В документе представлено обновленное определение ХОЗЛ, даны новые подходы к классификации и диагностике заболевания, предложены современные схемы лечения, основанные на результатах многоцентровых международных клинических исследований, что обосновывает высокую доказательность такой терапевтической тактики. Изложены новые принципы распределения больных на клинические группы, основанные исключительно на оценке субъективных проявлений ХОЗЛ, представлены алгоритмы начальной и поддерживающей терапии для каждой клинической группы больных, строго определены показания для назначения ингаляционных кортикостероидов, основанные на учете количества эозинофилов в периферической крови, новые подходы к лечению обострений заболевания. Руководство адресовано врачам всех звеньев оказания медицинской помощи − семейным врачам, терапевтам, пульмонологам, аллергологам, кардиологам, реабилитологам. В первой части публикации руководства было представлено обновленное определение хронического обструктивного заболевания легких (ХОЗЛ), изложены новые подходы к классификации и диагностике заболевания, даны принципы комплексной оценки ХОЗЛ с учетом спирометрической классификации и риска обострений. В части 2 руководства представлены алгоритмы начальной и поддерживающей терапии больных, новые подходы к лечению обострений заболевания. Руководство адресовано врачам всех звеньев оказания медицинской помощи-семейным врачам, терапевтам, пульмонологам, аллергологам, кардиологам, реабилитологам. РезюмеВ основе патогенеза COVID-19 лежит поражение клеток организма человека главным образом вследствие непосредственного цитопатогенного воздействия вируса SARS-CoV-2, а также чрезмерной реакции иммунной системы на репродукцию вируса. Ведущим проявлением COVID-19 является поражение разных структур дыхательной системы с развитием первичной вирусной пневмонии, которая имеет свои морфологические, клинические и лабораторные отличия, а также особенности лечения.В статье приведены вирусологические свойства возбудителя коронавирусной болезни COVID-19, представлена патоморфологическая картина и рентгенологическая семиотика поражений легких. С целью разделения первичной вирусной и вторичной вирусно-бактериальной пневмонии предложено использование ряда клинико-инструментальных и лабораторных дифференциально-диагностических критериев.Важное место в лечении больных с первичной вирусной пневмонией занимает противовоспалительная патогенетическая терапия, которая включает применение глюкокортикостероидов. При возникновении вторичной вирусно-бактериальной пневмонии назначение антибактериальной терапии целесообразно осуществлять согласно национальных рекомендаций, принятых для лечения пациентов с внебольничной пневмонией. Среди приоритетных направлений научных исследований -создание вакцин против COVID-19. Медико-организационный кризис, вызванный вспышкой коронавирусной инфекции, указывает на необходимость усовершенствования противоэпидемических мер на уровне медицинского учреждения, страны и мира, модернизации систем здравоохранения и пересмотра их финансирования.
