Xanthogranulomatous Salpingitis and Oophoritis

Abstract: A case of xanthogranulomatous salpingitis and oophoritis in a 47-year-old woman is presented. Xanthogranulomatous inflammation is an uncommon form of chronic inflammation that is destructive to affected organs; it is characterized by the presence of lipid-filled macrophages with admixed lymphocytes, plasma cells, and neutrophils. Only a few cases of xanthogranulomatous salpingitis and oophoritis have been reported to date. The case presented here is associated with Escherichia coli infection, endometriosis, an… Show more

“…However, it is rarely seen in the female genital tract [ 6 ]. In the female genital tract, it has been reported involving the endometrium [ 6 , 10 , 11 ], fallopian tubes [ 6 , 12 , 13 ], and ovary [ 6 , 14 ], primarily presenting as endometritis and/or salpingitis, with a tubo-ovarian abscess [ 8 ]. However, xanthogranulomatous inflammation with uterine myometritis, without endometritis has rarely been reported in the literature [ 8 ].…”

“…The etiology of this disease is not clear yet, but the infection theory (pelvic inflammatory disease) is one of the most accepted theories since, in many cases, the presence of certain pathogenic microorganisms such as Escherichia coli , Bacteroides fragilis , Proteus , and Salmonella has been demonstrated [ 9 , 10 ]. Nonetheless, some cases have been reported as without being related to infectious processes [ 11 ].…”

The antibiotic therapy alone is not sufficient and operative treatment is necessary in the form of salpingo-oophorectomy, depending on the site of lesion.…”

“…The microscopic examination shows the presence of foamy histiocytes, along with a mixture of inflammatory cells like lymphocytes, plasma cells, and occasional neutrophils. 21 Foamy histiocytes, the so-called “xanthoma cells,” are histiocytes with abundant lipid-laden cytoplasm having a vacuolated appearance. These impart the yellowish color to the specimen, when seen on the gross examination.…”
