This is the first study assessing severe incidents and complications from a national outcome-tracking database. Annual identification and review of cases, perhaps with standardized database queries in the respective departments, might provide more detailed information about the cascades that lead to unfortunate outcomes.
SummaryThe use of extracorporeal membrane oxygenation in adults has increased in popularity and importance for the support of patients with cardiac or pulmonary failure. Although it is now quite commonly used in the intensive care unit, its use has rarely been described as a means of support during anaesthesia and surgery. We report the case of a patient who required curative resection of the oesophagus following previous left pneumonectomy where veno-venous extracorporeal membrane oxygenation was required both during surgery and for the first three days postoperatively. We describe the anaesthetic management of this patient who only had a single lung, review other alternatives and discuss why extracorporeal membrane oxygenation was particularly suited to this case. To the best of our knowledge, the anaesthetic literature to date does not contain a case report of this type. Case reportWe report the case of a 66-year-old male patient for whom oesophageal resection was planned. His clinical symptoms included mild dysphagia and weight loss of about 10 kg. The patient had undergone leftsided pneumonectomy 12 years previously, followed by chemotherapy and radiotherapy, for non-small lung cancer. His medical history also included left recurrent nerve palsy and mild fibrosis of the lower lobe of the right lung. Oesophagoscopy showed an exophytic ulcerating lesion extending from 24 to 26 cm; histology and endosonography confirmed a pT1N0M0 squamous cell carcinoma. The interdisciplinary tumour board conference agreed that only a radical oesophagectomy would provide a curative approach and the patient was referred for an anaesthetic assessment. The surgeons planned to perform the procedure in two stages: resection of the tumour via a right-sided thoracotomy with at least partial collapse of the only remaining lung, followed by extra-anatomical reconstruction in a separate operation.Pre-operative investigations included a normal echocardiographic study and stress-spirometry which revealed mildly reduced peak oxygen uptake and exercise capability. Hintergrund Stark ansteigende COVID-19-Erkrankungen drohten im Herbst 2020 zu einer bedrohlichen lokalen Überlastung von Intensivstationen zu führen. Um dies zu verhindern, wurde in Baden-Württemberg ein landesweites Verlegungskonzept entwickelt. Methoden Basierend auf von Robert Koch-Institut (RKI) und Deutscher Interdisziplinärer Vereinigung für Intensiv- und Notfallmedizin (DIVI) veröffentlichten Zahlen zum Infektionsgeschehens wurde das Verlegungskonzept entwickelt. Es sind 6 Versorgungsregionen (Cluster) definiert, die anhand der Anzahl der Intensivbetten pro 100.000 Einwohner um ein Zentrum der maximalen Intensivversorgung (ECMO-Möglichkeit) zur Darstellung kommen. Führungsstruktur, strukturelles Organigramm, Aufgaben- und Organisationsbeschreibung sowie die Clusterzuordnung der jeweiligen Kliniken wurden definiert. Die Intensivverlegungen innerhalb und zwischen den Clustern wurden vom 11.12.2020 bis 07.02.2021 erfasst. Ergebnisse In Deutschland und Baden-Württemberg benötigten Mitte Dezember 2020 1,5 % der mit SARS-CoV‑2 neu infizierten Patienten eine intensivmedizinische Behandlung. Bei einer deutschlandweiten 7‑Tage-Inzidenz von 191 Neuinfektionen betrug die Hospitalisierungsrate 10 %, und es waren 28–35 % der Intensivbetten in Deutschland mit COVID-19-Patienten belegt. Nur noch 16,8 % der Intensivbetten waren frei. Zwischen den Clustern wurden im Median 12,5 ± 1/14 Patienten verlegt. In andere Bundesländer wurden 21 Intensivpatienten verlegt und 21 Intensivpatienten aus anderen Bundesländern aufgenommen. Insgesamt betrug die Zahl der transferierten Intensivpatienten 261. Schlussfolgerung Bei steigenden Infektionszahlen mit SARS-CoV‑2 soll frühzeitig ein landesweites Verlegungskonzept für COVID-19-Intensivpatienten und Non-COVID-19-Intensivpatienten installiert werden, um die Kapazitäten besonders stark betroffener Kliniken nicht zu überfordern. Versorgungsregionen um eine führende Klinik mit maximaler Intensivmöglichkeit sind zu definieren, die in Zusammenarbeit mit regionalen und überregionalen rettungsdienstlichen Leitstellen notwendige Verlegungen organisiert. Mit diesem Konzept und den durchgeführten Intensivtransporten konnte in Baden-Württemberg effektiv die Überlastung einzelner Kliniken durch COVID-19-Patienten verhindert werden.
Transfusion of homologous blood may be associated with various complications. In this review, the authors present concepts of prevention of homologous blood transfusion. Different autotransfusion techniques are discussed, i.e., predeposit of autologous blood, plasmapheresis, normovolaemic haemodilution, blood salvage with/without cell washing, postoperative blood salvage of drainage blood, anaesthesia and surgical techniques as well as drugs for minimizing blood loss. The third part of the essay deals with indications of each technique depending on the surgical field. The authors conclude with unanswered questions and problems of the future.
