Funções executivas permitem ao indivíduo resolver problemas complexos e lidar com novos contextos. Dentre elas, o planejamento é considerado uma função de alta ordem que está fortemente relacionada aos circuitos frontoestriatais, cujos déficits podem ser encontrados em diversos transtornos, como autismo, transtorno do déficit de atenção com hiperatividade (TDAH) e depressão. Um dos principais instrumentos para avaliar planejamento é a Torre de Londres (ToL), que, apesar de boas qualidades, tem apresentado problemas como efeito teto e baixa capacidade discriminativa em indivíduos normais. O presente estudo visa avaliar as propriedades psicométricas de uma versão computadorizada, brasileira, da Torre de Londres (ToL-BR). Foram testadas todas as possibilidades de itens possíveis no instrumento (35); após as exclusões dos itens com baixo poder discriminativo (rpb ≤ 0,30), ficaram na ToL-BR um total de 19 itens. Os resultados preliminares, somados a estudos anteriores, sugerem que esse instrumento se mostra mais adequado para avaliar pessoas nos níveis inferior a médio das habilidades de planejamento.
A expertise médica constitui um dos principais determinantes da qualidade da assistência médica prestada ao paciente. Desenvolve-se ao longo do curso de graduação e da capacitação profissional, dependendo tanto da aquisição de conhecimento quanto da forma como esses conhecimentos são utilizados quando da abordagem do caso clínico. Ao processamento dos conhecimentos acumulados para adequada aplicação na clínica é dado o nome de raciocínio clínico, que é categorizado, por alguns autores, como raciocínio clínico não analítico ou automático (sistema 1) e raciocínio clínico analítico ou reflexivo (sistema 2). O sistema 1, comumente utilizado nos casos rotineiros, caracteriza-se por requerer pouco ou nenhum esforço mental na solução do caso, sendo rápido e intuitivo. O sistema 2, por sua vez, utilizado para solução de casos menos comuns ou complexos, é elaborado, consciente e lento. Embora ambas as formas de raciocínio clínico sejam igualmente eficazes na solução dos casos, o médico deve estar capacitado para utilizar uma ou outra, de acordo com a sua própria experiência prática e com a complexidade do caso abordado, para garantir maior acurácia diagnóstica e melhor abordagem ao paciente.
Introdução: A Torre de Londres (Tower of London – ToL) é uma tarefa neuropsicológica desenvolvida para avaliação da capacidade cognitiva de planejamento. Embora seja amplamente utilizada na neuropsicologia, existem evidências de que suas propriedades psicométricas são insatisfatórias. Objetivo: Comparar a versão original da tarefa com uma versão computadorizada e ampliada. Métodos: Foram submetidos à versão original e à computadorizada 91 adultos brasileiros (média = 37,6 anos; desvio padrão = 12,2 anos) com um mínimo de 8 anos de educação formal. Resultados: Foi observada uma correlação moderada entre a pontuação nas duas versões (rho = 0,42; p < 0,05). A versão tradicional apresentou consistência interna baixa (α = 0,471) em comparação à versão computadorizada (α = 0,771). Conclusão: Os resultados sugerem a ausência de equivalência entre as duas versões, sendo a computadorizada e ampliada mais robusta em termos psicométricos e com bom potencial para uso na clínica e pesquisas em neuropsicologia.
