Эндогенная интоксикация как типовой патологический процесс была и остается одной из ключевых проблем практиче ски во всех областях медицины и прежде всего-в реаниматологии. Накопление эндогенных токсических веществ яв ляется неотъемлемым компонентом любого критического состояния и сопровождает тяжелую травму, генерализован ные инфекции, деструкцию органов брюшной полости. Утрата функции детоксикационных систем при полиорганной недостаточности влечет за собой прогрессирование эндотоксикоза и усугубление нарушений газообмена, в том числе и в самих органах детоксикации, тем самым замыкается порочный круг танатогенеза. Эндогенная интоксикациямультидисциплинарная проблема, являющаяся интересом самых разных областей практической медицины, экспери ментальной патофизиологии и биомедицинской технологии. Благодаря этому в клиническую практику внедряются со временные методы эфферентного лечения критических состояний и в частности-экстракорпоральной детоксикации. В НИИ общей реаниматологии им. В. А. Неговского РАМН на протяжении многих лет ведется изучение теоретичес ких и прикладных вопросов, касающихся проблемы эндотоксикоза. В данном обзоре мы постарались обобщить основ ные результаты деятельности Института в данном направлении. Ключевые слова: эндогенная интоксикация, эффе рентное лечение, детоксикация. Endogenous intoxication as a typical pathological process has been and remains one of the key problems in virtual ly all fields of medicine and mainly in reanimatology. Accumulation of endogenous toxic substances is an integral component of any critical condition and accompanies severe injury, generalized infections, abdominal organ derange ment. Loss of the function of detoxifying systems in multiple organ dysfunction leads to progressive endotoxicosis and worsening gas exchange disorders, including detoxification in the organs themselves, thus closing the vicious circle of tanatogenesis. Endogenous intoxication is a multidisciplinary problem that is the area of many different fields of practical medicine, experimental pathophysiology, and biomedical technology. Thus, current efferent thera peutic techniques for critical conditions and extracorporeal detoxification in particular are being introduced into clinical practice. The V. A. Negovsky Research Institute of General Reanimatology, Russian Academy of Medical Sciences, has been studying the theoretical and applied issues concerning the problem of endotoxicosis for many years. This review attempts to generalize the main results the Institute's activity in this area.
The pancreas was examined histologically in 146 cases of patients dying of shock. The finding of morphological evidence of disseminated intravascular coagulation (DIC) in the pancreas supports the opinion that the pancreas in highly sensitive to disturbances of blood perfusion. The presence of DIC combined with a variety of pathological lesions in the exocrine ad endocrine pancreas was sufficiently characteristic to justify use of the term " shock pancreatitis". The relative scarcity of data hitherto available on pathological changes in the pancreas caused by shock can be ascribed to changes in the management of shock. It is concluded that the pancreatic lesions described in this study could influence the outcome in shock treated by modern methods of resuscitation.

Благодаря этому в клиническую практику внедряются новые эффективные методы эфферентного лечения критических состояний. В Научно-исследовательском институте общей реаниматологии им. В. А. Неговского РАМН (ныне ФНКЦ РР) на протяжении многих лет ведется изучение теоретических и прикладных вопросов, направленных на решение проблемы эндотоксикоза. В представленном обзоре мы постарались обобщить основные результаты деятельности Института в данном направлении. Accumulation of endogenous toxic substances is an integral component of any critical condition. It accompanies severe trauma, generalized infections and destructive diseases of the cavity organs. The loss of the function of detoxification systems in multi-organ failure causes the progression of endotoxicosis and the disruption of oxygen transport (in the organs of detoxification as well), hence closing the vicious cycle of thanatogenesis. Endogenous intoxication is a multidisciplinary problem being a matter of interest in different areas of modern medical science, experimental pathophysiology and biomedical technology. As a result new effective methods of efferent treatments of critical conditions are introduced into clinical practice. For many years theoretical and applied issues aimed at solving the problem of endotoxicosis have been the focus of studies in the V. A. Negovsky Research Institute of General Reanimatology of the Russian Academy of Medical Sciences (now the Federal Research and Clinical Center of Intensive Care Medicine and Rehabilitology). In the presented review we have tried to generalize the main results of the Institute's activities in this direction.
