a apresentação de seu livro Salud y Enfermedad, H.San Martin (1968), sob o título "Initium", de maneira abrangente, comenta: "A vida do homem é a história da concepção, gravidez, nascimento, crescimento e desenvolvimento, felicidade e desgraça, saúde e doença e, finalmente, a morte. Recebemos um legado de nossos progenitores e a ele acrescentamos os elementos e as experiências que nos entrega o ambiente em que vivemos. Sob esse pano de fundo, desenvolve-se o drama da saúde e da doença". O que o autor refere para o indivíduo é perfeitamente aplicável para um conjunto deles, isto é, para a população. A saúde desta, alicerçada nos antecedentes próprios de sua biologia, sofre, de maneira acentuada, os efeitos do ambiente, considerado, aqui, no seu sentido mais amplo, incluindo o físico e o social.
