Allogeneic hematopoietic stem cells in peripheral blood transplantation (alloPBSCT) or bone marrow transplantation (alloBMT) have different biological characteristics which may affect differently prognostic factors for incidence and severity of chronic graft-versus-host disease (cGVHD). To determine the prognostic factors of cGVHD in patients receiving alloPBSCT, data on 87 patients who survived at least 100 days after matched related donor myeloablative transplantation were analyzed. Factors significantly associated with higher incidence of cGVHD after alloPBSCT included CMV-positive donor, acute skin GVHD, and diagnoses other than lymphoma. Factors predictive for poor survival following cGVHD diagnosis included platelet count < 100,000/mm 3 and history of acute liver GVHD. Acute liver GVHD and etoposide in the preparative regimen significantly increased risk of death due to cGVHD after alloPBSCT. All alloPBSCT multivariate models were fit to an independent cohort of comparable matched related donor alloBMT patients (n = 75). After alloBMT, only acute skin GVHD and diagnoses other than lymphoma retained prognostic significance for predicting cGVHD. Low platelet count was the only variable predictive for poor survival in cGVHD patients after alloBMT. Acute liver GVHD was the only factor that retained prognostic significance for risk of death due to cGVHD after alloBMT. These data suggest there are some cGVHD prognostic factors that may be unique to recipients of alloPBSCT. More studies are needed to determine whether cGVHD prognostic systems should be used interchangeably in patient populations receiving different stem-cell products. Am.
Coronary Heart Disease (CHD) is the number one killer in Indonesia. The mortality rate due to CHD 17.05% of total deaths. Risk factors can be modified ie: dyslipidemia, diabetes mellitus, stress, infection, smoking habits, poor diet, lack of movement, Obesity. Unavoidable risk factors are age, sex, and family history. The purpose of this research is to know the factors related to CHD. Methods Analytical observational studies with Cossectional designs were conducted in July 2015. Data were collected by interviews and measurements of IMT and blood lipid profiles. Subjects were CHD patients as case group and non-CHD as comparison group was taken by con cement sampling by matching. Analysis of Univariate, bivariate and multivariate data. The result was that subjects with BMI ≥25 m2 had 2.7 times higher risk of CHD (CI 1.04-7.3). Physical passive activity has no effect on CHD (P 0.27). Smoking does not pose a significant risk to CHD 1.8 (CI 0.84-3.7). While consuming high fat there is a significant relationship with CHD (p: 0,29> 0,05). The most influential factors for CHD are cholesterol and triglyceride levels in the blood. The conclusion of BMI and blood lipid profile has an effect on CHD. Subjek adalah pasien PJK sebagai kelompok kasus dan non PJK sebagai kelompok pembanding diambil secara concecutive sampling dengan matching. Analisis data univariate, bivariate dan multivariate. Hasilnya subjek yang mempunyai IMT ≥25 m2 mempunyai Risiko 2,7 kali lebih tinggi terkena PJK (CI; 1,04-7,3). aktifitas pasif fisik tidak mempunyai berpengaruh terhadap PJK (P; 0,27). Merokok tidak mempunyai risiko secara bermakna terhadap PJK 1,8 (CI; 0,84-3,7). Sedangkan Mengkonsumsi lemak tinggi ada hubungan yang bermakna dengan PJK (p; 0,29 > 0,05). Faktor yang paling berpengaruh terhadap PJK adalah kadar kolesterol dan trigliserida dalam darah. Kesimpulan IMT dan profil lipid darah mempunyai pengaruh terhadap PJK. Introduction: The diabetic foot at risk is the diabetic foot which is at risk of ulceration, hence the importance of identifying the foot at this stage. This study aims to assess the prevalence of diabetic foot at risk and its associated factors among type 2 diabetes mellitus patients’ attending primary health clinics in Kuantan. Methods and methods : This was a cross-sectional study conducted at four primary health clinics in Kuantan involving 450 study participants who were selected by using universal sampling method. Foot examination was carried out and foot at risk was classified based on the Kings’ Classification. Multiple logistic regressions were performed to identify the predictors for diabetic foot at risk. Results: The prevalence of diabetic foot at risk was 31.3%. Multivariate logistic regression analysis identified age (OR 1.04, 95% CI: 1.01-1.06), smoker (OR 4.11, 95% CI: 1.96-8.63) and duration of diabetes more than 10 years (OR1.77, 95% CI: 1.05-2.98) as risk factors for diabetic foot at risk. Respondents with higher diabetic foot practice score (OR 0.87, 95% CI: 0.77-0.98) have lesser risk of developing diabetic foot at risk. Conclusion: Patients who are older, smoker and/or have chronic diabetes are predicted to be at higher risk to develop the diabetic foot at risk. This study also showed that patients with better foot care practice has lesser risk. Therefore, these are the groups of patients that need to be targeted for early detection and intervention to prevent serious complications.
This research paper investigates: (1) the educational values in local wisdom tradition (2), the internalization strategy of local wisdom values in educational interaction and (3) the implication of the internalization of Acehnese local wisdom in educational interaction in Dayah on students’ behaviour. This qualitative study chose three dayah in Aceh as the sample, they were: Dayah Bustanul Huda (East Aceh), Dayah Mudi Mesra Samalanga (Bireun), and Dayah Darusslam Labuhan Haji (South Aceh). Accordingly, the findings revealed firstly there are three major of educational values in Acehnese local wisdom in Dayah interaction, namely ta’abbudi (worship) value, akhlaqi (moral) value, and the last one is tabarruqi (seek for Allah’s blessing by doing benevolence) value. Secondly, the strategies used in the internalization of local wisdom values are (1) traditional strategy, (2) free strategy, (3) reflective strategy and (4) trans internal strategy. And the last is the implication of those values on the students’ behaviour, those are; sincerity attitude, ta’zim (honor) to teacher, togetherness, modesty, creativity, tolerance, self-reliance, discipline and mutual respect.
