The methods handbook includes a scoring system for qualitative and quantitative criteria of contrast-enhanced ultrasound (CEUS) in diff use liver diseases. Clinical cases of multiparametric ultrasound are given: B-mode + color Doppler techniques of liver vessels + strain elastography (SE) and shear wave elastography (2DSWE) + qualitative and quantitative criteria of CEUS in various nosological forms of diff use liver diseases.
It is generally agreed that nonalcoholic fatty liver disease (NAFLD) is a component of metabolic syndrome and is frequently associated with obesity, type 2 diabetes mellitus, atherogenic dyslipidemia, and other components of the syndrome. However, there is no doubt that not all overweight people develop NAFLD and, conversely, the latter may be present in normal weight individuals. The prevalence of NAFLD without obesity in different countries is very variable from 3 to 30%. Its risk factors are considered to be both exogenous (for example, excess intakes of cholesterol and rapidly assimilable fructose) and genetically determined (allelic variants of the genes encoding adiponutrin, the cholesteryl ester transport protein, sterol-regulatory element-binding protein 2). The methods for the diagnosis of NAFLD without obesity do not differ in essence from those for classic NAFLD. Analysis of the conducted investigations gives grounds to claim that lifestyle modification as exercises and dietary restrictions improves biochemical parameters and histological pattern. The efficiency of drug treatments needs further investigation.
Цель исследования: оценить возможности использования качественных параметров контраст-усиленного ультразвукового исследования (КУУЗИ) для ранней неинвазивной диагностики у пациентов с хроническими вирусными гепатитами (ХВГ). Дизайн: открытое рандомизированное сравнительное исследование. Материалы и методы. Обследованы 54 пациента с ХВГ В и С в возрасте от 31 года до 65 лет, которых разделили на две группы: с выраженным клиническим течением (n = 33) и минимальными клиническими проявлениями (n = 21). Контрольную группу составили 19 здоровых людей. Проведено мультипараметрическое УЗИ всех участников: В-режим + цветовое допплеровское картирование сосудов печени + эластография сдвиговой волны (2DSWE) + УЗИ с применением контрастного препарата 2-го поколения SonoVue в режиме Contrast c низким значением механического индекса (0,06). Согласно предложенной системе оценки качественных параметров КУУЗИ (полуколичественной), каждому признаку присваивается значение от 1 до 5 баллов. Результаты. Значение таких признаков, как симметричность накопления контрастного препарата и неоднородность контрастирования, в первой группе составило 5 баллов у 100% пациентов, во второй группе-2 и 3 балла у 80% и 20% больных соответственно, в контрольной группе-1 и 2 балла у 70% и 30% соответственно. Распределение таких признаков, как кривые ослабления визуализации контрастирования и снижение общей интенсивности контрастирования, было одинаковым у всех участников в каждой группе: 5, 2 и 1 балл соответственно. Максимальное число баллов получено в первой группе (от 28 до 30), а минимальное-в контрольной (от 6 до 8), что подтверждает работоспособность предложенной модели. Заключение. Предложенная система полуколичественной оценки КУУЗИ предназначена для диагностики состояния паренхимы печени при диффузных заболеваниях, в том числе и при ХВГ. Стандартная качественная оценка параметров КУУЗИ по стадиям менее эффективна (r = 0,58-0,69), чем предложенная полуколичественная балльная оценка этих же параметров (r = 0,78-0,93).
