Introduction: Combined endocrine pathology is a serious healthcare problem in Ukraine. This prospective study assessed the blood levels of 25-hydroxyvitamin D [25(OH)D] and markers of immune function in response to vitamin D intervention in patients with type 1 and type 2 diabetes mellitus (T1DM and T2DM, respectively) and autoimmune thyroiditis (AIT).Objective: This study evaluated the relationship between the metabolic and immune status of DM + AIT patients with respect to their vitamin D status and changes after vitamin D3 supplementation.Material and Methods: Patients with type 1 or type 2 DM in combination with AIT and decreased circulating levels of 25(OH)D were divided into two groups of 30 patients each. All patients with AIT were euthyroid and receiving hormonal replacement therapy. The levels of carbohydrate and fat metabolism markers, Immunologic markers, namely, Th1-type cytokines [interferon (IFN)-γ, tumor necrosis factor (TNF)-α, interleukin (IL)-2, IL-6, IL-12], Th2-type cytokines (IL-4, IL-5), IL-10, and IL-17 were measured before and after vitamin D3 supplementation. The vitamin D status was assessed according to the level of 25(OH)D.Results: Patients with combined endocrine disorders (DM and AIT) with a decreased vitamin D status had significantly increased background concentrations of Th1-type cytokines and reduced concentrations of Th2-type cytokines (IL-4 and IL-5), IL-10, and IL-17. The results of our study showed that vitamin D3 supplementation in patients with T1DM and T2DM reduced the concentrations of the inflammatory Th1-type cytokines and increased the levels of Th2-type cytokines.Conclusion: The presence of two endocrine diseases, aggravated by decreased circulating levels of 25(OH)D, leads to disorders wherein the immune status is markedly changed. These decreased levels of 25(OH)D contribute to an autoimmune inflammatory process and to the progression of complications in addition to the metabolic disorders. A vitamin D intervention resulted in significant changes in the blood levels of 25(OH)D that are related to parameters of autoimmunity and glucose metabolism. Vitamin D3 supplementation should be considered for the prevention and treatment of combined endocrine pathology.

Актуальність. Довготривале введення глюкокортикоїдів (ГК) супроводжується порушенням мінерального обміну і, як наслідок, призводить до розвитку вторинного остеопорозу. Важливу роль у регулюванні процесу ремоделювання кісткової тканини відіграє вітамін D, який може реалізовувати свої біологічні ефекти через рецептор вітаміну D — VDR-опосередкований вплив на цитокінові системи, зокрема RANKL-RANK-OPG (ліганд рецептора активатора ядерного фактора транскрипції NF-κB — рецептор RANKL — остеопротегерин). Мета. Дослідження ГК-індукованих змін експресії детермінантних для процесів формування та резорбції кісткової тканини регуляторних протеїнів (RANK, RANKL, OPG, остеокальцин, VDR) залежно від біодоступності вітаміну D. Матеріали та методи. Щури-самиці лінії Wistar отримували преднізолон (5 мг/кг маси тіла) разом з вітаміном D3 (100 МО) або без нього протягом 30 днів. Рівні VDR, остеокальцину, RANK, RANKL і OPG у кістковій тканині визначали за методом вестерн-блот-аналізу. Рівень гідроксивітаміну D (25OHD) у сироватці крові аналізували, використовуючи метод ELISA. Вміст мінеральних компонентів (Ca2+ та Pi), активність лужної фосфатази (ЛФ) та її ізоформ визначали спектрофотометрично. Результати. Преднізолон значно знижував рівень 25OHD у сироватці крові та вміст протеїну VDR у кістковій тканині. Це супроводжувалось підвищенням загальної активності сироваткової ЛФ і її кісткової ізоформи, гіпокальціємією, гіпофосфатемією, а також зниженням вмісту мінеральних компонентів у кістковій тканині. Спостерігалось значне зниження експресії остеокальцину, ключового маркера остеосинтезу. Зміни у D-вітамінному статусі тварин, викликані ГК, призводили до зниження рівня RANK та OPG у кістковій тканині, тоді як рівень RANKL залишався без змін. Уведення вітаміну D3, підвищуючи рівні 25OHD і VDR, сприяло частковій або повній нормалізації викликаних преднізолоном змін мінерального обміну й кісткового гомеостазу через підвищення рівнів RANK, OPG та остеокальцину. Висновки. Преднізолон-індуковане дерегулювання цитокінової системи RANKL — RANK — OPG та остеокальцин-опосередковане формування кісткової тканини можуть бути тісно пов’язані зі зниженням біодоступності вітаміну D і порушенням клітинного сигналювання через VDR. Відновлення біодоступності вітаміну D є необхідною умовою, що протидіє негативним ефектам глюкокортикоїдної терапії у кістковій тканині.

Resume A retrospective analysis of the screening results was carried out (970 subjects of the cohort underwent an in-depth (4-fold) examination (at each visit: ultrasound examination of the thyroid gland, determination of the levels of thyroid-stimulating hormone (TSH) in the blood serum and antibodies to thyroid peroxidase (TPOAb), examination by an endocrinologist) for thyroid diseases. Medical examinations (visits) were carried out every 2 years.It was noted that within the cohort subjects, the most common clinical and laboratory variant of AIT was subclinical with preserved size and function of the thyroid gland or its transient decrease. The frequency of occurrence of the atrophic form of AIT is minimal (only 2/71 patients had a thyroid volume less than 6 ml) as well as the hypertrophic form (in 5/71 cases of AIT the thyroid volume exceeded 24 ml).Analysis of the combination of laboratory and instrumental criteria showed the dependence of the diagnosis of AIT on one of two examinations: Ultrasound (decreased echogenicity) or increased TPOAb. The proportion of each criterion was almost equal (echogenicity was decreased in 10 cases – 14,1%; TPOAb was increased in 16 cases – 22,5%). In most cases (43 cases – 60,6%) there was a combination of two criteria (decreased echogenicity and increased TPOAb). In one case (1,4%), against the background of an increased ratio of antibodies to thyroid peroxidase, an increased level of TSH was noted; ultrasonographic signs were normal. In the second case, against the background of decreased echogenicity of the thyroid gland and low TSH levels, ratio of antibodies to thyroid peroxidase is increased. In all cases, the patients did not report any clinical symptoms, i.e. it manifested as a subclinical form. The TSH level (in 70/71 cases) was within the normal range (0,3-4,0 mIU/l), but its level was significantly higher in patients with AIT (2,4±0,12 mIU/l) than in healthy individuals (TPOAb – normal and thyroid echogenicity – normal) 1,6±0,02 mIU/l; p<0,001.The simultaneous presence of high concentrations of TPOAb in the blood serum and sono -graphic criteria of AIT, including decreased echogenicity of the thyroid gland, increases after 2 years the risk of primary hypothyroidism (PHT) by 12,8 (5,19-31,61) times (p<0,001), in the case of TPOAb content in the blood serum – 29,9 IU/ml and higher, 9,4 (3,86-23,11) times – when the concentration of these autoantibodies TPOAb is more than 60 IU/ml.
