O período gestacional não é isento de infecções que comprometam a saúde materno-fetal, de forma especial aquelas que se apresentam assintomáticas ou subclínicas. Nessas situações, o tratamento, em geral, não seria necessário. Entretanto, os riscos da transmissão materno-fetal (vertical)
Objetivos: estabelecer a freqüência da toxoplasmose aguda em gestantes, a taxa de transmissão vertical e o resultado perinatal dos fetos infectados. Objetivou-se, ainda, avaliar a relação entre os principais testes materno-fetais de diagnóstico da toxoplasmose durante a gestação, bem como a relação entre faixa etária e a infecção aguda pelo Toxoplasma gondii. Métodos: estudo prospectivo longitudinal com 32.512 gestantes submetidas à triagem pré-natal pelo Programa de Proteção à Gestante de Mato Grosso do Sul, no período de novembro de 2002 a outubro de 2003. Utilizaram-se método ELISA (IgG e IgM) e teste de avidez de anticorpos IgG para diagnóstico da toxoplasmose materna, e PCR no líquido amniótico, para diagnóstico da infecção fetal. A avaliação das variáveis foi feita pelas médias, ao passo que a correlação entre algumas variáveis foi avaliada pelo teste do χ 2 e teste de Fisher bicaudado em tabelas de contingência de dupla entrada. Resultados: encontrou-se freqüência de 0,42% para a infecção aguda pelo T. gondii na população de gestantes, sendo 92% delas expostas previamente à infecção e 8% suscetíveis. Nas gestantes com sorologia IgM reagente, a faixa etária variou de 14 a 39 anos, com média de 23±5,9 anos. Não houve relação significativa estatisticamente entre faixa etária e infecção materna aguda pelo T. gondii (p=0,73). Verificou-se taxa de transmissão vertical de 3,9%. Houve relação estatisticamente significativa (p=0,001) entre o teste de avidez (IgG) baixo (<30%) e presença de infecção fetal, e ausência de toxoplasmose fetal quando a avidez apresentava-se elevada (>60%). Houve associação significativa estatisticamente (p=0,001) entre infecção fetal (PCR em líquido amniótico) e infecção neonatal. Conclusões: a freqüência da toxoplasmose aguda materna apresentou-se abaixo do observado em outras investigações no Brasil. Entretanto a taxa de transmissão vertical não foi discordante do encontrado em outros estudos. O teste de avidez dos anticorpos IgG, quando associado à idade gestacional e data de realização do exame, mostrou-se útil para orientar a terapêutica e avaliar o risco de transmissão vertical, permitindo afastá-lo quando havia avidez elevada previamente a 12 semanas. O PCR positivo foi associado à pior prognóstico neonatal, demonstrando-se método específico para diagnóstico intra-útero da infecção fetal.
Objetivos: avaliar a prevalência, características epidemiológicas (idade e procedência) e a taxa de transmissão vertical da infecção pelo HTLV I/II em gestantes submetidas à triagem pré-natal de acordo com o Programa de Proteção à Gestante do Estado de Mato Grosso do Sul. Métodos: estudo descritivo transversal que incluiu 32.512 gestantes submetidas à triagem pré-natal no período de novembro de 2002 a outubro de 2003. Todas as gestantes da amostra foram submetidas aos testes sorológicos pelo método ELISA para o diagnóstico da infecção pelo HTLV, sendo os casos positivos confirmados pelos métodos Western blot e/ou PCR. O diagnóstico neonatal de infecção congênita foi realizado pela pesquisa de anticorpos anti-HTLV I/II confirmados por Western blot e PCR. A relação entre as variáveis (idade e procedência) foi avaliada pelo teste do χ 2 , em tabelas de dupla entrada, considerando p<0,05 para rejeição das hipóteses de nulidade (não existência da associação entre a idade materna e infecção pelo HTLV I/II e associação entre procedência e positividade para a infecção pelo HTLV I/II.). Resultados: encontrou-se prevalência de 0,1% de gestantes infectadas pelo HTLV I/II (37) dentre as 32.512 pacientes triadas. A média de idade das gestantes infectadas pelo HTLV foi de 25,4±6,4 anos, sendo que houve predomínio de pacientes procedentes do interior do estado (78,4%). Não houve associação da idade com faixa etária materna e procedência. Apenas 8 (21,6% da amostra) recém-nascidos foram avaliados quanto à presença da infecção congênita pelo HTLV I/II, todos com infecção congênita confirmada. Apenas 1 recém-nascido (9%) recebeu amamentação natural. Conclusões: a prevalência da infecção pelo HTLV I/II em gestantes sul-matogrossenses foi menor que os valores encontrados em estudos com gestantes de países endêmicos. No entanto, esteve próximo às taxas encontradas em países considerados não endêmicos e em alguns estudos brasileiros. A transmissão vertical ocorreu em 100% da amostra avaliada, mesmo a amamentação sendo proscrita. Verificou-se a necessidade de aprimorar o seguimento das gestantes e seus recém-nascidos no estado, uma vez que a minoria dos recém-nascidos foram investigados quanto à ocorrência de transmissão vertical do HTLV. RESUMOLeishmaniose visceral (LV) é doença endêmica no Estado de Mato Grosso do Sul, Brasil, principalmente nas áreas próximas dos rios Paraguai e Paraná. Nos últimos anos vem aumentando o número de casos, especialmente na Capital, Campo Grande, com conseqüente surgimento de casos em gestantes e conseqüentemente havendo risco de transmissão vertical do parasita. No presente trabalho é relatado um caso de LV em gestante, acompanhada por nosso grupo e tratada com anfotericina B lipossomal, não tendo ocorrido a transmissão vertical do parasita. O presente relato demonstra opção terapêutica em casos de calazar durante a gestação, tendo em vista a contra-indicação relativa do uso do antimoniato, droga de primeira escolha para tratamento em pacientes não-gestantes. ABSTRACTVisceral leishmaniasis (VL) is an endemic disease in the state of Mato Grosso do Sul, Brazil, mainly in the areas near the Paraguay and Paraná rivers. An increasing number of cases have been occurring especially in the state capital Campo Grande, with consequent occurrence of VL cases in pregnant women. This situation causes an elevated risk of vertical transmission of the parasite. In this report, we describe a case of VL in a pregnant woman followed up by our group, who was treated with liposomal amphotericin B, with no vertical transmission of the parasite. In our report, we demonstrate a therapeutic option for kala-azar during pregnancy, since antimoniate, the first-choice drug, is not prescribed during the gestational period.
