La thrombose de la veine dorsale profonde de la verge (TVDPV) est une urgence rare et mal connue en urologie. Elle nécessite une prise en charge précoce symptomatique et étiologique afin de préserver la fonction érectile et d’éviter les récidives. Nous rapportant à travers notre observation un cas de thrombose veineuse dorsale de la verge révélée par un priapisme spontané non résolutif, et confirmé par un écho-doppler pénien. Apres prise en charge du priapisme et de la TVDPV, l'enquête étiologique a révélé une maladie de Behçet dont le diagnostic a été retenu sur l'association d'un critère majeur qui est l'aphtose buccale, et de 3 critères mineurs que sont: l'aphtose génitale, l'atteinte oculaire, et un test pathergique cutané positif à 24h. Un traitement étiologique a été instauré avec bonne évolution clinique, et conservation de la fonction érectile.
Le but de notre étude est de discuter la place de la chirurgie ouverte dans le traitement de la lithiase rénale, et notamment le coralliforme complexe, devant l'avènement de nouvelles techniques moins invasives. Nous rapportons une série de 53 calculs coralliformes colligés au sein de notre formation durant une période de 7 ans, de janvier 2011 au janvier 2018, traités par néphrolithotomie ouverte par lombotomie. La moyenne d'hospitalisation postopératoire était de 10 jours. Les suites postopératoires immédiates et précoces étaient simples chez 36 patients, 6 patients ont nécessités une transfusion sanguin, 2 ont représenté un sepsis sévère en postopératoire, 5 cas une infection de la paroi et 4 cas une fistule urinaire jugulée secondairement par un drainage endoscopique. Les calculs résiduels sont retrouvés dans 9 cas (16,9%). Ces calculs sont traités essentiellement par la lithotripsie extra corporelle. Les suites tardives étaient marquées par une atrophie rénale chez 2 patients, une récidive lithiasique chez 9 patients. Une amélioration de la clairance de créatinine chez 9 patients, et une légère aggravation chez 5 patients. La chirurgie à ciel ouvert de la lithiase rénale a de nombreuses complications, elle n'est pas recommandée en première ligne, mais il est important de reconnaître les patients chez lesquels une néphrolithotomie par voie ouverte pourrait représenter un choix valide de traitement.
Nutcracker syndrome (NCS) is a rare entity defined by the symptomatic hypertension of the left renal vein (LRV) compressed in the majority of cases between the superior mesenteric artery and the aorta. Hematuria, pelvic or back pain and left varicocele are the most commonly symptoms. Doppler ultrasound, computed tomography (CT) and selective left renal vein phlebography are useful to confirm the diagnosis. Treatment criteria are not well established, however for asymptomatic patients with no haematuria, the treatment remains conservative. We report the case of a 17-year-old man, with an intermittent left flank pain for 4 years and increasing volume of the left testicle for 8 months.
Les fistules scrotales sont rares, et souvent secondaires à des lésions de tuberculose. Les adénocarcinomes mucineux sont des tumeurs qui renferment au moins 50% de mucus extracellulaire, et siègent préférentiellement sur le recto sigmoïde, la localisation scrotale est inhabituelle. Nous rapportons un cas d’adénocarcinome mucineux secondaire du scrotum révélé par des fistules scrotales, chez un patient âgé de 54 ans, sans antécédents pathologiques notables, qui présente depuis deux ans des fistules scrotales à répétition, sans autres signes associés. L’examen trouve de multiples fistules scrotales avec issue de pus épais, le toucher rectal est normal. Les explorations urologiques (UIV, UCRM, Cystoscopie, …) sont normales, le bilan infectieux ainsi que la recherche de BK dans les urines et le crachat sont négatives. La biopsie de la peau scrotale est revenue en faveur d’un adénocarcinome mucineux moyennement différencié, dont l’étude immun histochimique est en faveur d’une origine primitive colorectale. L’évolution a été marqué par l’apparition de fistules anales complexes, ayant a l’IRM une caractéristique active, alimentant plusieurs collections pelvi périnéales comportant des bourgeons charnus. Sur le plan thérapeutique et vu la rareté de ce cancer, il n’y a pas de consensus. La résection chirurgicale reste le traitement de choix pour cette affection. La radio chimiothérapie pré opératoire est préconisée pour ce type de cancer mais son rôle n’est pas bien établi. Chez notre patient on a opté pour une radio chimiothérapie néo adjuvante première, avant la réalisation d’une amputation abdomino- périnéale.
We here report the case of a 75 year old patient, with lower urinary tract obstructive syndrome such as dysuria, nocturnal pollakiuria and dribbling. Clinical examination showed good general condition by examining the rectum with the finger, soft, painless prostate increased in volume with obliteration of the median sulcus. PSA level was 2,1ng/ml, prostatic weight was 240 g on ultrasound. The patient underwent surgery: transvesical adenomectomy with extraction of a giant adenoma weighing 246 g. Histologic examination showed adenomyofibroma.
