The first part of the article describes a rare case of the onset and course of juvenile dermatomyositis. In the second part the comparative clinical analysis of results of own supervision with other similar reports is carried out. The main attention is focused on atypical variants of the onset of the disease, in particular signs of severe intoxication syndrome, hemorrhagic-necrotic rash, anasarca, lesions of the oral cavity in the form of stomatitis and ulcerative-necrotic glossitis. The difficulties of early diagnosis of the disease on the background of delayed manifestation of pathognomonic skin signs and the absence of reliable features of myopathic syndrome are shown. Clinical and laboratory characteristics of amyopathic juvenile dermatomyositis with persistent skin lesions have been defined. There are three differentiated plans-recommendations of the Childhood Arthritis and Rheumatology Research Alliance for the treatment of patients who have never had muscle damage and those who have had clinical manifestations of myopathy only in the first months of the disease. Current trends in achievements and gaps in the classification, diagnosis and treatment of various forms of juvenile dermatomyositis have been observed. On the basis of scientific publications the possible prognostic consequences of the amyopathic form of this disease in children are highlighted. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interest was declared by the authors. В історичній послідовності викладено основні етапи розвитку дитячої ревматології з особливим ухилом на передумови виникнення терміну «ювенільний ревматоїдний артрит». Зазначено причини виокремлення цього захворювання з аналогічної патології суглобів у дорослих. Показано еволюцію номенклатури ювенільного ревматоїдного артриту, починаючи з 1946 року і до наших днів. Окремо розглядаються причини введення терміну «ювенільний ідіопатичний артрит». Викладено класифікаційні та термінологічні розбіжності щодо патології у різних країнах світу. Показано внесок визначних вчених та міжнародних організацій у вивчення цієї проблеми. Звертається увага на проблемні питання трактування номенклатури хронічних захворювань суглобів дітей в Україні і світі. Зроблено припущення, що вже найближчим часом буде запропоновано нову номенклатурну та класифікаційну платформу ювенільних артритів. Автори заявляють про відсутність конфлікту інтересів. The main problems that arise in children operated on for Hirschsprung's disease (HD) or anorectal malformations (ARM) in the long term are persistent defecation disorders manifested in the form of constipation or fecal incontinence (FI), as well as their combination. Violation of the act of defecation, regardless of its cause, contributes to the formation of a number of complex psychological, medical and social problems that affect the moral and physical condition of children, resulting in reduced quality of life (QoL). The purpose - to study QoL in children operated on in the clinic for congenital colorectal pathology and to evaluate the objectivity of the pathology-specific QoL questionnaire. Materials and methods. Clinical examination and treatment of 92 children with HD and ARM were performed. Questionnaires to determine the level of QoL were conducted in 79 operated children 6-17 years, including 51 (64.56%) children with HD and 28 (35.44%) patients with ARM. These patients formed the main group. Questionnaires were also conducted in 30 somatically healthy children without colorectal pathology of the same age. They formed a control group. Children who had postoperative complications were additionally interviewed before and after treatment of these complications (conservative or surgical). We selected the Hirschsprung’s disease Anorectal malformation QoL questionnaire (HAQL). We translated the HAQL questionnaire into Ukrainian and adapted it to the social, educational and cultural characteristics of Ukrainian people. We also modified it, simplified the issues a bit and integrated it into one form for better comparison of QoL between different groups of patients. Results. We identified 4 degrees of QoL violation in children of the main group: mild - with a sum of 74-105, moderate - with a score of 38-73, severe - with a score of 10-36, very severe - 0-9 points. In 19 children (24.05%) QoL was not violated, the average score was 108.4±2.5. The majority (39 patients, 49.37%) had a mild degree of QoL violation, the mid degree was in 17 (21.52%) children, severe QoL violation was found in 4 (5.06%) patients. There were no children with very severe QoL disorders. In children, the level of QoL was slightly higher after correction of ARM. The severity of QoL disturbance depended on the form of congenital colorectal pathology and the presence of pre- and postoperative complications. Among children with ARM, more severe QoL disorders were observed in high forms, among patients with HD more severe disorders were found in patients with a long zone of agangliosis, with acute and subacute forms of the disease and in children with serious preoperative complications. There were no significant gender differences in the violation of QoL in children of the main group. The level of QoL was higher in children aged 12-17 years than in patients aged 6-11 years. In older children, a higher average score was observed in the sections of the questionnaire related to defecation control. At the same time, children in this age group have lower scores in the sections on emotional and social functioning and body sensation. When re-interviewing 45 (56.96%) patients who had distant postoperative complications and defecation disorders, after conservative or surgical treatment, the level of QoL was increased in all cases. Conclusions. QoL disorders of varying severity were found in 60 (75.95%) of children operated on for HD and ARM. The majority of patients (39 children, 49.37%) had a mild degree, and 4 (5.06%) of children had a severe degree of QoL violation. The proposed HG and ARM-specific child quality of life questionnaire is a convenient and objective tool for assessing the quality of life of operated children. It allows you to assess not only postoperative functional outcomes, but also psychosocial and emotional aspects of the lives of operated children. It also allows you to compare the results of treatment in different clinics and makes these comparative results transparent. Описано випадок дебюту та перебігу рідкісного ювенільного дерматоміозиту (ЮДМ). Особливість цього випадку полягає в тому, що хвороба розпочалася з домінування ознак вираженої інтоксикації, виразково некротичного ураження язика та анасарки. Типові шкірні клінічні прояви хвороби були відстрочені на один місяць. Лабораторне дослідження виявило підвищення рівня показників гострої фази запального процесу та титрів антинуклеарних антитіл, а серед ферментів, специфічних для міопатичного синдрому, позитивною виявилася тільки збільшена активність аланінамінотрансферази. Електроміограма будь яких порушень не зафіксувала. Через два роки констатовано коморбідність хвороби з простою формою псоріазу. Тривале (семирічне) спостереження дозволило констатувати хронічний аміопатичний перебіг хвороби. Показано, що використання стандартного лікування метилпреднізолоном, метотрексатом та плаквенілом в такому випадку виявилось недостатньо ефективним.. У другій частині статті буде наведено огляд літератури стосовно інших нетипових варіантів прояву та перебігу ЮДМ та здійснено порівняльний клінічний аналіз цього клінічного випадку з іншими подібними спостереженнями. Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом зазначеної у роботі установи. Актуальность. Инфекция Helicobacter pylori является важным фактором хронического гастрита (ХГ) и дуоденальной язвы у детей. За последнее десятилетие установлена разная вирулентность штаммов H.pylori, которая зависит от его генов токсигенности. Цель: изучить клинические проявления, эндоскопические и гистологические изменения слизистой оболочки желудка (СОЖ) у детей с хроническим гастритом, индуцированным H.pylori, с разной его токсигенностью. Материалы и методы. Проведено клинико-инструментальное обследование 136 детей с хроническим гастритом, индуцированным H.pylori, в возрасте 7–17 лет (основная группа) и 30 здоровых детей этого же возраста (контрольная группа). Пациенты были разделены на 2 группы: IА группу составили 72 ребенка с хроническим гастритом, индуцированным H.pylori CagA «+»; IБ группу — 64 ребенка с хроническим гастритом, индуцированным H.pylori CagA «–», по результатам определения суммарных антител IgG к антигену CagA сыворотки крови и/или определения антигена CagA в биоптатах СОЖ методом полимеразной цепной реакции. Изучали и анализировали клинические проявления, эндоскопические и гистологические изменения слизистой оболочки желудка. Статистическая обработка проводилась с помощью пакета статистических программ Statgrafics 16.0. Результаты. У детей с ХГ, индуцированным H.pylori CagA «+» (IА группа), клинические проявления болевого абдоминального и диспептического синдромов существенно не отличаются от таковых у детей с ХГ, индуцированным H.pylori CagA «–» (ІБ группа). Не выявлены существенные эндоскопические изменения в виде эритематозной гастропатии и антральной нодулярности как у детей с ХГ, индуцированным H.pylori CagA «+», так и у детей с ХГ, индуцированным H.pylori CagA «–». Гистологические изменения в ІА и ІБ группах были разные: детям с ХГ, индуцированным H.pylori CagA «+», свойственны II и I степень активности воспаления и ІІ, І, ІІІ степени обсемененности Н.pylori слизистой оболочки желудка, а детям с ХГ, индуцированным H.pylori CagA «–», свойственны I, II степень активности воспаления слизистой оболочки желудка и II и I степень обсемененности Н.pylori. Выводы. Клиническими проявлениями ХГ, индуцированного H.pylori, у детей в стадии обострения является эпигастральная боль, симптомы диспепсии и хронического интоксикационного синдрома. Эндоскопическими проявлениями являются эритематозная гастропатия и/или антральная нодулярнисть. У детей с ХГ, индуцированным H.pylori СagA «+», и у детей с ХГ, индуцированным H.pylori СagA «–», по клиническим и эндоскопическим проявлениям существенного различия не выявлено. Гистологическим изменением СОШ при ХГ, индуцированном H.pylori, в стадии обострения является диффузная лимфоцитарно-плазмоцитарная инфильтрация собственной пластинки СОЖ с нарушением эпителия. При ХГ, индуцированным H.pylori СagA «+», преобладает II и III степень обсеменения H.pylori в сочетании с I и II степенью активности воспаления СОЖ. При ХГ, индуцированном H.pylori СagA «–», преобладает II и I степень обсеменения H.pylori СОЖ. У детей с ХГ, индуцированным H.pylori, целесообразно диагностировать его токсигенность CagA «+» для оценки H.pylori CagA-статуса ребенка.
