Introduction. The features of the development of infrastructures, regional resource and methodological centers for early care for children and their families in the Russian Federation, the specifics of identifying children of the target group and families in need of early care services are considered.Goal: To develop scientifically based proposals for the further development of early care for children and their families in the Russian Federation.Materials and methods: Information from Federal Statistical Observation No. 1-RP "Information on early care for children of the target group" from 85 regions of the Russian Federation (hereinafter referred to as Form No. 1 RP). General scientific methods (content analysis, analytical, system-structured, comparative, statistical) and methodological methods of systematization, comparison, generalization, interpretation using the standard Excel-7.0 software package for IBM PC are applied.The object of the study: Children from birth to three years of age who need early care services.The subject of the study: Early care services provided for children of target groups and their families in the constituent entities of the Russian Federation.Results. Organizations providing early care services operate in all 85 constituent entities of the Russian Federation. According to statistical observation, in the period 2019–2021, there is an increase in the number of organizations providing early care services (in general, an increase of 21.4% was registered in the Russian Federation), regional resource and methodological centers of early care were established in 53 constituent entities. There is an increase in the number of organizations from the healthcare sector (by 3.8%), in the education sector (by 16.5%), in the social protection of the population (by 2.3%). Early care services for children and their families are provided in 17 constituent entities of the Russian Federation by non-governmental organizations. In the Russian Federation, 75 regional resource and methodological centers for early care for children and their families have been established in 53 constituent entities of the Russian Federation, 12 of them in the field of healthcare, 27 in education, 36 in social protection of the population. The number of children assigned to the target group in the Russian Federation amounted to 248,108 people (4% of the total number of children aged from birth to the age of three). The proportion of children in need of early care services in the constituent entities of the Russian Federation in 2021 varies from 0.3% to 23.4%. According to the data of the constituent entities of the Russian Federation, as of 31.12.2021, the number of specialists providing early care services amounted to 38,484 people.Discussion. Since 2019, the constituent entities of the Russian Federation have been monitoring the state of early care for children and their families in the form of statistical observation No. 1-RP "Information on early care for children of the target group". Information is provided from 85 constituent entities of the Russian Federation. According to statistical observation data, as of 31.12.2021, 6429 organizations provide early care services in the Russian Federation. The formation and development of the infrastructure of early care services and the improvement of human resources are necessary conditions for the development of early care for children and their families in the constituent entities of the Russian Federation. There is a positive trend in the growth of the number of specialists providing early care services. The results of the analysis of the statistical form No. 1-RP allow us to assess the state and level of development of early care for children and their families in the constituent entities of the Russian Federation.Conclusions. The data from the monitoring results allow us to develop proposals for the development of early care services in the constituent entities of the Russian Federation.
Introduction. Currently, assisted living technologies are actively developing in the Russian Federation. When receiving social services, education, promotion of employment, disabled people need to be accompanied. The effectiveness of these measures depends both on the conditions for the provision of services, the qualifications of accompanying persons, determining the required amount of support, on the ability of a disabled person to live independently and work, and on the readiness of the disabled person to be accompanied when receiving services, including a joint venture.Target. Rationale for the use of the International Classification of Functioning, Disabilities and Health in assessing the degree of readiness of persons with disabilities to be accompanied when receiving services, including accompanied accommodation.Materials and methods. Analysis of the results of monitoring the development of joint venture technologies, presented by 85 constituent entities of the Russian Federation for the period 2020–2021 on a wide range of issues, including the organization of employment of people with disabilities who are on a permanent SP and employed people with disabilities, social employment of people with disabilities, training of people with disabilities who are on a permanent accompanied accommodation, organizing a accompanied accommodation in stationary social service institutions, etc., using the ICF in determining the readiness of people with disabilities to be accompanied.Results. The number of disabled people receiving services within the framework of accompanied accommodation was studied based on materials submitted from 85 subjects of the Russian Federation as of 01.10.2022. The monitoring analysis showed that in 80 subjects of the Russian Federation (94.1 %), work is underway to organize accompanied accommodation. In 2022, there is an increase in the number of disabled people receiving services within the framework of accompanied accommodation to 6649 people, of which 4245 people are on educational (training) accompanied accommodation, 2404 people are on permanent accompanied accommodation.Discussion. Using the methodology of the ICF, questionnaires were developed, as well as a methodology for assessing the readiness of persons with disabilities to be accompanied when receiving services, including accompanied accommodation, carried out using them.Conclusions. The developed criteria and methodology for their use make it possible to quantify the degree of readiness of persons with disabilities and children with disabilities to be accompanied when receiving services, including accompanied accommodation, and be used in the preparation of an individual support program.
On the basis of studying of statistical data, medical documents, results of the carried-out questioning in 2012-2015 the specific weight of the patients who have transferred amputation of the lower extremity owing to complications of diabetes and the obliterating diseases of vessels of the lower extremities, their distribution for the reason and level of amputating defects is estimated. Circulation periods of citizens on primary prosthetics of the lower extremities are studied and analysed. The problems of primary prosthetics connected with presence at patients of the accompanying pathology of the blood circulatory system, breath and the musculoskeletal device are designated. The prevalence of elderly people with amputating defects of the lower extremities is revealed. Features of prosthetics for elderly people, which have decreasing of weight-bearing on safe lower extremity, physical weakness, decreasing of the cognitive functions leading to difficulty of use of technical means of rehabilitation, decrease in learning ability to skills of use of artificial limbs is characteristic are specified. Problems of early rendering the prosthetic and orthopedic help and primary prosthetics of patients after amputation of the lower extremities in the megalopolis are considered. The new technology of early primary prosthetics of the lower extremities developed by specialists of Federal Scientific Center Rehabilitation of the Disabled named after G.A. Albrecht is presented by express method. Results of introduction of modern techniques of prosthetics by express method and their value for rendering primary parotezno-ortopedicheskoyy the help in the megalopolis are discussed. This type of the help can be rendered before formation of the individual program of rehabilitation and an abilitation of disabled people in the conditions of the medical organizations when rendering out-patient and stationary medical care. Rendering early primary prosthetic and orthopedic help is feasible when healing a wound of a stump after amputation, lack of contraindications to prosthetics, high motivation of the patient. Advantages of this technology of early primary prosthetics by express method is restoration of ability of the patient to movement and self-service, decrease in dependence on assistance and leaving, prevention of emergence of some defects and diseases of a stump at uluchsheny the general condition of an organism.
For the effective development of the social state, it is necessary to take measures to improve the quality and life expectancy of citizens on the basis of improving health-saving technologies, which requires proper scientific support. The purpose of the work is a retrospective and prognostic study of the Russian publication scientific stream on rehabilitation and scientometric indicators for a reliable assessment of its dynamics. A high (third) place of Russia in the international publication rating from 53 countries in 1991 under the «Rehabilitation» subsection in the Web of Science database was revealed, which was replaced by a substantial, fourfold, downgrade in subsequent years, and an increase in the array of publications in this field of knowledge in the last 5 years, which, however, did not lead the country to the number of leaders. The growth of the national publication stream in 2018 to the 20th place among 100 countries in the Web of Science database and the 15th place among 152 in Scopus is associated with the results of the implementation of government measures. It is proved that the share of scientists in the global array of publications registered in highranking databases is a more objective indicator of publication activity than their country’s place in the ranking. Improving the quality of scientific support for the development of rehabilitation technologies in the context of an increasing proportion of elderly and senile people, growth of diseases with a long life span, increasing demands on the level of self-care and mutual care of the population due to urbanization and small families will contribute to the formation of a socially stable society and increase national security of Russia. A prognostic model is proposed for changing the volume of Russian publications on rehabilitation, according to which the 1991 level in the Scopus database is expected in 2020, and in the Web of Science — only in 2028.
