Diagnosis of acute appendicitis in children with equivocal signs is often difficult requiring admission for observation, many of whom are finally discharged because they do not have appendicitis. This study was designed to see whether Alvarado score can aid in diagnosis of acute appendicitis. 71 children with age in between 5-15 were scored. The decision of surgery was made independent of the score. Appendicitis was confirmed in operated cases on operative and on histopathological findings. It was found that patients with low score have little chance to suffer from acute appendicitis. Therefore Alvarado scoring system can be used in taking decision in the diagnosis of acute appendicitis in children. Case reports are the foundational 'building blocks' of scientific inquiry 7 . Many original observations were first presented as case reports. The discovery of AIDS was founded on the basis of a case report on extensively disseminated Kaposi's sarcoma in a young homosexual man published in the American Journal of Dermato-pathology in 19813.Unfortunatelyfrom late 1970sin scientific community there was a tendency to see the case report as least important and a 'second class' publication 1,8 . In Bangladesh and probably most of the peer reviewed journal all over the world; case reports are published in a separate section, not in the 'original article' sections. The authors tried to find out why a case report was not valued as an original publication or an important source of information in scientific progress. How they are valued in the scientific community was also an inspiring quarry. This article probably will help to focus on the way to answer the questions. Case descriptions suggestive of cancers were found in papyrus records in Egypt; as early as 1600 BC 2 . Even before Hippocrates case reports have provided a rich resource for teaching and research in medicine 3 . It is one of the best ways for beginners to get familiar with scholarly writing 5 . A case report may not have as much potential impact on the science or practice of health care as randomized controlled trial or other research results. However, it may be the only way to make others in the field aware of unusual presentations or complications. Most importantly it is the timehonored vehicle for teaching others. A 6 years and 6 month old child hailing from Dhaka admitted into AKMMCH with the complaints of abdominal pain for 3 days and vomiting for 1 day. Abdominal pain was in umbilical region, severe, colicky, non-radiating and aggravated after taking meal. Patient’s mother also complained about vomiting for 1 day, 3-4 times, contained ingested food particle and associated with anorexia. Patient did not pass the stool for 1 day. Patient was infested with Ascaris lumbricoids 6 months back. On examination vitals were normal, abdomen was soft and tender, there was ill defined lump present in right iliac fossa, firm in consistency, adhere to underlying structures and free from overlying skin. So it is clinically diagnosed as a case of intestinal obstruction with appendicular lump. Further investigations confirmed that it is a case of intestinal obstruction due to worm bolus. The word ‘gossypiboma’ or ‘textiloma’ is used to describe a retained surgical sponge in the body after an operation. If it is left in the abdomen, it may cause serious morbidity and mortality of the patient as well as medico legal problems. It varies between 1 out of 1000-1500 intra-abdominal operations and 1 out of 300-1000 of all operations1. Herein, we report one case, the first presenting about 2 months after caesarean section with fever with chills and rigor and pain at the operative area. Imaging detected presence of foreign body in the abdomen. Then she went for exploration of the wound and a mop was removed. A 20 year old lady presented with asymptomatic papulo-pustular eruption over upper, lower extremities and the abdomen. There was no manifestation of tuberculosis elsewhere in the body.
