ObjectiveTo assess the relationship between periodontal treatment needs by elderly Brazilians and contextual as well as individual variables.MethodsA cross-sectional study was carried out to assess the need for clinical periodontal treatment, based on National Oral Health Survey (SB Brasil 2010) data on the presence of dental calculus, shallow (3–5 mm) and deep (≥ 6 mm) periodontal pockets, and gingival bleeding in elderly people (n = 7,619). The contextual variables included the Municipal Human Development Index (MHDI), income inequality (Gini Index) and coverage of the municipal population by the Family Health Strategy (FHS) program oral health teams.<0} The individual variables were sex, income, education level and self-reported skin color. Multilevel logistic regression models were used to calculate the odds ratio (OR) and 95% confidence intervals (CI95%) between periodontal treatment needs and the contextual as well as individual variables.ResultsGingival bleeding was found in 20.7% of the elderly analyzed (n = 1,577), dental calculus in 34% (n = 2,590), shallow periodontal pockets in 15.6% (n = 1,189), and deep periodontal pockets in 4.2% (n = 320). Individual factors were correlated with all the outcomes assessed. Sex was a protective factor in regard to gingival bleeding (OR = 0.87; CI95% 0.76–1.00), dental calculus (OR = 0.86; CI95% 0.75–0.99), shallow periodontal pockets (OR = 0.69; CI95% 0.60–0.80) and deep periodontal pockets (OR = 0.58; CI95% 0.45–0.74). It was found that fewer women needed treatment. Elderly people who self-reported having nonwhite skin had higher chances of needing periodontal treatment. Skin color was a risk factor for gingival bleeding (OR = 1.32; CI95% 1.14–1.53), dental calculus (OR = 1.32; CI95%1.14–1.54) and shallow periodontal pockets (OR = 1.27; CI95% 1.09–1.49). Education level was associated with the presence of dental calculus (OR = 0.77; CI95% 0.66–0.89), shallow periodontal pockets (OR = 0.86; CI95% 0.73–1.00) and deep periodontal pockets (OR = 0.74; CI95% 0.57–0.97), thus acting as a risk factor for undereducated elderly people. There was a correlation between population coverage by the Family Health Strategy (FHS) program oral health teams and the presence of gingival bleeding (OR = 0.67; CI95% 0.52–0.88), shallow periodontal pockets (OR = 0.76; CI95% 0.58–0.98) and deep periodontal pockets (OR = 0.62; CI95% 0.44–0.89), making these teams act as a protective factor.ConclusionsThis study showed evidence of the sociocontextual as well as individual sociodemographic characteristics influencing periodontal treatment needed by elderly Brazilians, based on the clinical features of periodontal disease. The results suggest the existence of inequality related to periodontal treatment needs among elderly Brazilians, especially in regard to sex and ethnicity, in addition to a potentially positive impact from the expansion of the Family Health Strategy (FHS) program oral health teams.
O artigo analisou as ações de Educação Permanente em Saúde na Atenção Básica em Mato Grosso do Sul, Brasil, na perspectiva de 184 equipes participantes da primeira fase do Programa de Melhoria do Acesso e da Qualidade (PMAQ), de junho de 2012 a fevereiro de 2013. Entre as ações de Educação Permanente, os cursos presenciais foram mais prevalentes, seguidos por troca de experiência, teleducação, ensino à distância e tutoria/preceptoria, tanto na capital quanto no interior. As ações de planejamento e apoio à gestão foram mais prevalentes na capital. As ações de apoio da gestão estavam diretamente relacionadas com o planejamento e a organização do processo de trabalho.
Resumo O objetivo desse estudo foi analisar, por meio de uma revisão integrativa da literatura, os possíveis impactos das crises financeiras sobre os indicadores de saúde bucal em diferentes países, bem como verificar as medidas adotadas de forma a traçar um paralelo com a realidade brasileira. Uma busca de artigos que atendessem a estes critérios foi realizada nas bases PUBMED, EMBASE, Lilacs, SCOPUS e também na literatura cinzenta. Ao final, nove estudos foram incluídos. Os resultados indicam que a população em maior vulnerabilidade, menor renda e menor escolaridade são as mais afetadas, independentemente do indicador avaliado (cárie dentária não tratada, acesso aos serviços de Atenção Odontológica e hábitos de higiene). Quando medidas protetivas com alocação de recursos financeiras foram tomadas, as disparidades diminuíram. Concluiu-se que, frente às crises econômicas, a saúde bucal passa a não ser prioridade enquanto centro nucleador de políticas, o que impacta o acesso ao cuidado dos estratos sociais menos favorecidos.
The aims of this study were to estimate the prevalence of prosthetic requirements and dental treatment needs among the elderly Brazilian population and the associations with contextual and individual determinants. Data were taken from the Brazilian Oral Health Survey (SB Brasil 2010) and a sample of 7,619 Brazilian people aged 65-74 years were investigated. Multilevel logistic regression models were used to estimate the odds ratio (OR) and 95% confidence intervals between treatment needs and contextual variables (Municipal Human Development Index, Gini coefficient and oral health coverage in the Family Health Strategy) and individual (gender, income, education and skin color). The prevalence of treatment need ranged from municipalities (p < 0.05). Of the individual variables, gender was associated with dental treatment need (OR = 1.18, 95% CI 1.05 to 1.31); lower education and non-white skin were risk factors (OR = 1.18; 95% CI 1.05 to 1.31 and OR = 1.28, 95% CI 1.15 to 1.43); the non-white skin color represented a risk factor (OR = 1.83; 95% CI 1,38- 2,42) and for the need for prosthesis income was a protection factor (OR = 0.39; 95% CI 0, 30-.51). The results showed an association between the individual factors and treatment needs.
BackgroundTutored laboratorial activities could be a manner of improving the competency development of students. However, its impact over conventional theoretical classes has not yet been tested. Additionally, different university contexts could influence this issue and should be explored.ObjectiveTo assess the impact of a tutored theoretical-practical training for teaching undergraduate students to detect caries lesions as compared with theoretical teaching activities. The impact of these teaching/learning activities will be assessed in terms of efficacy, cost/benefit, retention of knowledge/acquired competences, and student acceptability.MethodsSixteen centers (7 centers from Brazil and 9 centers from other countries throughout the world) are involved in the inclusion of subjects for this protocol. A randomized controlled study with parallel groups will be conducted. One group (control) will be exposed to a 60- to 90-minute conventional theoretical class and the other group (test) will be exposed to the same theoretical class and also a 90-minute laboratory class, including exercises and discussions based on the evaluation of a pool of images and extracted teeth. The mentioned outcomes will be evaluated immediately after the teaching activities and also in medium- and long-term analyses. To compare the long-term outcomes, students who enrolled in the university before the participating students will be interviewed for data collection and these data will be used as a control and compared with the trained group. This stage will be a nonrandomized phase of this study, nested in the main study. Appropriate statistical analysis will be performed according to the aims of this study. Variables related to the centers will also be analyzed and used to model adjustment as possible sources of variability among results.ResultsThis ongoing study is funded by a Brazilian national funding agency (CNPq- 400736/2014-4). We expect that the tutored theoretical-practical training will improve the undergraduate students’ performance in the detection of caries lesions and subsequent treatment decisions, mainly in terms of long-term retention of knowledge. Our hypothesis is that tutored theoretical-practical training is a more cost-effective option for teaching undergraduate students to detect caries lesions.ConclusionsIf our hypothesis is confirmed, the use of laboratory training in conjunction with theoretical classes could be used as an educational strategy in Cariology to improve the development of undergraduate students’ skills in the detection of caries lesions and clinical decision-making.
