This review intends to address the importance of avulsion fractures in the pediatric patient, given that they are relatively frequent and an underdiagnosed pathology. We have searched the most recent bibliography available in concern with this topic to reveal the most important aspects of this pathology such as the pathophysiology, and the diagnostics techniques and treatments available. Muscular traction is seen to be the main cause of this particular growth plate fractures. For the diagnosis a proper interview and physical examination is needed, to explain the mechanism of production and physiopathology, we will also need the use of simple radiography and CT to characterize these lesions. As far as the treatment is concern, it will be conservative with a slowly progressive rehabilitation program in most of the cases, nevertheless some complications may ap-pear, and surgical rescue may be indicated. Surgical techniques are variable, multiple, and sur-geon-dependent, also the localization of the lesion and characteristics of the patient should count to decide the best treatment. Due to possible complications, an early diagnosis is needed.
Las fracturas de pelvis suponen un reto tanto en su diagnóstico como en su tratamiento. El manejo inicial de estas fracturas es clave a la hora del pronóstico y del correcto tratamiento posterior de estos pacientes. Además, tienen una alta morbimortalidad que en ocasiones condiciona el tratamiento definitivo. Es clave conocer la clasificación, entender el mecanismo de producción de la fractura, puesto que nos puede dar información valiosa de otras lesiones orgánicas asociadas. El tratamiento lo más precoz posible cuando el estado del paciente lo permita, es clave para obtener una reducción de la fractura posible, así como un resultado funcional óptimo. Por último, es importante no olvidarse de las secuelas asociadas a estas fracturas, que pueden condicionar la calidad de vida del paciente.
Scheuermann kyphosis debuts in young adolescents and it is defined as a fixed hyperkyphosis >40º Cobb, with associated anterior wedging of vertebral endplates of >5º in three or more adjacent vertebrae. It is thought to be caused by mechanical stress on structurally deficitary vertebral endplates. For its diagnosis, clinical history and plain radiographs are used, but also MRI when there is neurological affection. Treatment depends on the severity of the curve: those <60º can be treated orthopedically with Milwaukee bracing in combination with rehabilitation treatment (stretching or muscle strengthening) and postural hygiene. If >70º, surgical treatment is indicated using a posterior-only approach, as it reduces de apparition of complications when compared with anterior and combined approach. Vertebral osteotomies will be made depending on the DAR. We will use pedicular screws and bars, and transverse hooks to end instrumentation, as we attach distally the SSV and proximally the first kyphotic vertebra, to avoid revision surgery. Among the complications (14%) we find surgical site infection (up to 10%), neurological injury (8%), or pulmonary complications derived from anterior approach (20%)
La osteocondritis disecante es una enfermedad que aparece más frecuentemente en varones jóvenes, a nivel de la rodilla, y con mejor pronóstico a esas edades tempranas. Su etiopatogenia se considera multifactorial, influyendo la interacción de aspectos constitucionales y del metabolismo, con otros mecánicos (siendo éstos últimos, como los microtraumatismos, de mayor relevancia actualmente). El dolor a la carga es el síntoma más frecuente (80%), y para su diagnóstico nos apoyaremos en radiografías simples, Tomografía Computarizada (TC) y Resonancia Magnética (RM), pero el gold-standard es artroscópico. En cuanto al tratamiento de la enfermedad, primero se intenta actuar con terapias conservadoras, y posteriormente cobra importancia la cirugía. Existen dos bloques de tratamiento quirúrigo: el reparativo (técnicas de fijación in situ, micro-/nanoperforaciones) y el sustitutivo (injerto óseo autólogo, injertos osteocondrales de diversa naturaleza, cultivos de condrocitos y, como última opción la artroplastia). En la literatura existe gran discusión acerca del manejo, que depende en su mayoría de factores como el tamaño de la lesión, la demanda funcional del paciente o la edad, sin embargo, no hay consenso, y cada autor suele proponer un algoritmo diferente. Tampoco se ha encontrado en la literatura evidencia lo suficientemente potente que permita recomendar un tratamiento en concreta, siendo cierto que el cultivo autólogo de condrocitos parece una opción muy prometedora cuando se desarrolle por completo. Si hay algo seguro en esta enfermedad, es que hay que tratar primero la deformidad que la ocasiona (en caso de haberla), y adecuar e individualizar los tratamientos, de manera que no sobre-/infratratemos a los pacientes
