A description of handover processes in hospital: A survey from Jakarta, IndonesiaBackground: Handover at the change of nurse shift is one of an important role in patient safety and continuiting quality of care.Purpose: To optimization of implementation of handover processes in hospital: A survey from Jakarta, IndonesiaMethod: An analysis study with approach fishbone diagram, start with filling in questionnaires, observations and interviews and then analyzed using fishbone diagrams. Based on fishbone diagrams, the problem was obtained that the implementation of handover of patient patients was not optimal.Results: To optimize the implementation of handover, implementation was carried out in the form of improving SPO, improving nurses' effective communication with SBAR and making role play video handover.Conclusion: The result is the handover can be optimal with the role and function of management of the head nurse especially a directing role. Recommendations given by follow up and revising the hand over standars, make a pilot project nursing ward and evaluate the implementation of the patient's handover through regular supervision.Keywords: Fishbone Diagram; Handover; Head Nurse; Optimization; DirectingPendahuluan: Serah terima perawat antar shift jaga merupakan salah satu komponen penting dalam mencapai keselamatan pasien dan mempertahankan kualitas asuhan.Tujuan: Meningkatkan optimalisasi fungsi pengarahan kepala ruang dalam pelaksanaan serah tertima antar shift di RS X Jakarta.Metode: Menggunakan study analysis situasi dengan pendekatan diagram tulang ikan, dimulai dengan mengisi kuesioner, observasi dan wawancara dan kemudian dianalisis menggunakan diagram tulang ikan. Berdasarkan diagram tulang ikan, masalah diperoleh bahwa pelaksanaan serah terima pasien pasien belum optimal.Hasil: Untuk mengoptimalkan pelaksanaan serah terima, implementasi dilakukan dalam bentuk meningkatkan SPO, meningkatkan komunikasi efektif perawat dengan SBAR dan membuat role play video serah terima.Simpulan: Hasilnya adalah serah terima dapat dioptimalkan dengan peran dan fungsi manajemen kepala ruang khususnya fungsi pengarahan. Rekomendasi yang diberikan dengan menindaklanjuti dan merevisi SOP serah terima, membuat ruang percontohan dan mengevaluasi pelaksanaan serah terima pasien melalui pengawasan rutin.
Outpatient units have high activity and interaction, which increases the risk of neglecting full documentation of nursing care. This study aims to analyze factors contributing to the lack of optimal documentation of nursing care in an outpatient unit. This study employed a fish bone analysis approach to identify the root of problems of documenting nursing care in an outpatient unit. This research was conducted in an outpatient unit of the Children's and Mother's Hospital in Jakarta. The data collection techniques of this study were questionnaires, observations, and interviews with the head of the room, Clinical Instructors, implementing nurses, Case Managers, and Nursing Fields. The analysis reveals several results. Nurses, clinical instructors, activities in high work environments, as well as policies and tools for assessment and supervision are inadequate. The absence of effective systems and mechanisms for supervising nursing care documentation and manual documentation systems contributes to the lack of optimal documentation of nursing care in the outpatient unit. Documentation of nursing care extremely depends on the workforce, work climate, sets of policies, systems, and facilities. This study recommends programs and supervision activities for outpatient nursing care documentation performed by the Nursing Division, head of rooms, and Clinical Instructors, arranges supervision tools, arranges patients’ effective and efficient assessment documentation according to accreditation, policy re-socialization and documentation techniques, as well as energy management and implementation time documentation of nursing care in an outpatient unit. Serah terima pasien merupakan bagian dari komunikasi efektif dimana metode SBAR menjadi salah satu metode yang tepat digunakan dan sudah diterapkan di RS Pemerintah.i masa pandemi Covid-19, pelaksanaan serah terima harus tetap berlangsung, oleh karena itu pelaksanaan handover perlu dianalisis kembali agar rumah sakit dapat mengetahui pelaksanaan yang sesuai dan dapat diterapkan di era pandemi Covid-19. Studi ini bertujuan untuk menganalisis situasi gambaran pelaksanaan handover perawat di RS Pemerintah, menganalisis faktor-faktor yang perlu diperhatikan dalam pelaksanaaan handover di era pandemi Covid-19, dan memberikan rekomendasi pelaksanaan serah terima pasien di era pandemi Covid-19. Data diperoleh dari hasil pengkajian melalui pembagian kuesioner kepada perawat dan wawancara kepada manajer keperawatan kemudian dilanjutkan dengan analisis menggunakan tulang ikan (fishbone). Hasil analisis yang diperoleh adalah kurang opimalnya pelaksanaan handover di era adaptasi kebiasaan baru (AKB) pandemi Covid-19 di rumah sakit pemerintah. Dengan demikian, pelaksanaan serah terima di era Covid-19 dapat tetap optimal dengan terus menerapkan metode SBAR sebagai cara komunikasi efektif, pengisian formulir Covid-19 safety briefing dapat dijadikan sebagai salah satu cara untuk memudahkan kegiatan perawat, dan memperhatikan faktor-faktor dalam pelaksanaan handover di era AKB. Rekomendasi yang dapat diberikan adalah menerapkan percobaan penggunaan formulir safety briefing dalam pelaksanaan handover di salah satu ruangan dengan memperhatikan protokol kesehatan dan mengevaluasi keefektifan pelaksanaan handover di masa pandemi Covid-19 secara rutin.
