Also in chronic ligamentous mallet finger, conservative therapy should be attempted before an operation, irrespective of the initial extension deficit, especially because the stress and the therapy risk for the patient with this therapy is small.
ZusammenfassungFragestellung: Qualitätsindikatoren müssen zum Zwecke einer effektiven und effizienten Bewertung von Qualität in der medizinischen Versorgung ihrerseits Qualitätsansprüche erfüllen. Da bislang keine allgemein anerkannte Methode der Validierung von Qualitätsindikatoren zur Verfügung steht, war es das Ziel dieser Arbeit, eine neue Methode zu entwickeln um mit ihrer Hilfe das bereits existierende Indikatorenset der operativen Gynäkologie zu evaluieren und zu überarbeiten. Methodik: Die Methodik wurde auf der Basis einer umfassenden Literaturanalyse neu erarbeitet. Die anschlieûende Validierung der Indikatoren für die operative Gynäkologie basierte auf anonymisierten Daten der Routineversorgung aus Hessen aus den Jahren 1997 ± 1999. Ergebnisse: Die Evaluation des bereits existierenden Indikatorensets der operativen Gynäkologie führte zu Empfehlungen für eine Überarbeitung bzw. Neuformulierung der Indikatoren. Schlussfolgerung: Das validierte Indikatorenset erfüllt in hohem Maûe wichtige Bewertungskriterien und sollte deshalb in der Routine zur Qualitätsbewertung in der operativen Gynäkologie eingesetzt werden.

To be effective and efficient these indicators have to meet quality criteria themselves, but at present there is no approved method for validating quality indicators. We developed a comprehensive validation procedure and used it to assess and revise the existing indicator set used in Germany for gynecologic surgery. Methods: The validation method was developed according to a review of the literature. The validation procedure was then used to evaluate quality indicators in a database of gynecologic operations performed in the German state of Hesse between 1997 and 1999. Results: The validation procedure led to modifications and redefiniton of various quality indicators. Conclusion: The revised set of quality indicators fulfills the most important methodologic criteria for quality indicators. This suggests that the revised indicators should be used for routine quality assessment in gynecologic surgery.
In 40 gynecological patients 44 different determinations of the bladder volume were made using ultrasonic methods. The product of bladder depth, height, and width, as determined from transverse and sagittal scans, showed the best correlation with the bladder volume measured by urethral catheter (r = 0.981). For 73% of the measurements the error was under 20% when the true bladder volume was > 100 cm3. This method gives a reasonable assessment of the bladder residual volume. It is quick, safe, and repeatable and, therefore, useful in postoperative clinical practice.
