Background: Adverse drug events (ADE) and medication errors (ME) provide large numbers of victims. Older people are more susceptible to these events, due to the continuing search for several chronic degenerative disease treatments. The Third Global Patient Safety Challenge announced the objective of reducing unnecessary polypharmacy, encouraging deprescription, and aiming to ensure the prescription of medications in an appropriate manner, based on the best evidence and taking into account the individual factors of people. Objective: To evaluate whether the Pharmaceutical Care (PC), when inserted in a geriatric ward and in the context of person-centered health care, cooperates with the safety of pharmacotherapy in older individuals in Brazil. Methods: This is an investigative, single-arm, preliminary study. Inclusion criteria: individuals aged ≥60 years and admitted in the geriatric ward between August 2019 to January 2020. The PC (with the practice of pharmacotherapeutic follow-up, medication reconciliation, and pharmacotherapy review) was made available to identifying ADE and ME, as well as the associated factors and clinical outcomes, were analyzed. Results: 60 participants were included. It was found that, on hospital admission, 93.3% of them were polymedicated and 86.7% had a history of using potentially inappropriate medications (PIM). ADE and ME were detected in 43 individuals (71.7%) and, in total, 115 incidents were identified, with drugs that act on the nervous system associated with them (31.9%). Acceptance of the PC's recommendations reached the rate of 85.2%. Polypharmacy (p=0.03) and the presence of multiple diseases (p=0.03) had effect on the presentation of ADE and ME. The number of medications in use decreased in the comparison between admission and hospital discharge (p<0.0001). Conclusion: This investigative study indicated that ADE and ME are linked to the polypharmacy in use at the beginning of hospitalization. On the other hand, we showed that the PC (inserted in the multidisciplinary team) contributed to the deprescribing of medications at hospital discharge. Therefore, the PC can provide improvements in this scenario. Trial registration: Brazilian Registry of Clinical Trials (registration number: RBR-34f2px4).
Introdução: Reações adversas e os incidentes associados com medicamentos ocasionam a morte e ameaçam a segurança do paciente. No Brasil, persiste-se uma escassez de dados epidemiológicos e uma abordagem ainda insuficiente. Objetivo: Descrever a frequência e as características das notificações de queixas técnicas e reações adversas a medicamentos (RAM) em um hospital regional. Métodos: Estudo transversal, realizado em um hospital de ensino referência para o noroeste do Estado de São Paulo. Por meio do sistema Notivisa da Anvisa, foram contabilizadas as notificações de RAM e queixas técnicas do período de junho de 2012 a julho de 2014. Excluíram-se as não finalizadas e com dados incompletos. Os casos de RAM foram classificados de acordo com o critério de gravidade, sistema de órgãos afetado e caracterização da classe do fármaco suspeito. Ademais, as características do sexo e da idade dos pacientes envolvidos também foram investigadas. Os dados foram descritos como frequência simples e proporções. Resultados: No total, contabilizaram-se 151 (84,8%) notificações de potenciais RAM e 27 (15,2%) de queixas técnicas. Mulheres (62,9%) com idades entre 26-59 anos (42,4%) foram predominantes entre os pacientes notificados com RAM. A maioria das reações foram distúrbios da pele (33,1%), de gravidade moderada (70,2%), provindas de medicamentos que atuam no sistema nervoso (35,8%). Sobre as queixas técnicas, o extravasamento de líquido do material de acondicionamento foi a ocorrência mais descrita (40,7%). Conclusão: As notificações associadas com medicamentos são recorrentes no âmbito hospitalar e as características relatadas embasam conhecimentos sobre o perfil clínico dos episódios adversos apresentados.
Objective: To describe the experience of the implementation of pharmaceutical care in a geriatric hospital unit and to propose an instructional protocol for the practice. Methods: Experience report that became the practice manual of pharmaceutical care in geriatrics (MaP-CFarmaGeri) of a Brazilian hospital and was structured in three topics (1. Situational diagnosis; 2. Adequacy of the procedure and service provision; 3. Practice exercise). Results: The situational diagnosis comprised the collection of data on the structure of the ward and the epidemiological profile. The pharmaceutical services provided included pharmacotherapeutic follow-up, medication reconciliation and pharmacotherapy review. The certification of the content of this procedure was attested by specialists from a multiprofessional team and the technique served more than 60 patients in practice, with good acceptance by the participants. Final considerations: The MaP-CFarmaGeri proved to be a satisfactory strategy in the implementation of pharmaceutical care in geriatrics and can support this insertion in similar locations.
RESUMO Objetivo: Descrever a experiência da implantação do cuidado farmacêutico em uma unidade hospitalar de geriatria e propor um protocolo instrutivo da prática. Métodos: Relato de experiência que se converteu no manual da prática do cuidado farmacêutico na geriatria (MaP-CFarmaGeri) de um hospital brasileiro e foi estruturado em três tópicos (1. Diagnóstico situacional; 2. Adequação do procedimento e oferta do serviço; 3. Exercício da prática). Resultados: O diagnóstico situacional compreendeu o levantamento dos dados sobre a estrutura da enfermaria e o perfil epidemiológico. Os serviços farmacêuticos ofertados incluíram o acompanhamento farmacoterapêutico, com a conciliação de medicamentos e a revisão da farmacoterapia. A certificação do conteúdo desse procedimento foi atestada por especialistas de uma equipe multiprofissional e a técnica atendeu mais de 60 pacientes na prática, com boa aceitação dos participantes. Considerações finais: O MaP-CFarmaGeri mostrou ser uma estratégia satisfatória na implantação do cuidado farmacêutico na geriatria e pode amparar essa inserção em locais semelhantes.
Objetivos: relatar a experiência da prática da assistência farmacêutica em um hospital do Sistema Único de Saúde (SUS) e descrever os desfechos de desenvolvimento das atividades técnicas e clínicas realizadas. Método: trata-se de uma descrição resultante da experiência de trabalho no Hospital Estadual de Ribeirão Preto, no ano de 2021. A instituição é um complexo hospitalar de médio porte que disponibiliza assistência à saúde (hospitalar e ambulatorial) para Ribeirão Preto, bem como para outros 25 municípios vizinhos no estado de São Paulo. Os serviços da assistência farmacêutica, que se caracterizaram por práticas operacionais e clínicas, foram executados por nove farmacêuticos e por uma auxiliar de farmácia. O relato do cotidiano do setor foi, então, detalhado como uma experiência exitosa no SUS. Resultados: a assistência farmacêutica estava estruturada por meio de uma política institucional de segurança com relação ao uso de medicamentos, que padronizava a seleção e distribuição dos mesmos, levando em consideração as boas práticas e a adoção de métodos para evitar erros e eventos adversos. Além disso, os serviços clínicos referentes ao cuidado farmacêutico contavam com a reconciliação de medicamentos, a revisão da farmacoterapia e o acompanhamento farmacoterapêutico, que eram norteados pela prática em saúde baseada em evidências. Assim, as mais de 750 intervenções (técnicas e clínicas) realizadas proporcionaram a validação do método utilizado. Uma taxa de 91,3% de aceitação da equipe de saúde às ações da assistência farmacêutica suportaram o êxito da experiência. Conclusão: o estudo em questão, além de expor uma experiência de vida real relacionada ao desenvolvimento da assistência farmacêutica em uma instituição hospitalar do SUS, pode amparar a estruturação dessas práticas em outras unidades semelhantes.
Although underdiagnosed, delirium is a common and potentially preventable problem in older patients, being associated with morbimortality. Drugs have been associated with the development of delirium in the geriatric population and may be considered the most easily reversible trigger. Polypharmacy, prescription of deliriogenic, anticholinergic and potentially inappropriate drugs are contributing factors for the occurrence of the disturb. Furthermore, changes in pharmacokinetic and pharmacodynamic parameters, which are intrinsic of the aged process, may contribute for cognitive impairment. Identification and reversal of clinical conditions associated with delirium are the first step to treat the disturbance, as well as mitigation of environmental factors and the exposition to deliriogenic drugs. Current evidence does not support the prescription of antipsychotics and benzodiazepines for the treatment of delirium. However, the judicious use of first- or second-generation antipsychotics can be considered in severe cases. Multi-component non-pharmacological, software-based intervention to identify medications that could contribute to delirium, predictive models, tools, training of health professionals and active actions of pharmacovigilance may contribute to the screening, prevention, and management of delirium in older people. Besides, it is also important to improve the report of drug-induced delirium in medical records, to develop properly risk management plans and avoid cascade iatrogenesis.
