Bu çalışmanın amacı; infertilite nedeniyle in vitro fertilizasyon (IVF) tedavisi gören Polikistik Over Sendrom'lu (PKOS) hastalarda, dondurulmuş embriyo transferi (DET) sürecinde beslenme eğitimi vermenin gebelik sonuçlarına etkisini değerlendirmektir. Gereç ve Yöntemler: Retrospektif olarak planlanan bu çalışma Özel Hüma Kadın Hastalıkları ve Doğum Hastanesi, Tüp Bebek Ünitesi'nde yapılmıştır. Çalışmaya Ocak 2018-Aralık 2021 yılları arasında infertilite nedeniyle başvuran PKOS'lu ve IVF tedavisi gören 151 hasta dahil edilmiştir. PKOS'lu hastalar beslenme uzmanına gitme durumuna göre 2 gruba ayrılmıştır. Beslenme eğitimi almayı kabul eden 1. gruptaki 75 hastaya embriyo transfer sürecinde nasıl beslenmeleri gerektiği ile ilgili beslenme uzmanı tarafından 30 günlük eğitim ve danışmanlık verilmiştir. İkinci gruptaki 76 hasta herhangi bir beslenme eğitimi ve danışmanlık almamıştır. IVF tedavisinde DET yapılan her 2 gruptaki hastaların gebelik sonuçları birbiriyle karşılaştırılmıştır. Bulgular: Embriyo transferi yapılan hastaların toplam yaş ortalaması 27 ± 6.34 idi. Gruplar arasında yaş, boy, kilo, beden kitle indeksi (BKİ) gibi demografik ozellikler açısından istatistiksel olarak anlamlı bir farklılık bulunmadı (p>0.05). IVF gebelik sonuçları bakımından değerlendirildiğinde beslenme eğitimi almış grupta implantasyon, klinik gebelik ve canlı doğum oranları sırasıyla %56 (42/75), %46.7 (35/75), %30.7 (23/75) daha fazla bulundu ve bu fark istatistiksel olarak anlamlıydı (p<0.05). Sonuç: İnfertil PKOS'lu hastaların DET sürecinde beslenme eğitimi almasının implantasyon ve gebelik üzerine olumlu etkisi olduğuna inanmaktayız. Kan glukoz yüksekliğinin, inflamatuvar süreci tetikleyerek vücüttaki bir çok sistemi olumsuz etkilediği bilinmektedir. Beslenme eğitimi ile birlikte endometriyumdaki bu olumsuz sürecin azaldığı ve IVF sonuçlarının olumlu yönde etkilendiğini düşünmekteyiz. The aim of this study is to evaluate the perspectives of infertile patients on the Coronavirus disease-2019 (COVID-19) vaccine and to measure the effect of the COVID-19 vaccine administered on women's ovarian reserve tests and in vitro fertilization (IVF) treatment outcomes. This prospectively planned study was conducted in Hüma Gynecology and Obstetrics Hospital, IVF Unit. The study included 187 patients who applied due to infertility between April 2022 and June 2022. The patients were divided into 2 groups according to whether they had the COVID-19 vaccine or not. Follicle Stimulating Hormone (FSH), Estradiol (E2), Anti Müllerian Hormone (AMH) values, and total antral follicle counts (AFS) of 94 patients in the 1st group who accepted to be vaccinated were measured. The same parameters of 93 patients who refused to be vaccinated were also examined. The results of ovarian reserve tests and IVF treatment were compared between these two groups with and without vaccination. When the ovarian reserve tests of the vaccinated patients were evaluated before and after vaccination, no statistically significant difference was found in terms of FSH, AMH, E2, and AFS. No statistically significant difference was observed in the number of oocytes collected, the number of M2 oocytes, fertilization, implantation, and clinical pregnancy rates between the group that received the vaccine and the group that did not receive the vaccine during the IVF treatment process (p>0.05). It has been shown in our study that the COVID-19 vaccine does not reduce ovarian reserve while protecting people from infection. It has also been proven in this study that vaccination before IVF treatment in infertile patients does not adversely affect the treatment process and results.
Giriş: Ektopia kordis (EK), 0.5-1/100.000 sıklıkta nadir görülen, kalbin tamamının veya bir kısmının toraks dışında bulunması ile karakterize, bir konjenital anomalidir. Postnatal dönemde prognozları kötü olduğu için bu tür vakaların viabilite öncesi tespit edilmesi durumunda terminasyonu uygun görünmektedir. Gereç ve Yöntemler: Bu çalışmaya, 2018-2021 yılları arasında kliniğimizde EK tanısı almış 5 hasta dahil edilmiştir. Çalışmaya dahil edilen tüm hastalarda terminasyon sırasında uygulanan tedaviler, sonuçları ve komplikasyon olup olmadığı incelendi. Bulgular: Beş olguluk seride hastaların genç yaşta olduğu ve genellikle ilk gebelikleri olduğu görüldü. Hastaların hepsinde yapılan obstetrik ultrasonografi inceleme sonucunda kistik higromaya sahip nuchal translusensi artışı mevcuttu. Misoprostol uygulanması sonucu 48 saat içinde yanıt alındı ve abortuslar gerçekleşti. Sonuç: Prenatal erken dönemde tanı konulan EK olgularında gebelik sonlandırılması anne sağlığı açısından en uygun yaklaşım olarak görünmektedir. Sonlandırılmayan olgularda ise multidisipliner yaklaşım önem arz etmektedir.
