Thirty years experience of treatment of soft tissue desmoid fibroma (Moscow Oncologic Institute) is presented. Efficacy of surgical treatment versus combined and radiation therapy was studied. The rate of relapse of extraabdominal desmoids after surgical treatment was 94%, after combined treatment with preoperative radiation therapy the relapse rate decreased up to 27.5% but after postoperative radiation therapy up to 53%. After radiation therapy the relapse rate was 15.7%. However radiation therapy has limitations related the necessity of massive tumor irradiation and application of high doses of irradiation that may cause postradiation affect of tissues. During subsequent search the original method of chemiotherapy (tamoxiphen, vinblastin, methotrecoat) was tested. This method allows to achieve the persistent remission in 81.3% of patients. Principles of therapeutic tactics in desmoid fibromas are formulated.
Antitumor efficacy of the combined suicide gene therapy and radiotherapy was studied on the model of CT26 murine colon adenocarcinoma. CMV-FCU1-IRES-mGM-CSF-pGL3 construct with PEG-PEI-TAT (FCU1-mGM/5-FC) block copolymer as a vector was used for intratumoral administration. Tumors were irradiated with a single 5 Gy dose. The efficacy was evaluated according to the grade of tumor growth inhibition (T/C) and lifespan of the animals. Pronounced antitumor activity of the combined use of FCU1-mGM/5-FC system with radiotherapy on the background of prolonged lifespan and the synergism of the applied methods was revealed.
Background: A cognitive reserve level has a significant impact on changes in cognitive performance in the process of natural aging. The cognitive reserve level consists of multiple variables, which are often shaped by the current sociopolitical environment, including a socioeconomic status, the level of education, and occupation. Aim of the study: A comparison of the cognitive reserve and selected cognitive abilities of seniors from different cultures (Poland and Ukraine), and the analysis of the impact of both variables (cognitive reserve and cultural background) on the level of current cognitive abilities. Material and methods: The study was conducted in two countries (Poland and Ukraine) and included 30 participants aged 60-93. Research tools used in the study were the following: Montreal Cognitive Assessment (MoCA), subtests: Vocabulary and Digit Span from Wechsler Adult Intelligence Scale-Revised (WAIS-R), Written Metaphor test in RHLB-PL (Right Hemisphere Language Battery), semantic and emotional fluency tasks, Benton Visual Retention Test, Satisfaction with Life Scale (SWLS), Beck Depression Inventory-II (BDI-II) and the author's questionnaire for the cognitive reserve assessment. Results: Polish seniors obtained a significantly higher overall score in MoCA, verbal fluency tasks, Benton Visual Retention Test and in MoCA subtests: Language, Abstraction and Delayed Recall. The results of a two-way analysis of variance showed a significant effect of the cultural background and/or the cognitive reserve on the level of language skills (MoCA: Language), abstract thinking (MoCA: Abstraction), and the level of depression (BDI-II). Conclusions: Cognitive reserve factors show a diverse relationship with different spheres of human functioning in different cultures, and thus their role in successful aging may be various.

Герцена-филиал ФГБУ «Национальный медицинский исследовательский центр радиологии» Минздрава России, Москва, Россия Цель исследования-ретроспективная оценка эффективности хирургического и комбинированного лечения больных раком грудного отдела пищевода. Материал и методы. Проведена ретроспективная оценка результатов лечения 222 больных раком грудного отдела пищевода I-III стадии, находившихся на лечении в торакоабдоминальном отделении МНИОИ им. П.А. Герцена с 2007 по 2017 г. Больных разделили на две группы: 1-я группа-только хирургическое лечение (n=128), 2-я-комбинированное лечение (n=94). Комбинированное лечение включало предоперационную лучевую терапию с радиомодификацией (5-ФУ/цисплатин) либо химиолучевую терапию по протоколу NCCN (паклитаксел/карбоплатин) с последующей операцией. Результаты. Общая частота осложнений составила 35%, послеоперационная летальность-8%. У 60% больных в группе комбинированного лечения констатирован лечебный патоморфоз III-IV степени. Отдаленные результаты прослежены у 150 (68%) больных. Медиана продолжительности жизни в 1-й группе составила 44 мес против 38 мес во 2-й группе (разница недостоверна, p=0,34). 3-и 5-летняя выживаемость в 1-й группе составила 60 и 40% соответственно, во 2-й-60 и 33% соответственно. Проведен анализ выживаемости во 2-й группе в зависимости от выраженности лечебного патоморфоза. Медиана продолжительности жизни в подгруппе с патоморфозом III-IV степени составила 45 мес по сравнению с 21 мес в группе с патоморфозом I-II степени. При сравнении выживаемости больных местно-распространенным раком пищевода (T3 или N+) достоверное улучшение выживаемости отмечено в группе комбинированного лечения с лечебным патоморфозом III-IV степени по сравнению с группой только хирургического лечения. Медиана продолжительности жизни составила 45 и 32 мес соответственно, 3-, 5-летняя выживаемость-78 и 28% против 47 и 23% соответственно; p=0,13, log-rank. Заключение. Хирургическое вмешательство остается основным методом лечения больных раком грудного отдела пищевода, позволяющим добиться долговременной выживаемости. Операции на пищеводе сопровождаются высоким риском послеоперационных осложнений и летальностью. При местно-распространенных опухолях стандартом является комбинированное лечение, однако улучшение выживаемости отмечено только при лечебном патоморфозе III-IV степени.
