Isolated bronchus resection for central cancer was performed in 25 patients including preoperative bronchoscopic removal of exophytic tumor in 20 (80%) observations in thoracic department of P. Gertsen Moscow Research Cancer Institute. According to morphological study typical carcinoid was diagnosed in 23 (92%) patients, atypical - in 2 (8%) cases. All patients underwent conventional mediastinal lymphadenectomy. Postoperative complications after bronchus resection developed in 6 (33.3%) patients. There were no deaths. Overall 5- and 10-year survival was 100% and 96% respectively. The authors consider that by strict indications combination of endoscopic removal with isolated bronchus resection preserves all pulmonary parenchyma without prejudice for surgical radicalism.
The paper presents the results of photodynamic therapy (PDT) of early-stage cancer of lung (17 patients), esophagus (8 patients) and stomach (10 patients). Fifteen patients had second primary tumors. New drugs photoheme and photosens were used as photosensitizers. Complete remission was obtained in 87%. The patients are followed up without relapses to 2,5 years. 1.INTRODUCTIONThe endoscopical methods of early malignansies treatment have •been developed in P.A. Hertzen Moscow Research Oncology Institute since 1984 [1]. For removal of primary and second primary tumors we used electroresection, Nd:YAG laser destruction, intracavitum radiation therapy, local chemotherapy and photodynamic therapy ( PDT). Up to now, 194 patients with early (Tis-Ti) carcinomas of larynx (17), bronchus (98), esophagus (35), stomach (36), rectum and colon (8) were treated endoscopically. The patients for endoscopical tumor removal were selected individually from the data of X-ray, computer tomography (CT), endosonography, endoscopical and morphological investigation.The indications for endoscopical treatment are as follows: limitation of tumor spreading to numcosa (submucosa); location of all tumor sites within reach of endoscope; absence of signs of metastatic spreading; multiple primary tumors; old age; hard intercurrent pathology and refusal of surgery or radiation therapy.The methods used and the sequence of their application depend on anatomical location, size and growth rate of lesion as well as on anatomical and technical capacity of each method to reach the lesion.The paper presents the results of PDT of early-stage cancer of lung, esophagus amid stomach (35 patients, 39 tumors).PDT is a new method of cancer treatment which is currently evaluated in the course of clinical trials2'3'4. This method combines the use of a photosensitizing drug, laser light and fiber optics for destruction of malignant tumors with the least damage of normal surrounding structures. At present, PDT is indicated mainly for the following: hard dysplasia of mucous of internal organs; early stage cancer; malignant tumors of various localization, resistant to radiation and chemotherapy; recurrences, metastases of cancer in uncureable patients; multiple primary tumors.The basis of fluorescence diagnostics of tumors is selective retention in malignant tissues of photosensitizers used for PDT and possibility of detection of their presence using specific fluorescence spectra from tissue area, illuminated by laser radiation5'6'7. 466

Каприн) Минздрава России, Москва, Россия Высокий биологический потенциал злокачественности мелкоклеточного рака легкого (МРЛ) долгое время обусловливал использование отдельной клинической классификации и неоправданность хирургического лечения больных. Частота МРЛ не превышает 20% в структуре опухолей данной локализации и имеет тенденцию к снижению за последние десятилетия. Многие десятилетия идет интенсивное изучение возможности и роли хирургического метода в лечении этой категории больных. Проведен анализ более 60 научных публикаций за 1980-2016 гг., объединяющих 6624 оперированных больных МРЛ. Обсуждены особенности хирургического лечения, частота рецидивирования МРЛ при адъювантной лучевой терапии и/или полихимиотерапии. Средняя общая 5-летняя выживаемость больных МРЛ при резектабельных стадиях составляет 29,1%. Гетерогенность оперированных больных как по распространенности опухоли, так и коморбидному статусу влияет на разброс показателей (от 9 до 50%) отдаленных результатов лечения. Ученые единодушны во мнении о высокой прогностической значимости стадии заболевания, особенно состояния регионарных лимфатических узлов. Лучшие в среднем показатели 5-летней выживаемости достигнуты у больных МРЛ I стадии (51,7%), удовлетворительные при II (31,5%) и более низкие при IIIA (16,9%). Накопленный мировой опыт (по публикациям ученых Англии, Белоруссии, Германии, Китая, Кореи, Норвегии, России, США, Японии) хирургического компонента комбинированного лечения больных МРЛ позволил сформулировать небесспорные клинические рекомендации в отношении показаний к операции при данном морфологическом типе рака. Выделено несколько преимуществ хирургической тактики лечения: а) относительно высокие отдаленные результаты лечения по отношению к таковым при консервативной терапии; б) возможность выявления диморфных опухолей и окончательная морфологическая дифференциальная диагностика с карциноидом; в) оценка истинной распространенности процесса по лимфатическому коллектору и степени лечебного патоморфоза в опухоли на фоне неоадъювантной терапии; г) более надежный локорегионарный контроль.
