diagnosis of sacroiliac joint pain, and for transforaminal epidural injections in the preoperative evaluation of patients with negative or inconclusive imaging studies, but with clinical fi ndings of nerve root irritation.Moderate to strong evidence was shown for multiple therapeutic interventional techniques including medial branch blocks and medial branch neurotomy; caudal epidural steroid injections and transforaminal epidural steroid injections; lumbar percutaneous adhesiolysis; and implantable therapies.These guidelines do not constitute infl exible treatment recommendations. It is expected that a provider will establish a plan of care on a case-by-case basis, taking into account an individual patient's medical condition, personal needs, and preferences, and the physician's experience. Based on an individual patient's needs, treatment different from that outlined here could be warranted. These guidelines do not represent "standard of care."

“…Translaminar facet screw fixation was also used and was thought to have better fixation properties than direct facet screws because it fixed a longer segment of bone including the lamina and the facet joint. 2,4,9,25,28 Percutaneous facet screw fixation system was also invented, however, it is expensive, uses the C-arm guidance and only used for cases of instability not requiring discectomy or foraminotomy. 12,15 In this study, we evaluate the role of bilateral facet screw fixation together with posterior lumbar interbody fusion (PLIF) using autologous bone graft as an easy and cheap means for reducing or preventing postoperative instability and recurrence of disc prolapse after lumbar discectomy.…”

Background Data: Lumbar disc prolapse is a common neurosurgical diagnosis. A trial of medical treatment, bed rest and physiotherapy is tried at first. Surgical treatment is resorted to if conservative treatment fails. However, postoperative complications including recurrent lumbar disc prolapse, postlaminectomy spondylolisthesis and failed back syndrome can occur. This is usually caused by instability at the discectomy level. In a trial to reduce these complications, we will use posterior lumbar interbody fusion together with bilateral facet screw fixation implanted during discectomy to reduce postoperative instability. Purpose: To study the effect of posterior lumbar interbody fusion with bilateral facet screw fixation to decrease the incidence of postoperative complications. Study Design: A prospective randomized controlled trial including 40 patients. Patients and Methods: Forty patients diagnosed with lumbar disc prolapse causing sciatic pain with or without lower limb weakness not responding to conservative treatment were included in the study. Twenty patients (group A) was submitted to PLIF together with bilateral facet screw fixation after having conventional lumbar laminectomy and discectomy, while 20 other patients (group B) was submitted only to conventional lumbar laminectomy and discectomy and will be used as controls.

Chronic low back pain secondary to involvement of the facet joints is a common problem. Facet joints have been recognized as potential sources of back pain since 1911. Multiple authors have described distributions of pain patterns of facet joint pain. The facet joints are paired diarthrodial articulations between the posterior elements of the adjacent vertebrae. Lumbar facet joints are innervated by medial branches of the dorsal rami of the spinal nerves from the L1 to L4 levels. At L5, the dorsal ramus travels between the ala of the sacrum and its superior articular process and divides into medial and lateral branches at the caudal edge of the process. Each segmental medial branch of the dorsal ramus supplies at least two facet joints.The existence of lumbar facet joint pain claims has a preponderance of evidence, even though there are a few detractors. Multiple studies utilizing controlled diagnostic blocks have established the prevalence of lumbar facet joint in-volvement in patients with chronic low back pain, as ranging from 15% to 52%, based on type of population and setting studied.Long-term therapeutic benefit has been reported from three types of interventions in managing lumbar facet joint pain, including intraarticular injections, medial branch blocks and neurolysis of medial branches. This review will discuss chronic low back of facet joint origin and covers anatomy, pathophysiology, diagnosis, and various aspects related to treatment, including clinical effectiveness, cost effectiveness, technical aspects and complications.
