BackgroundHealth personnel retention in remote areas is a key health systems issue wordwide. To deal with this issue, since 2002 the government of Burkina Faso has implemented a staff retention policy, the regionalized health personnel recruitment policy, aimed at front-line workers such as nurses, midwives, and birth attendants. This study aimed to describe the policy’s development, formulation, and implementation process for the regionalization of health worker recruitment in Burkina Faso.MethodsWe conducted a qualitative study. The unit of analysis is a single case study with several levels of analysis. This study was conducted in three remote areas in Burkina Faso for the implementation portion, and at the central level for the development portion. Indepth interviews were conducted with Ministry of Health officials in charge of human resources, regional directors, regional human resource managers, district chief medical officers, and health workers at primary health centres. In total, 46 indepth interviews were conducted (February 3 - March 16, 2011).ResultsDevelopmentThe idea for this policy emerged after finding a highly uneven distribution of health personnel across urban and rural areas, the availability of a large number of health officers in the labour market, and the opportunity given to the Ministry of Health by the government to recruit personnel through a specific budget allocation.FormulationThe formulation consisted of a call for job applications from the Ministry of Health, which indicates the number of available posts by region.The respondents interviewed unanimously acknowledged the lack of documents governing the status of this new personnel category.ImplementationDuring the initial years of implementation (2002-2003), this policy was limited to recruiting health workers for the regions with no possibility of transfer. The possibility of job-for-job exchange was then approved for a certain time, then cancelled. Starting in 2005, a departure condition was added. Now, regionalized health workers can leave the regions after undergoing a competitive selection process.ConclusionThe policy was characterized by the absence of written directives and by targeting only one category of personnel. Moreover, there was no associated incentive—financial or otherwise—which poses the question of long-term viability.
Le mélanome est une tumeur maligne développée aux dépens des mélanocytes. Ses caractéristiques épidémiologiques restent peu étudiées dans les populations africaines. D'où le but de cette étude. Il s'est agit d'une étude rétrospective descriptive de janvier 2009 à décembre 2012 et prospective en 2013. Pour la partie rétrospective, nous avons inclus tous les patients atteints de mélanome confirmé par l'examen histopathologique et ayant un dossier clinique exploitable. Pour la partie prospective, nous avons inclus tous les cas de mélanomes recensés en 2013. Les variables étudiées étaient les caractéristiques sociodémographiques, anatomocliniques et thérapeutiques. Au total 19 patients étaient inclus, avec un âge médian de 53,42 ans et 10 patients de sexe féminin. La durée moyenne d’évolution de la maladie était de 3,7 ans. Les lésions étaient surtout ulcero-bourgeonnantes (13), hyperpigmentées (15), et siégeaient sur les plantes (14). Le type et acro-lentigineux était le plus fréquent (13). L'indice de Clarck a été mesuré dans 3 cas, l'indice de Breslow dans aucun cas. Les métastases étaient présentes chez 10 patients. Le traitement était surtout chirurgical. Les 11 patients inclus de manière retrospective étaient ont été perdus de vue. Un décès était noté parmi les 8 patients inclus de manière prospective. Le retard à la consultation semblait être le principal facteur de mauvais pronostic dans notre contexte. Une sensibilisation de la population et du personnel de santé sur les signes précoces de la maladie, permettrait probablement de réduire la morbidité.

RESUMEAu Burkina Faso, le riz occupe la 4 ème place des céréales tant du point de vue des superficies, de la production que de la consommation annuelle par habitant. Cependant, la production nationale est faible et ne couvre que 47% des besoins des populations. Au Burkina trois, types de rizicultures se côtoient ; la riziculture pluviale ; la riziculture de bas-fond et la riziculture irriguée. La riziculture pluviale offre un fort potentiel mais est mal maîtrisée avec de faibles rendements. Cette étude a été conduite sous forme d'enquête auprès de 157 producteurs afin d'identifier les potentiels et les contraintes de la riziculture pluviale dans la zone Ouest du Burkina. Les résultats montrent une récente introduction de la riziculture pluviale dans la zone. La superficie individuelle moyenne est de 0,81 ha. Les rendements moyens sont d'environ 1075 kg/ha. Les quantités d'engrais par hectare sont faibles et sont en moyenne de 44,5 kg pour l'azote, 25,5 kg pour le phosphore et 15,5 kg pour le potassium. Le riz pluvial est cultivé en pure sans rotation régulière. La faible maîtrise de l'itinéraire technique de production nécessite une formation des producteurs pour accroître les rendements et booster la production du riz pluvial. © 2015 International Formulae Group. However, local production is low and covers only 47% of the needs of the population. In Burkina three types of rice production coexist; upland rice; lowland rice and irrigated rice. Upland rice has a great potential but is poorly mastered. That leads to low yields. This study was conducted through a survey from 157 farmers in order to identify the potential and constraints of upland rice production in the western region of Burkina. The results show a recent introduction of upland rice in the area. The average of individual area is 0.81 ha. Average yields are about 1075 kg/ha. The quantities of fertilizer used per hectare are low and are about 44.5 kg for nitrogen, 25.5 kg for phosphorus and 15.5 kg for potassium. Upland rice is grown in pure without regular rotation. The low mastery of the technical itinerary production requires training of farmers to increase yields and boost the production of upland rice.
