Wound Care in Immunobullous Disease

Abstract: The chapter introduces the steps to achieving proper wound care in immunobullous disease. It describes the clinical characteristics and nature of "wounds" formed in pemphigus versus pemphigoid diseases. Namely, pemphigus diseases typically result in acantholysis in the epidermis and the formation of flaccid blisters. In contrast, bullous pemphigoid presents with basal keratinocyte hemidesmosomes in the dermoepidermal junction, which results in a split at the dermoepidermal junction and clinically forms tense b… Show more

“…If the lesion is not treated, the lesion will not heal and will increase in size. 2,8,15 Platelet-rich plasma (PRP) is an endogenous therapeutic technology that has the potential to stimulate and accelerate wound healing. PRP is defined as an autologous biological product derived from the patient's blood where after the centrifugation process, a blood fraction with a higher platelet count is obtained than the circulating blood.…”

“…Several wound treatments play a role in cellular and molecular mechanisms in the wound microenvironment. 2 Platelet-rich plasma (PRP) containing a variety of different growth factors can be used in wound care. It is extracted from whole blood using a series of centrifugation processes and, when activated, releases several antimicrobial microproteins, cytokines, and growth factors.…”

“…In addition to the more controversial therapeutic roles that systemic steroids, intravenous immunoglobulins, and plasmapheresis may play, the mainstay clinical TEN management is excellent "burn care," ideally in a burn center with careful attention to pain management, electrolyte balance, topical disinfection, access to burn beds and nonadherent dressings, and prompt treatment of secondary infections. An ophthalmologic consultation is also required because of the risk of corneal erosions and scarring [218].…”

“…Topical steroids are widely used in treatment of all clinical symptoms of OMD, with a prime aim of reducing pain and inflammation. These are generally applied topically, and include 0.1% triamcinolone acetonide in orabase, 0.05% fluocinolone acetonide, 0.05% clobetasol propionate, or 0.05% halobetasol, A further approach has been intralesional injection of triamcinolone acetonide (20 μg/L) [ 96 ]. They often used as a first line drug with rare side effects and low cost [ 97 ].…”
