angle and its traction resistance are the main subjects of this study.Twenty trials were realized using threaded suture anchors in four diferents angulations (30 º /45 º /60 º /90 º ) in human bone (distal femur) and another twenty trials in artificial bone (Sawbone TM ). The anchors were pulled out being tractioned uprightly from its bone surface by a Kratos Universal test machine.The human bone results found no relation between the main subjects of this study, so whithout statistical value. On the other hand at the artificial bone the insertion angle of 90 º beared more traction, being statistically significant compared to the other angles.
Anterior knee instability associated with a varus deformity is a complex condition with several treatment possibilities. Among these, anterior cruciate ligament (ACL) associated to a simultaneous valgus tibial osteotomy is a increasing indication. This simultaneous procedure adds technical issues to those related to the isolated surgeries. Thus, the osteotomy plane and location of fixation hardware shouldn’t conflict with tibial tunnel and ACL graft fixation. Authors analyze the relations between a opening tibial valgus osteotomy stabilized with a Puddu plate and ACL reconstruction with a patellar tendon graft fixated with interference screws in 10 human cadaver knees. A straight oblique tibial osteotomy starting on the medial tibial cortex and oriented laterally and proximally was performed on all knees with a 10mm opening medially and stabilized with a Puddu plate on the most posterior aspect of the medial tibia, and a tibial tunnel drilled 50° to tibial plateau. With this technique there was no intersection between tibial tunnel or interference screw and the osteotomy or the plate fixation screws.

No Serviço do professor J. Alves de Lima, tivemos a opportunidáde de observar, nos doentes a cargo do Dr. Dario de Carvalho Franco, juntamente com o Dr. Mathias Roxo Nobre, estes casos que julgamos dignos de ser apresentados aos collegas. OBSERVAÇÃO I S. P., 19 annos, solteiro, commercio, MOLÉSTIA ACTUAL: Ha 7 dias sentiu uma leve dõr na região inguinal direita, mais ou menos no meio da arcada de Poupart; dor que attribue aos gânglios que ahi são palpáveis "ingúa". No dia seguinte, pela manhã, jogou futebol, sentindo a tarde uma dor no mesmo local, cuja intensidade augmentou á noite. No dia seguinte a dôr attingiu a fossa illiaca, a coxa e o joelho. Sentia essas regiões geladas embora os circumstantes verificassem-nas quentes.Permanece com a perna fletida sobre a coxa, e esta sobre a bacia, não só por sentir nessa posição menor dôr, como por não ter forças nem mesmo para mover o membro. Duas noites passou sem dormir devido as dores. A região glutea é dolorosa.ANTECEDENTES PESSOAES: Sarampo, catapóra e febre typhoide na infância. Na idade de 9 annos, tendo em uma queda fracturado um dente, appareceu uma hemorrhagia, que embora ,, REVISTA DE MEDICINA 36 fosse tratada severamente por um medico, não cessou senão 10 dias depois com applicaçãe de nitrato de prata, feita por um ou ; £\n3£! Ambofdisseram que o paciente era "hemophihco-Ha 3 annos, não precedida de nenhum trauma conhecido, apparefeu uma dôr localizada no flanco e fossa ilhaca direita, que attin-E iu o seu máximo no fim de dois dias. Neste hospital, onde se infernou lhe diagnosticaram "pertubação hepatica , sendo-lhe applicada bolsas quentes no local, ficando bom ao cabo de 7 dias. Ha 1 anno, após uma .queda, appareceu dôr, augmento de volume, e impossibilidade de movimento na articulação da anca direita, com uma echymosis em seu nivel. Quando soffre traumatismo nítidos apparecem manchas echymoticas na pelle. Sem passado venereo-syphilitico. EXAME PHYSICO: Facies de soffredor e posição no leito descripta. Temperatura: 38,8°. A tentativa de extensão da perna provoca dôr. Na metade inferior do hemiabdomen direito, ha resistência muscular. Comtudo, é possível, pela palpação, conhecer um tumor, localizado profundamente, cujos limites, o superior attinge uma linha que passa pela cicatriz umbelical, o interno não attinge a linha mediana, o externo e o inferior, são mal delimitados, mais superficial em sua parte externa, doloroso a palpação e endurecido, de superfícies lisas e não pulsatil.A punção do tumor revelou a presença de um liquido hemorrrhagico com numerosos coágulos. Exames de laboratório. EvoluçãoDe 1 a 5-6-33: o tempo de sangria variou de 5' a 19'; e o tempo de coagulação de 11' a 50' e empregou-se quotidianamente 20 c. c. de autohemotherapia.Em 5-6, irradiação do baço e em 7-13 e 15-6 usou-se o autohemotherapia e a sangria variou de 5' a 9' e a coagulação de 3' a 12'. Posteriormente deu-se inicio a diathermia e massagens. Deixou o Serviço com bôa locomoção, embora se queixasse da parte interna da coxa esquecida e o membro enfraquecido, soffrendo nve...
